Development of the public health promotion system in the implementation of the functions of the federal state sanitary and epidemiological supervision

The article deals with the current aspects and issues of regulation of the activities of the bodies that carry out the federal state sanitary and epidemiological supervision and their possibilities in the development of the system of strengthening public health in the Russian Federation in the implementation of various directions of the national project «Demography». In accordance with the passport of the specified project, within the framework of the goals of the federal projects included in it, «Promoting the employment of women — creating conditions for preschool education for children under three years of age», «Older generation», «Strengthening public health», «Sport — norm of life», key measures to fulfill the state function in the field of sanitary and epidemiological welfare of the population and protection of consumer rights. In order to solve the set tasks, a review and analysis of regulatory legal acts on the implementation of state policy in the field of people’s conservation was carried out, as well as the powers of the Federal Service for Supervision of Consumer Rights Protection and Human Welfare to protect the constitutional rights of citizens to health in the performance of their functions were studied. As a result of the study, the main directions in the activities of Rospotrebnadzor on participation in solving issues of federal projects included in the national project «Demography» in accordance with the powers and professional competencies of specialists were identified; As the most important way of informing about health risk factors, it is recommended to expand the list of contingents for professional hygienic training as one of the directions of hygienic education.

The collection covers a very wide range of disciplines, including: hygienic sciences, epidemiology, social hygiene and the organization of the state sanitary and Epidemiological service, bacteriology, disinfection, sanitary and hygienic research methods, public health and public health. The publication is addressed to specialists of Federal Service for Supervision of Consumer Rights Protection and Human Welfare - bodies and institutions, health care organizers, employees of specialized scientific and practical organizations; as well as teachers, students, residents and postgraduates of medical universities. Scientific articles are published in the author's editorial office.

According to the federal law «Formation of a system for motivating citizens to a healthy lifestyle, including healthy eating and giving up bad habits» (or «Strengthening public health») which is a part of «Demography» National project, there is a work needed for healthy food’s popularization including the development and realization of unique educational programs dedicated to rational and balanced nutrition. The aim of the investigation is a discussion of approaches to implementation target educational healthy nutrition programs. The scientific and educational centers of healthcare institutions and Federal Service for Supervision of Consumer Rights Protection and Human Welfare scientific organizations took part in developing the academic programs. Eight target programs were designed such as for preschool children, for pregnant and lactating women; for persons over working age; for persons with an increased level of physical activity; for the population working in hazardous and challenging working conditions; for people living in regions with specific environmental factors (micro-and macronutrients insufficiency, climate conditions). The development of interaction between particular departments of regional executive authority that take part in regional programs for implementing federal law «Strengthening public health» ministries or departments of the education and healthcare system is needed for reaching declared goals for the were educating the population on relevant knowledge and skills. The distribution of health nutrition educational program’s materials should be started among pupils in educational organizations considering the children’s physiologically determined learning ability. The projected difficulties can be the next: staff insufficiency in Federal Service for Supervision of Consumer Rights Protection and Human Welfare local authorities and organizations; lack of creativity during the choosing of using methods; the passivity of other ministries and departments; insufficient technical equipment in educational organizations; psychological resistance to innovations.

The article shows the main problems of application of risk assessment methodology in the work of the Office of Service for Supervision of Consumer Rights Protection and Human Welfare in the city of Moscow. There are established priorities on the improvement of the risk assessment methodology, the solution of which as a whole will ensure not only the integrity and objectivity of the results, but also help, eventually, to harmonize risk assessment in our country with the world's leading systems in this area.

The article shows the classification of the supervised objects of the Office of Service for Supervision of Consumer Rights Protection and Human Welfare in the city of Moscow depending on the index of the potential risk for harm to public health in the conditions of the application of the risk-oriented approach. There is considered the possibility of the control and surveillance activities with the redistribution of human resources of the Office of Service for Supervision of Consumer Rights Protection and Human Welfare in the city of Moscow with respect to supervised objects 1 and 2 classes of the potential risk for harm to the health of the population on the risk-oriented approach.

Abstract Until now, systematic surveillance for infectious diseases in refugees is still missing. For that reason, this study aims at illustrating the prevalence of tuberculosis (TB) and notifiable skin diseases in a representative cohort of refugees in Lippe, a district of the federal state of North Rhine-Westphalia, Germany. We collected data on notifiable communicable diseases (TB, scabies, and head lice) from refugees (n = 8946) based on a medical screening after their arrival from October 2014 until May 2016. Data were analysed using descriptive and bivariate methods. Overall, we found 63 of 8764 screened refugees to have an abnormal TB screening result (0.72%). Of 8946 screened refugees, only 51 individuals were found to have scabies (0.57%) and a further 49 were tested positive for head lice (0.55%). The absolute number of TB cases indicate only a moderate public health burden. However, communicable skin diseases can become a problem in crowded holding camps, where they can cause local outbreaks.

The paper examines the opportunities and obstacles to increasing the employment of women with children in Russia. There is a tight correlation between Russia’s lagging behind in the share of working women with children under the age of three and a lack of supply of preschool and childcare institutions. Using quantitative analysis of the Russian regions, we show that the expansion of the supply of preschool education services is associated with an increase in the employment of women, and the cost of introducing additional places in preschool organizations is recouped by additional tax revenues from working women with children in two years. Our cross-country analysis shows that the transition from traditional gender and social roles to more equal ones, the reduction of gender inequality, the encouragement of fathers to take parental leave, and the increased availability of part-time or flexible-schedule employment for women with children could further facilitate the employment of women with children. Our estimates show that an increase of preschool enrollment in Russia to the level of European countries would materialize a sizable economic growth potential: an increase in income per capita would be 3.5%.
