Tulisan ini menjelaskan tentang identifikasi waste pada perawatan kereta bagian bogie dan memberikan rekomendasi perbaikan yang bertujuan untuk mengurangi waste yang terjadi. Dengan menggunakan pendekatan lean concept yang lebih dominan ke arah lean service, penelitian ini dimulai dengan menggambarkan big picture mapping dan pembobotan nilai waste berdasarkan kuesioner menggunakan VALSAT. Waste juga diidentifikasi menggunakan fault tree analysis yang selanjutnya dilakukan perhitungan nilai kritis waste menggunakan FMEA kemudian dilakukan rekomendasi perbaikan menggunakan pendekatan 5W+1H. Hasil perhitungan berdasarkan data yang dikumpulkan, didapatkan bahwa total lead time pengerjaan perawatan kereta yang sebelumnya sebesar 995 menit menjadi 873 menit. Dengan presentase dari value added activity sebesar 93% dan non – necessary value adding sebesar 7%. Dan berdasarkan analisa FMEA didapatkan bahwa waste kritis yang diperlukan perbaikan adalah waste unnecessary motion dengan skor RPN sebesar 140.

Perkembangan industri di Indonesia, khususnya pada industri manufaktur, mengalami kemajuan yang cukup pesat. Populasi tenaga kerja industri bertambah seiring perkembangan industri dan nilai tambah industri nasional yang meningkat dengan jumlah sebanyak 30,992 orang pada tahun 2018, meningkat 17,92% dibanding tahun 2017. Namun, masalah dan penanggulangan pada kasus kecelakaan kerja masih kurang diperhatikan, dibarengi dengan jumlah kecelakaan kerja yang masih tinggi, tidak terkecuali di PT. Astra Daihatsu Motor. Maka dari itu penelitian dilakukan di area Assembly 2 di area pabrik PT. Astra Daihatsu Motor di kawasan Sunter, Jakarta, dengan metode Hazard and Operability Study (HAZOP) dan Fault Tree Analysis (FTA) untuk mengetahui potensi bahaya secara teliti serta mengungkap unsur-unsur yang berpotensi berbahaya dalam sistem. Pada penelitian ini ditemukan sebanyak 17 sumber bahaya dengan 2 sumber termasuk dalam level risiko tinggi, 3 sumber level risiko sedang, dan 12 sumber level risiko rendah. Analisis deduktif dengan Fault Tree Analysis didapatkan hasil dengan munculnya 5 basic event dari Thinner dan 4 basic event dari Lampu Heater dengan rekomendasi perbaikan pada aspek desain, konstruksi, dan pelatihan handling yang baik pada material tersebut. Abstract[Proposed Improvement of Hazard Sources In The Area of Assembly 2 With The Hazard And Operability Study And Fault Tree Analysis (Case Study: PT. Astra Daihatsu Motor)]The development of industry in Indonesia, especially in the manufacturing industry, has experienced rapid progress. The industrial labor population increased as industry development and national industrial value added increased by as many as 30,992 people in 2018, an increase of 17.92% compared to 2017. However, the prevention and countermeasures in cases of workplace accidents are still not considered as priority, including in PT. Astra Daihatsu Motor. Therefore a research has been conducted in the Assembly 2 area at the PT. Astra Daihatsu Motor in the Sunter area, Jakarta, using the applicability of Hazard and Operability Study (HAZOP) method and the Fault Tree Analysis (FTA) method to carefully identify potential hazards and reveal potentially dangerous elements in the system. In this study 17 sources of hazards were found with 2 sources including high risk level, 3 medium risk level sources, and 12 low risk level sources.  Deductive analysis with Fault Tree Analysis is obtained, namely 5 basic events from Thinner and 4 basic events from the Heater Lamps with recommendations on aspects of design, construction, and handling training in the material.Keywords: work accident; hazard and operability; risk level; fault tree analysis

A company, will experience a process of ups and downs where this is a common thing. At the end of 2020, the world was shocked by the corona virus pandemic in all parts of the world. The impact of this pandemic has led to the confinement of employees as in this company. So that it makes productivity decrease, especially in the Quality department. To deal with this, the company asks employees to make changes in the sample testing flow process, to identify non-value-added activities. This resulted in disruption of the shipping process and many samples piled up. The purpose of this study is to identify and plan improvements that lead to non-value-added activities. This study uses the Fault Tree Analysis (FTA) and 5W1H methods. The FTA analysis shows that the cause of non-value-added activity is the mixing process is too long, too many processes during molding and the proofing process is too long. Based on this FTA analysis, priorities for corrective action plans can be determined to implement these improvements. The improvement plan uses the 5W1H method, which is to review the current test methods. The results of this study were very effective because they were able to reduce the processing time by 59 minutes.Keywords: Fault Tree Analysis, 5W1H, flow process, effectiveness and pandemic.

Wireless communications became one of the most widespread means for transferring information. Speed and reliability in transferring the piece of information are considered one of the most important requirements in communication systems in general. Moreover, Quality and reliability in any system are considered the most important criterion of the efficiency of this system in doing the task it is designed to do and its ability for satisfactory performance for a certain period of time, Therefore, we need fault tree analysis in these systems in order to determine how to detect an error or defect when happening in communication system and what are the possibilities that make this error happens. This research deals with studying TETRA system components, studying the physical layer in theory and practice, as well as studying fault tree analysis in this system, and later benefit from this study in proposing improvements to the structure of the system, which led to improve gain in Link Budget. A simulation and test have been done using MATLAB, where simulation results have shown that the built fault tree is able to detect the system’s work by 82.4%.
