Every job has the opportunity of the risk because occurrence of work accidents can be caused by various factors. PT. CA is a large distributor of motorcycle spare parts which has activities including receiving goods from fabrication and then storing them in temporary warehouses before being distributed to dealers in various regions. The company's activities have a large potential for accidents due to several operations. To hazard identification, risk assessment, and determining control in any work that occurs or has the potential to occur, it is necessary to analyze using HIRADC. The method used is a descriptive method described in the flow process and then identified possible hazards that occur in each activity step. After the identification process is complete, a risk assessment is carried out to determine the form of control. The object of research is the receiving section of the work area. The results of this study are that the receiving section has 7 existing activities and there are 20 potential hazards that are possible, from these potentials the current control is to maximize APD to minimize danger. The highest level of risk is substantial in the activity of moving cargo to temporary storage or inspection areas with a potential danger of crashing into workers which has an impact on limb injuries, fractures, and even death, so this activity is a concern to avoid work accidents. In addition, it is determined that hazard control is carried out by means of forklift lanes that are not allowed to be passed by workers and control recommendations are in the form of setting SOPs, ensuring that forklifts complete their activities to carry out inspections. A company, will experience a process of ups and downs where this is a common thing. At the end of 2020, the world was shocked by the corona virus pandemic in all parts of the world. The impact of this pandemic has led to the confinement of employees as in this company. So that it makes productivity decrease, especially in the Quality department. To deal with this, the company asks employees to make changes in the sample testing flow process, to identify non-value-added activities. This resulted in disruption of the shipping process and many samples piled up. The purpose of this study is to identify and plan improvements that lead to non-value-added activities. This study uses the Fault Tree Analysis (FTA) and 5W1H methods. The FTA analysis shows that the cause of non-value-added activity is the mixing process is too long, too many processes during molding and the proofing process is too long. Based on this FTA analysis, priorities for corrective action plans can be determined to implement these improvements. The improvement plan uses the 5W1H method, which is to review the current test methods. The results of this study were very effective because they were able to reduce the processing time by 59 minutes. Keywords: Fault Tree Analysis, 5W1H, flow process, effectiveness and pandemic.

PT. Selamat Sempurna, Tbk is a company that manufactures  automotive parts, namely car filters. This filter product has many part numbers. In this company has one of the production lines, namely Line Seat Press Shop, this line produces components from filters, namely seat components. Seat is the main part of the filter component that functions as a filter holder to the engine. This Seat component consists of several part numbers (shapes or variations) following the filter product, and some part numbers handled by the manual engine B070 & L224 will affect the changes in the production engine settings. This production machine includes machines that require time in a slow setup process for switching dies when switching from one part number to another part number compared to other machines, because the element of the process setting is long. With a fishbone diagram, the root cause of the Change Over Part long problem on the machine is from the method category (there is no preparation desk dies and there is not separation of external setup and internal setup). Previously the time needed for Change Over Part was 1961 seconds. After knowing the root cause of the problem and the improvement of the time needed is only 866 seconds. The time difference is 1067 seconds, so save on average time 1095 seconds or 18 minutes / setup. Keywords: Filter product, Change Over part (COP), Fishbone Diagram, Setup, Process Elements.

PT Mitsuba Indonesia is a factory engaged in the manufacture of electrical automotive spare parts. The LED winker lamp is the newest product whose production demand is always increasing. The large demand for LED winker lamp products must of course be balanced with the readiness of production machines which are always in good condition. But in practice, the results of the production achievement are still low, this is due to the high machine loss time, especially in infrared welding machines. This study discusses the application of kaizen which aims to find the causes of loss time, make improvements in order to reduce loss time and determine loss time and productivity after repairs are made. The method used to analyze the causes of loss time is to calculate the value of six big losses and analysis of the cause and effect diagram. The analysis results showed that the idling minor factor and stoppages losses caused by part drop error and the set up and adjustment losses factor caused by a change in the type of production jig. Improvements made to the part drop error were adding 1 set of regulators. Meanwhile, the change in the production jig type is to modify the locking system on the lower and upper heater jig. Based on these improvements, it was able to reduce the loss time to 4.86% and productivity to increase 7.50% in July. Loss time fell 4.70% and productivity increased 7.90% in August. Keywords: LED winker lamp, infrared welding machine, loss time, six big losses, productivity.

PT. Ching Luh Indonesia is a company engaged in the manufacturing industry that produces sports shoes with well-known global brands, with annual revenues of more than US $ 1 billion, and has 8,000 workers. PT. Ching Luh Indonesia is a company that is responsive to technological advances and pays great attention to the comfort of employees in working, especially in the Laboratory section. In that section, the bonding checking process on the shoe samples currently still uses direct checking without any time reference in each sample. This is considered less effective because it makes the addition of the remaining sample for the next shift. This study aims to determine the standard time of each shoe sample variant and to determine the number of samples received by each operator. From the research results using the calculation of the standard time can be determined the standard time of each variant of the shoe sample, so it can also be determined the number of samples received for each operator. By determining the standard time for each shoe sample, it is expected that each operator can complete each sample according to the predetermined standard time, so that the specified target can be achieved.. Keywords: Standard Time, Bonding, Shoe Samples, Laboratory, Operator
