Splenic abscess is a rare entity. Delay in its diagnosis and treatment can lead to high morbidity and mortality. Here, we describe a case of a female patient with air-uid level in the parenchyma of the spleen caused by a gas forming bacterial infection. There was no preceding history of trauma or instrumentation in our case. This case was managed by prompt diagnosis followed by percutaneous drainage under broad spectrum antibiotic coverage. Agas forming infection of the spleen without manipulation or trauma is extremely rare, with only 3 reported cases in the literature

Obturator hernias are rare but pose a diagnostic challenge with relatively high morbidity and mortality. Our patient is an elderly, thin female with an initial evaluation concerning for gastroenteritis, and further evaluation revealed bilateral incarcerated obturator hernias, which confirmed postoperatively as well as a right femoral hernia. An 83-year-old female presented to the outpatient office initially with one-day history of diarrhea and one-week history of episodic colicky abdominal pain. She returned 4 weeks later with diarrhea resolved but worsening abdominal pain and left inner thigh pain while ambulating, without changes in appetite or nausea and vomiting. Abdominal CT scan then revealed bilateral obturator hernias. Patient then presented to the emergency department (ED) due to worsening pain, and subsequently underwent hernia repair. Intraoperatively, it was revealed that the patient had bilateral incarcerated obturator hernias and a right femoral hernia. All three hernias were repaired, and patient was discharged two days later. Patient remained well postoperatively, and 15-month CT of abdomen showed no hernia recurrence.

ABSTRACT Methotrexate is one of the most widely used broad-spectrum immunomodulator. It can be used as a primary option or as in combination of drugs in various immunological conditions. Generally, it is safe when use in lower dosages. However, a clinician has to be alert regarding some of its less common but toxic side-effects. Introduction.Osteochondritis dissecans (OCD) of the capitellum is a localized disorder of the subchondral bone, in a region with limited healing capacity. Although its aetiology is still unknown, it has been associated with repetitive microtrauma. The natural history of this disease involves the evolution for degenerative joint disease in approximately half of the patients, with early identification and treatment being critical to optimizing the outcome.Case Presentation.We present a rare case in our practice, illustrating a capitellar OCD in a fifteen-year-old White male without an identified cause of repetitive microtrauma.Conclusion.In this case prompt diagnosis and arthroscopic-assisted treatment led to a successful result.

Background. Bile contamination from the digestive tract is a well-known risk factor for postoperative complications. Despite the literature concerning prevalence of bacterobilia and fungobilia in patients with biliary pathologies, there are no specific recommendations for perioperative antimicrobial coverage for biliary/pancreatic procedures. We evaluated the effect of at least 72 hours of perioperative broad spectrum antibiotic coverage on outcomes of pancreaticoduodenectomy (PD). Materials and Methods. A retrospective review of all patients at Case Medical Center of Case Western Reserve University undergoing PD procedure, from 2006 to 2011, was performed (n=122). Perioperative data including demographics, comorbidities, biliary instrumentation, antibiotic coverage, culture results, and postoperative outcomes were analyzed. Propensity score matching method was used to match the patients according to duration of antibiotic coverage into two groups: 72 hours (A72) and 24 hours (A24). Results. Longer broad spectrum antibiotic coverage in group A72 resulted in significantly less surgical site infections after PD, compared to routine 24 hours of perioperative antibiotics in group A24. This study did not reveal a statistically significant decrease in postoperative fungal infections in patients receiving preoperative antifungals. Conclusion. Prolonged perioperative antibiotic therapy in conjunction with intraoperative bile cultures decreases the short-term infectious complications of PD, with no significant increase in Clostridium difficile colitis incidence.

Endomyometritis following parturition is a major cause of maternal morbidity. It is most common following cesarean delivery, especially in certain high-risk patient populations. The infection is usually caused by bacteria in the cervicovaginal tract that are inoculated into the uterus during labor and delivery. Both anaerobes and aerobes are thought to be involved in the disease process. A prompt diagnosis based on clinical suspicion, a thorough physical examination, and adjunctive laboratory measures is necessary to insure effective therapy and prompt resolution of the infection. The treatment consists of supportive care and broad-spectrum antibiotic coverage either with single extended-spectrum drugs or with combinations of antimicrobials. In cases appropriately treated, recovery without sequelae is the rule.

Abstrak Pendahuluan: Abses retrofaring adalah terkumpulnya nanah di ruang retrofaring yang merupakan salah satu daerah potensial di leher dalam. Abses retrofaring merupakan kasus yang jarang tetapi dapat menyebabkan kematian terutama pada umur di bawah 5 tahun. Sejak ditemukannya antibiotika, angka kesakitan dan kematian akibat abses menurun drastis. Metode: Dilaporkan satu kasus abses retrofaring dengan riwayat ketulangan pada anak gizi kurang umur 9 tahun. Diagnosis ditegakkan berdasarkan anamnesis, pemeriksaan fisik dan pemeriksaan radiologi. Pada pemeriksaan foto polos jaringan lunak leher, terlihat gambaran pelebaran ruang retrofaring dan air fluid level. Diskusi: Penatalaksanaan meliputi pemberian antibiotika, drainase dan eksplorasi abses serta perbaikan keadaan umum. Kata kunci: abses retrofaring, benda asing, drainase Abstract Introduction: Retropharyngeal abscess is defined as accumulation pus in retropharyngeal space which is a potential area in deep neck space. Retropharyngeal abscess is a rare case but it can cause death especially in children under five years old. Since antibiotics were found, morbidity and mortality of this case was drastically decreased. Methods: A retropharyngeal abscess of child 9 years old with history of swallowed foreign body (fishbone) and lack of nutrition has been reported. Diagnosis was based on anamnesis, physical examination and radiographic finding. In soft tissue cervical radiograph we found, widening of retropharyngeal space with air fluid level. Discussion: Management for abscess is intravenous antibiotics, drainage and exploration abscess and improve general condition has been performed Keywords:Retropharyngeal abscess, foreign body, drainage

<p>Carcinosarcoma is a malignant tumor with a dual histological appearance, a squamous epithelial (sarcomatous), and a mesenchymal component. Carcinosarcomas are characterized by a high recurrence rate, metastatic spread, and high morbidity and mortality. Carcinosarcoma can arise from any squamous epithelial lining but rare in the sino-nasal tract. We are presenting a case of a 19-year-old male with a 3-month history of recurrent epistaxis associated with nasal blockage, anosmia, and bilateral complete loss of vision over a few weeks. Carcinosarcoma of the sino-nasal tract is a rare entity and can mimic any sino-nasal malignancy. The optimal treatment of this disease is controversial and lacks a clear guideline, therefore challenging to the treating oncologists.<strong> </strong>Carcinosarcoma of the head and neck is a rare and aggressive tumor. It poses diagnostic and treatment challenges.</p>

A rare case of synchronous ipsilateral pleomorphic adenomas (PA) of the left parotid and submandibular glands is reported. Simultaneous multiple PA in major salivary glands are a very rare entity, and merely few cases of ipsilateral synchronous PA involving parotid and submandibular glands are reported in the literature. The case of a 40-year-old female with a six-year history of asymptomatic growing lesion in both left parotid and left submandibular regions is presented. Left superficial parotidectomy and left submandibular gland excision at the same surgery have been performed. The aim of this article is to highlight the importance of an accurate head and neck presurgery examination both clinically and radiologically, keeping in mind the possibility of multiple tumor location.

An accessory lobe of the liver is a rare entity in clinical practice which is diagnosed incidentally. Infrequently, it may present as torsion with a clinical picture of an acute abdomen, a palpable mass, and may be associated with liver function abnormalities. Many of these patients have a history of previous surgery for congenital abdominal wall defects such as omphalocele. We present an extremely rare case of torsion of an accessory hepatic lobe in an 11-month-old male patient who presented in a state of shock. The infant underwent laparotomy and excision of the accessory lobe. Here, we aim to emphasize the importance of prompt management and early resection which is the cornerstone of a favorable outcome.

Cervical tuberculosis is a rare entity with only limited number of case reports in the literature. A 53 year old, P1, post-menopausal housewife presented with history of one episode of post-menopausal bleeding and whitish discharge per vaginum. Speculum examination revealed an ulceroproliferative cervical growth. Clinically she was diagnosed to have carcinoma cervix. However, cervical biopsy confirmed the diagnosis of cervical tuberculosis. She was treated with anti-tubercular drugs for six months and has been disease free for the last two years. This case emphasizes the fact that though cervical tuberculosis is rare, it should be considered in the differential diagnosis of carcinoma cervix.
