Abstract: Background – Herpes  zoster that is also called with shingles, dampa, or cacar ular is a disease caused by the infection of varicella zoster virus (VZV) and affects the skin and mucosa. This infection is a virus reactivation after primer infection characterized with unilateral radicular pain and vesicular lesion that manifests on  the skin and its innervation. Objctive – The aim of this research is to find out the profile of outpatient diagnosed with  herpes zoster in Dermatovenereology Department RSUP Prof. DR. R. D. Kandou Manado from January to December 2012. Methods – It is a retrospective study that describe several factors related to the subjects, using medical records data from January to December 2012. There were 22 patients evaluated. Result – 2% of 1096 new patients in the period of January to December 2012 were diagnosed as herpes zoster. 73% were 45 – 64 years old, with 54,5% men and 45,4% women, oftalmicus nerve involved counted to 32%, the most common therapy were the combination of Antivirus + Analgesic + Neurovitamin (36,3%). Conclusion – Increasing of the age could be the factor to reactivate the viral infection. The incidence were not affected by sex. Antivirus remain the drug of choice to treat herpes zoster.Key words : herpes zoster – reactivation – virus   Abstrak: Latar Belakang – Herpes zoster atau disebut juga dengan shingles, dampa, cacar ular adalah penyakit yang disebabkan oleh infeksi virus varisela – zoster (VVZ) yang menyerang kulit dan mukosa. Infeksi ini merupakan reaktivasi virus yang terjadi setelah infeksi primer yang ditandai dengan adanya nyeri radikuler unilateral serta timbulnya lesi vesikuler yang terbatas pada dermatom yang dipersarafi. Tujuan Penelitian – Mengetahui profil herpes zoster di poliklinik Kulit dan Kelamin RSUP Prof. Dr. R. D. Kandou Manado periode Januari – Desember 2012. Metode – Penelitian ini bersifat deskriptif retrospektif, yaitu mendeskripsikan beberapa faktor yang berhubungan dengan subjek penelitian yang ada. Data penelitian diambil dari rekam medik periode Januari – Desember 2012. Jumlah sampel penelitian sebanyak 22 orang. Hasil Penelitian – Terdapat 2% penderita herpes zoter dari 1096 pasien periode Januari – Desember 2012. Pada umur 45 – 64 tahun terdapat 73% penderita, laki – laki 54,5% dan perempuan 45,5%, lokasi yang sering terkena adalah oftalmikus 32%, serta kombinasi terapi yang sering digunakan adalah Antivirus + Analgesik + Neuronvitamin 36,3%. Kesimpulan – Peningkatan usia dapat memicu reaktivasi virus. Kejadian herpes zoster tidak dipengaruhi oleh jenis kelamin. Obat yang sering digunakan adalah antivirus.Kata kunci : herpes zoster – reaktivasi – virus

Abstract Herpes zoster is a common acute viral infection caused by reactivation of Varicella-Zoster virus. Acute pain of herpes zoster lowers the quality of life and interferes with day to day activities. We report a case of herpes zoster in a 68 year old, healthy male patient in which the prodromal symptoms started as toothache. This case highlights the importance of a thorough dental history and examination in patients with toothache.

Herpes zoster is a localized, painful, and vesicular rash involving one or adjacent dermatomes caused by varicella-zoster virus reactivation. Herpes zoster presenting as aseptic meningitis is prevalent among elderly population and people with immunocompromised status. However, it is a rare phenomenon in the young immunocompetent adult; hence, we are reporting a case of a herpes zoster infection presenting as aseptic meningitis and dermatological manifestation in a 19-year-old immunocompetent male.

Varicella-zoster virus vaccination is recommended for virtually all young children in the United States, Canada, and several other countries. Varicella vaccine is a live attenuated virus that retains some of its neurotropic properties. Herpes zoster caused by vaccine virus still occurs in immunized children, although the rate is much lower than in children who had wild-type varicella. It was commonly thought that 2 varicella vaccinations would protect children against the most serious complication of meningitis following herpes zoster; however, 2 meningitis cases have already been published. We now report a third case of varicella vaccine meningitis and define risk factors shared by all 3 immunized adolescents. The diagnosis in cerebrospinal fluid in this third case was verified by amplifying and sequencing portions of the viral genome, to document fixed alleles found only in the vaccine strain. Viral antibody was also detected in the cerebrospinal fluid by confocal microscopy. When compared with the other 2 cases, remarkably all 3 were 14 years old when meningitis occurred. All 3 were treated with intravenous acyclovir, with complete recovery. The adolescent in our case report also had recurrent asthma, which was treated with both prednisone tablets and beclomethasone inhaler before onset of meningitis. When the 3 cases were considered together, they suggested that immunity to varicella-zoster virus may be waning sufficiently in some twice-immunized adolescents to make them vulnerable to varicella vaccine virus reactivation and subsequent meningitis. This complication rarely happens in children after wild-type varicella.

Herpes Zoster also known as Shingles is an acute viral infection which is an extremely painful and incapacitating ailment. It results from the reactivation of the varicella zoster virus. The triggering factors for the onset of an attack of Herpes Zoster include some form of immunosuppression. The diagnosis of Herpes Zoster can be made on proper medical history and a thorough clinical examination. Here is the report of a male patient affected by Herpes Zoster infection which followed after extraction of a lower first molar.

Background: Serious adverse drug reactions of disease-modifying drugs in multiple sclerosis (MS) therapy may include enhanced susceptibility to reactivation of neurotropic herpes viruses like varicella-zoster virus (VZV) and the John Cunningham (JC) polyomavirus. Objective: Because symptomatic reactivation of these viruses are rare events, we determined the incidence of rises in anti-VZV IgG antibody levels as a potential marker for enhanced susceptibility to subclinical and symptomatic reactivation of neurotropic viruses. Methods: Anti-VZV IgG levels were measured in paired serum samples taken 6–8 months apart from natalizumab-treated MS patients, healthy blood donors and human immunodeficiency virus (HIV) infected patients. Results: The incidence of significant rises in anti-VZV IgG levels in natalizumab-treated MS patients was 4.26 per 100 person-years, which was significantly higher than in healthy blood donors. Retrospective evaluation of the available medical records of patients with rises of anti-VZV IgG levels did not reveal herpes zoster (i.e. shingles) manifestations. Conclusions: The increased incidence of significant rises of anti-VZV IgG levels in natalizumab-treated MS patients might indicate an association of natalizumab treatment of MS with an elevated risk of a subclinical VZV reactivation and/or reinfection events. Whether this is predictive of an increased risk of herpes zoster or even symptomatic reactivation of other neurotropic viruses remains to be determined in larger prospective studies.

AbstractObjective:Although neurogenic cough is increasingly recognised, its pathophysiology remains obscure. We describe two cases of chronic cough following laryngeal herpes zoster, a rarely described manifestation of varicella-zoster virus reactivation, and suggest that this may be analogous to post-herpetic neuralgia. The same mechanisms may cause both phenomena.Case reports:We describe two cases of chronic cough persisting for more than three months following an acute attack of laryngeal herpes zoster.Conclusion:Neuronal damage by varicella-zoster virus results in irritable nociceptors and deafferentation, mechanisms known to cause post-herpetic neuralgia. When the vagus nerve is affected, as in laryngeal herpes zoster, the result may be a chronic cough. Similar damage may underlie chronic neurogenic cough in other contexts.

INTRODUCTION Varicella zoster virus reactivation can cause localized, painfull and vesicular rashes that usually involves unilateral neural dermatomes. The most important risk factors for the incidence of herpes zoster ,are immunosuppression and age increasing. some studies defined the incidence of herpes zoster as 3.4 cases per 1,000 persons per year in the United Kingdom (1). The epidemiology of herpes zoster is affected by host factors that lead to the reactivation of latent virus. The incidence of herpes zoster increases with advancing age. Most cases of herpes zoster occur in patients who are more than 45 years old. More than 10 cases of herpes zoster per 1,000 persons per year are diagnosed in 75 years (2). Herpes zoster is also more common in patients treated with immunosuppressive drugs, malignant diseases, human immunodeficiency virus infection or in patients with organ transplants .(3, 4, 5, 6,7).

Abstract: Herpes zoster is a skin disease caused by varicella zoster virus infection, a reactivation of varicella zoster virus (VZV) after its primary infection. This disease especially attacks old age patients, and manifests in the skin and mucosa. This study was aimed to obtain the herpes zoster profile in Dermatovenereology Clinic of Prof. Dr. R. D. Kandou Hospital Manado from January to December 2013. This was a retrospective descriptive study. Data were obtained from medical records of new herpes zoster patients from Januari to December 2013. The results showed 28 cases (0.68%) of herpes zoster out of 4.099 new patients, 42.86% of cases were males and 57.14% were females. The majority of cases were 45-64 years old (78.57%) with the location of dermatome in the troracic region (46.43%). All cases were cured with antiviral drugs.Keywords: herpes zoster Abstrak: Herpes zoster adalah penyakit kulit yang disebabkan oleh infeksi virus varisela-zoster, terutama menyerang pasien usia lanjut, dan bermanifestasi pada kulit dan mukosa. Herpes zoster merupakan reaktivasi virus varisela –zoster setelah infeksi primer. Penelitian ini bertujuan untuk mengetahui profil herpes zoster di Poliklinik Kulit dan Kelamin RSUP Prof. Dr. R. D. Kandou Manado periode Januari-Desember 2013. Jenis penelitian ialah deskriptif retrospektif. Data penelitian diambil dari rekam medik periode Januari-Desember 2013. Hasil penelitian menunjukkan terdapat 28 (0,68%) kasus herpes zoster dari 4.099 pasien baru periode Januari-Desember 2013 terdiri dari laki-laki 12 orang (42,86%) dan perempuan 16 orang (57,14%). Mayoritas kasus berusia 45-64 tahun berjumlah 22 kasus (78,57%) dengan lokasi dermatom tersering pada regio torakalis sebanyak 13 kasus (46,43%). Semua kasus diberikan terapi antivirus. Kata kunci: herpes zoster
