Abstract: The cause of breech presentation is unknown, but there are some risk factors besides prematurity such as structural abnormality of the uterus, polyhydramnion, placenta previa, multiparity, uterine myoma, multiple pregnancy, fetal anomalies (anencephaly, hydrocephalus), and previous history of breech presentation. Before the age of 28 weeks, the incidence of breech presentation ranges from 25-30%, and most fetus will turn into a cephalic presentation after 34 weeks of gestation. This study aimed to determine the characteristics of breech delivery.This was a descriptive retrospective study using medical records of Prof. Dr. R. D. Kandou Hospital Manado period 1 January 2014 – 31 December 2014. This study obtained 214 cases of breech delivery out of the total of 3,347 deliveries. Breech delivery was most common in multigravida, at the age of 37-41 weeks The most common type of presentation was incomplete breech presentation and the management of labour was mostly per vaginam (spontaneous Bracht). Birthweights were more common in the range of 2500 - 3999 grams and the newborn babies were not asphyxiated. Complications were rarely found, however, there were 15 cases of infant death. Keywords: breech delivery, breech presentation Abstrak: Penyebab terjadinya presentasi bokong tidak diketahui, tetapi terdapat beberapa faktor risiko selain prematuritas, yaitu abnormalitas struktural uterus, polihidramnion, plasenta previa, multiparitas, mioma uteri, kehamilan multiple, anomali janin (anensefali, hidrosefalus), dan riwayat presentasi bokong sebelumnya.Sebelum umur kehamilan 28 minggu, kejadian presentasi bokong berkisar antara 25-30%, dan sebagian besar akan berubah menjadi presentasi kepala setelah umur kehamilan 34 minggu.Tujuan penelitian ini untuk mengetahui karakteristik dari persalinan letak sungsang. Penelitian ini menggunakan metode deskriptif retrospektif melalui rekam medik di RSUP Prof. dr. R. D. Kandou Manado periode 1 Januari 2014 – 31 Desember 2014. Dari penelitian ini diperoleh 214 kasus persalinan letak sungsang dari total persalinan 3.347 persalinan. Persalinan letak sungsang paling banyak ditemukan pada multigravida, kelompok usia kehamilan 37 – 41 minggu, jenis presentasi bokong kaki (incomplete breech) dengan penanganan paling banyak ialah persalinan pervaginam (spontaneus Bracht). Berat badan lahir bayi letak sungsang paling sering berkisar 2500 – 3999 gram, umumnya bayi tidak mengalami asfiksia. Walaupun jarang ditemukan komplikasi, mortalitas bayi letak sungsang terdapat sebanyak 15 kasus.Kata kunci: persalinan letak sungsang, presentasi letak sungsang

Objectives: To analyze the causative factors and short term complications in cases of placenta previa. D e s i g n a ndSettings: A retrospective study carried out in Obstetrics and Gynecology Department of Sheikh Zayed Post Graduate Institute Lahore.P e r i o d : From June 2004 to June 2007.Subject: Fifty patients with placenta previa. M a i n o u t c o m e m e a s u r e s : Age, parity, previouscesarean section, previous history of evacuation/myomectomy, average hospital stay, need for blood transfusions, emergency/electiveLSCS, associated maternal and fetal morbidity and mortality. Results: There were 50 cases of placenta previa over the period of 3 years.Most frequently occurring intra operative complication was postpartum hemorrhage in 19 patients (38%). Highest morbidity was due toanemia which was in 23 patients (46%). Maternal mortality was 2%, while neonatal mortality was 14%. C o n c l u s i o n : Placenta previa is amultifactorial disease. No doubt that the rising incidence of cesarean section is increasing the intraoperative complication associated withthe condition but other risk factors are also important like age, parity, history of evacuation etc.

Abstract Background: The purpose of our study was to assess the frequency of occult foreign body aspiration (FBA) and to evaluate the diagnostic difficulties and therapeutic methods for these patients. Methods: Between May 2000 and May 2020, 3557 patients with the diagnosis of FBA were treated in our department. Thirty-five patients with occult FBA were included in this study. A retrospective analysis of medical records was performed. Results: Twenty-three male patients (65.7%) and 12 female patients (34.3%) were hospitalized due to occult FBA. The average age was 3.60 years (range 9 months-12 years). Most of the patients were younger than 3 years old (n=25, 71.4%). Coughing (n=35, 100%) and wheezing (n=18, 51.4%) were the main symptoms and signs. All the patients were found to have a FBA under the fiberoptic bronchoscope. The most common organic foreign bodies were peanuts (n=10) and the most common inorganic foreign bodies were pen caps (n=5). The extraction of foreign bodies under rigid bronchoscopy was applied successfully in 34 patients. Only one patient needed a surgical intervention. Conclusions: Occult FBA should always be considered in the differential diagnosis of chronic or recurrent respiratory diseases that are poorly explained, even in the absence of a previous history of aspiration.

Although acne is usually recognized as an adolescent skin disorder, the prevalence of adults with acne is increasing. The clinical and epidemiological data of acne were evaluated with a view to establishing possible contributing etiological factors and observing whether clinical features differ from adolescent acne. Division of Dermatology and Venereology Outpatient Clinic Dr. M. Djamil hospital padang during January 2016 until December 2018.Retrospective study performed in Medical Cosmetic Division of Dermatology and Venereology Outpatient Clinic Dr. M. Djamil hospital padang during January 2016 until December 2018. Data was taken from medical records. Out of 224 patients included in the study 54.01% were women and 45.98 % were men. Majority of the patients had comedonal acne (45.53 %), whereas nodulocystic was the least common (13.39%). Most common predominant site of involvement was cheek (44.20 %), followed by chin (25.45 %), and mandibular area (14.58 %). Family history of acne was present in 57.70 %. Scarring was observed in a 39.2 %. Acne is predominant in women, with the most commonly involved of the cheeks, with the most common type was comedones type.

This study reviews the craniofacial clefts that presented at a Nigerian tertiary health facility, highlighting our experience with the pattern of presentation and surgical care of these patients. A retrospective review of the smile train database and medical records of all individuals who had been diagnosed with any of the Tessier craniofacial clefts and managed between 1st January 2007 and 31st December 2020 was done. The data were presented as numbers and percentages of cases. The cleft clinic of a tertiary health facility and a major cleft referral center in South-West Nigeria. Forty-five patients with craniofacial clefts were managed over the study period. 15.6% had associated syndromes, 2.2% had a family history of similar craniofacial cleft and 11% had a history of a possible teratogen. There were 21 (46.7%) middle clefts, 14(31.1%) lateral clefts and 10(22.2%) oblique clefts. The most common type of cleft was Tessier 0 while the Tessier 6 was the least common type. The median age at surgery was 10 months for male and 5months for female subjects, 15.3% complication rate was found in this study. Four patients had revision surgeries to correct residual deformities in this study. The diverse presentations and occurrence of the rare craniofacial clefts present complex aesthetic and functional problems that require individualized often multidisciplinary care. The execution of a properly planned treatment will reduce complications and the need for revision surgeries.

Background: Patient who had history of spontaneous abortion in her previous pregnancy is associated with adverse outcome in her present pregnancy.Methods: A total 63 pregnant women attending OPD and admitted in department of obstetrics and gynecology, Yenepoya Medical College, from April 2017 to September 2017, considered and outcome were studied.Results: Out of 63 patient’s majority (57.1%) of patients belong to the age group 21-29 year. Anemia was found to be very severe in 4.3%, severe in 10% and moderate in 30% patients. Maximum patients (45.7%) were with history of previous one abortion followed by previous two abortions (38.6%). The final outcomes were term livebirth 47 (74.3%), abortion 9 (14.3%), preterm delivery 5 (8.6%), and stillbirth 2 (2.8%) caesarean section (23.3%) for various indications. 19.23% had term PROM, 9.09% had PPROM, 5.76% had term IUGR, 3.84% term IUD, preterm IUD accounts for 9.09% and still birth accounted for about 1.92% which was term, pre-eclampsia accounted for 4.76%, malpresentation for 7.93%, total 3 cases of antepartum hemorrhage out of which  placenta previa accounts for about 3.1% and abruption for 1.58%, manual removal of placenta 4.7% and low birth weight 7.6%.Conclusions: Previous history of spontaneous abortion is associated with adverse pregnancy outcome. There is increased risk of abortion, preterm delivery, need for caesarean sections and fetal loss which can be reduced by booking and giving antenatal care.

Preterm labour is a common cause of neonatal morbidity and mortality. This chapter describes the definition, aetiology, diagnosis, and management of preterm labour and delivery with a focus on tocolytic therapy, the use of antenatal corticosteroids, and of magnesium sulphate. Anaesthesia for preterm delivery is discussed. The section on multiple pregnancy details the recommended antenatal careplan for dichorionic and monochorionic twin pregnancies, the fetal and maternal risks and potential complications, and the management of labour and delivery of twins, as well as the anaesthetist’s role in managing these high-risk pregnancies. There are a number of abnormal presentations managed by obstetricians, including abnormal cephalic presentations such as occiputo-posterior positions, breech, transverse, and compound presentations. This chapter focuses specifically on breech presentation, comparing the evidence for vaginal breech delivery versus planned caesarean delivery. It also discusses external cephalic version and vaginal breech delivery itself.

In an outbreak of measles in Gothenburg, Sweden, breakthrough infections (i.e. infections in individuals with a history of vaccination) were common. The objective of this study was to compare measles RNA levels between naïve (i.e. primary) and breakthrough infections. We also propose a fast provisional classification of breakthrough infections. Medical records were reviewed and real-time PCR-positive samples genotyped. Cases were classified as naïve, breakthrough or vaccine infections. We compared clinical symptoms and measles RNA cycle threshold (Ct) values between breakthrough and naïve infections. Sixteen of 28 confirmed cases of measles in this outbreak were breakthrough infections. A fast provisional classification, based on previous history of measles vaccination and detectable levels of measles IgG in acute serum, correctly identified 14 of the 16 breakthrough infections, confirmed by IgG avidity testing. Measles viral load was significantly lower in nasopharyngeal samples from individuals with breakthrough compared with naïve infections (median Ct-values: 32 and 19, respectively, p < 0.0001). No onward transmission from breakthrough infections was identified. Our results indicate that a high risk of onward transmission is limited to naïve infections. We propose a fast provisional classification of breakthrough measles that can guide contact tracing in outbreak settings.

Abstract Background High parity is a major public health concern in developing countries and it is a risk factor for postpartum hemorrhage (PPH). The aim of this study was to analyze the optimal parity cut-off values for predicting PPH in vaginal deliveries and cesarean sections in a rural Zambian setting.Methods All women who delivered at Zimba Mission Hospital in 2017 were reviewed in this retrospective survey. Those whose records were missing data on parity and those with risk factors for developing PPH (e.g. birth weight ≥ 4,000 g, multiple pregnancy, assisted vaginal delivery, and placenta previa) were excluded. We analyzed the association between parity and PPH using multiple logistic regression and ROC curve analyses.Results Among the 1,555 women included in the study, 72 (4.6%) women experienced PPH. The optimal cut-off values for parity in vaginal deliveries and cesarean sections were para 7 and 3, respectively. Using these cut-off values, the adjusted odds ratios (95% confidence intervals) were 3.26 (1.15, 9.21) and 8.28 (2.25, 30.5), respectively.Conclusions Proper preparation is required for vaginal deliveries in women with a history of ≥ 7 births and cesarean sections in women with a history of ≥ 3 births.

AbstractObjectives:(1) To assess the prevalence of arytenoid asymmetry during adduction, and (2) to correlate arytenoid asymmetry with vocal symptoms.Materials and method:The medical records and video recordings of 116 patients who presented to the voice clinic were reviewed for the presence of arytenoid asymmetry, as regards sharpening of the aryepiglottic fold angle and altered positioning of the cuneiform and corniculate cartilages.Results:There were 61 males and 55 females, with a mean age of 39 years and a standard deviation of 15 years. Almost one-third had a history of reflux, 25 per cent had a history of smoking and 9.6 per cent had a history of allergy. Hoarseness was the most common symptom, occurring in 42.2 per cent of patients, followed by vocal fatigue (25 per cent) and inability to project the voice. The most common type of asymmetry was corniculate asymmetry, present in 27.6 per cent of the cases and accounting for 74.39 per cent of cases. This was followed by cuneiform cartilage asymmetry, present in 15.5 per cent of cases. There was no correlation between arytenoid asymmetry and vocal symptoms, except for vocal fatigue (p = 0.038).Conclusion:The prevalence of arytenoid asymmetry during adduction is common. The presence of vocal symptoms such as hoarseness, breathiness, inability to project the voice and straining does not generally seem to correlate with the prevalence of arytenoid asymmetry. However, subjects with vocal fatigue are more likely to have cuneiform asymmetry.
