Community pharmacy services have evolved to include medical and pharmaceutical interventions alongside dispensing. While established pharmacogenomic (PGx) testing is available throughout the Netherlands, this is primarily based in hospital environments and for specialist medicines. The aim of this work was to describe how best to implement PGx services within community pharmacy, considering potential barriers and enablers to service delivery and how to address them. The service was implemented across a selection of community pharmacies in the Netherlands. Data were captured on test outcomes and through a pharmacist survey. Following testing, 17.8% of the clinical samples were recommended to avoid certain medication (based on their current medicines use), and 14.0% to have their dose adjusted. Pre-emptive analysis of genotyped patients showed that the majority (99.2%) had actionable variants. Pharmacists felt confident in their operational knowledge to deliver the service, but less so in applying that knowledge. Delivering the service was believed to improve relationships with other healthcare professionals. These results add to the evidence in understanding how PGx can be delivered effectively within the community pharmacy environment. Training pharmacists in how to respond to patient queries and make clinical recommendations may enhance service provision further.

AbstractIn periodontal practice microbial results of periodontal test kits for identification of key pathogens are an aid in the treatment planning. Information on the performance of commercially available test kits is therefore essential for the clinician. In this retrospective analysis three commercially available qPCR kits for detection and quantification of selected periodontal bacterial species were compared, using 100 clinical samples from patients with untreated periodontitis. The analysis involved two separate comparisons in which kit A (LabOral Diagnostics, The Netherlands) was compared with kit B (Advanced Dental Diagnostics, The Netherlands), and with kit C (OralDent diagnostics, The Netherlands). Analytic procedures for detection and quantification of selected periodontal bacterial species were carried out according to the instructions of the laboratories. Kit A detected target species more often, and absolute numbers of bacterial cells were higher than with kit B. A high degree of similarity was found between the test outcomes by kit A and kit C. All three kits performed satisfactory but small and significant differences exist between kits.

The hypothesis that the sex composition of an applicant pool affects the hiring probabilities of individual job applicants was tested using gender-distinctive information on accepted and rejected job applicants in The Netherlands. The evidence supports this hypothesis, although the effect sizes are moderate. Both men and women have a lower probability of being hired when the applicant pool contains fewer applicants from their own sex.

Abstract Background Although human longevity tends to cluster within families, genetic studies on longevity have had limited success in identifying longevity loci. One of the main causes of this limited success is the selection of participants. Studies generally include sporadically long-lived individuals, i.e. individuals with the longevity phenotype but without a genetic predisposition for longevity. The inclusion of these individuals causes phenotype heterogeneity which results in power reduction and bias. A way to avoid sporadically long-lived individuals and reduce sample heterogeneity is to include family history of longevity as selection criterion using a longevity family score. A main challenge when developing family scores are the large differences in family size, because of real differences in sibship sizes or because of missing data. Methods We discussed the statistical properties of two existing longevity family scores: the Family Longevity Selection Score (FLoSS) and the Longevity Relatives Count (LRC) score and we evaluated their performance dealing with differential family size. We proposed a new longevity family score, the mLRC score, an extension of the LRC based on random effects modeling, which is robust for family size and missing values. The performance of the new mLRC as selection tool was evaluated in an intensive simulation study and illustrated in a large real dataset, the Historical Sample of the Netherlands (HSN). Results Empirical scores such as the FLOSS and LRC cannot properly deal with differential family size and missing data. Our simulation study showed that mLRC is not affected by family size and provides more accurate selections of long-lived families. The analysis of 1105 sibships of the Historical Sample of the Netherlands showed that the selection of long-lived individuals based on the mLRC score predicts excess survival in the validation set better than the selection based on the LRC score . Conclusions Model-based score systems such as the mLRC score help to reduce heterogeneity in the selection of long-lived families. The power of future studies into the genetics of longevity can likely be improved and their bias reduced, by selecting long-lived cases using the mLRC.

In recent decades, so-called “mini-publics” have been organized in many countries to renew policy making and democracy. One characteristic of mini-publics is that the selection of the participants is based on random sampling or sortition. This gives each member of the community an equal chance of being selected. Another feature is that deliberation forms the core of the process of how proposals are developed. In this paper, we investigate the possibilities and challenges of sortition and deliberation in the context of the call for a deepening of democracy and more citizen engagement in policy making. Based on extensive research on citizens’ forums (G1000) in The Netherlands, we show the potential of mini-publics, but a number of shortcomings as well. Some of these are related to the specific design of the G1000, while others are of a more fundamental nature and are due to the contradictory democratic values that deliberative mini-publics try to combine. One of these concerns the tension between the quality of deliberation and political impact. We conclude that combining institutional approaches could be a way out to deal with these tensions and a step forward to both deepen and connect democratic processes.

BIOBREES is a co-operative scientific programme between Indonesia and The Netherlands, of which the rose part aims at breeding cut roses adapted to the tropical highland. Within that scope four direct plant characters of 62 cut rose genotypes in different physiological stages were studied. The genotypes originated from cross-breeding in 1995 and successive selection for cut rose properties in populations at Plant Research International, Wageningen, The Netherlands, in 1996. Plant stages stuAn died were (i) superior adult seedlings in Wageningen in 1996, (ii) clones grafted onto Natal Briar both in Wageningen and Cipanas in 1997. Clonal Plant Research International, Wageningen, Plant Research International, Wageningen, clonal plants in Cipanas had significantly shorter shoots, more thorns, smaller flowers and fewer petals than the adult seedlings in Wageningen. Clonal plants in Wageningen, however, had significantly longer shoots; more thorns, and larger flowers with more petals than the seedlings in Wageningen. Despite differences in level of expression, for each character the absence of genotype-location interaction was ascertained. The expression of characters as influenced by light (both quantitative and qualitative), temperature and cultivation is discussed in relation to selection of cut roses in the temperate zone, which are adapted to the tropical highland.

Best value PIPS has been introduced in the Netherlands in the procurement of construction systems. The Water Board De Dommel utilized best value PIPS to select engineering consultant services. The test differed from other Dutch tests in that the PIPS process used the pre-award phase before the award phase. The best value selection of engineers also proposes that design firms can compete on value (price and performance). The engineering selection process was much closer to the original BVP/PIPS than the construction phases. The major lesson learned is the BVP/PIPS is a paradigm shift. The success of the test led to the recommendation of more professional services using BVP/PIPS.

AimsTo assess whether patients have a good knowledge of basic nutrition compared to a group of staff. We hypothesise that the patient's knowledge will show deficits compared to the staff despite the group interventions.BackgroundThe Royal College of Psychiatrist's core standards for inpatient physical health outlines that patients should be engaged in healthy lifestyle groups. The women's secure service at Ardenleigh has developed healthy lifestyles groups to promote a better understanding of nutrition.MethodAn adapted University College London general knowledge nutrition questionnaire was used to investigate nutritional knowledge.All 22 inpatients and a random selection of staff were offered the chance to complete the questionnaire. As the groups run on a regular basis, it was presumed all patients had attended at least one group session. The staff are the comparator group.18 staff responses and 12 inpatient responses were obtained (54.5% response rate for inpatients).ResultNo participant in either group scored 100%. Both groups had a good awareness of what foods they should be eating more and less of. 83.3% of patients were aware that they should be eating breakfast everyday as opposed to 100% of staff.Poor areas of knowledge included knowledge of the number of oily fish servings per week. Staff and patients also performed poorly when estimating their recommended daily salt intake. 1/3 of patients were unable to provide an example of a serving of fruit and vegetables.The knowledge of the structure of the Eat-Well plate was poor in both groups. Only 16% of patients and 22% of staff were aware that starchy foods should make up 1/3 of the Eat-well plate. Knowledge of protein sources was poor. 25% of patients and 16.6% of staff thought that fruit and butter were good sources of proteinFurthermore, only 50% of patients were able to choose the healthiest evening meal choice from a list of 3 options compared to 100% of staff.ConclusionIn conclusion staff had better knowledge of nutrition than patients but knowledge was poor in areas amongst both groups. We conclude that groups should have more focus around practical applications of nutritional knowledge to everyday life.

Powdery mildew-resistant (PMR) cultivars of cucumber (Cucumis sativus L.) grown in greenhouses in the Netherlands during winter show chlorosis on the full-grown leaves of the main stem. The symptoms are yellowing and occasional necrosis between the main veins of the leaves, resembling symptoms of P toxicity. Severity of the chlorosis may vary from one winter to another. Variation is also observed in the severity of the symptoms between cultivars and individual plants of a cultivar. High P nutrition results in an increase of the severity of chlorosis and provides a better discriminating environment for the selection of PMR genotypes that are less susceptible for leaf chlorosis.
