The Nature and Frequency of Medical Emergencies in Dental Offices of Karachi, Pakistan

Background: Medical emergencies are an unfortunate part of the dental practice, to which the dentist must have the required knowledge and means to counter them. In this study, we aimed to determine the nature and frequency of medical emergencies occurring in the dental offices of Karachi, Pakistan. We also aimed to ascertain the awareness and preparedness of dentists and their attitude towards training in management of these medical emergencies. Methods: In this study, 300 questionnaire forms were distributed to dentist at various colleges in Karachi city in the year 2015, out of which 244 complete and valid forms were received and used, giving a response rate of 81%. SPSS version 16.0 was used to compute and analyze the collected data. Results: Out of 244 respondents, 23% were males and 77% were females. Since, 83% of the dentist had faced a medical emergency at least once in their practice. Highest recorded medical emergency was found to be syncope (faints) being reported by 71% of the dentist. Most of the medical emergencies (89%) took place during an extraction procedure. 55% dentist claimed that they were prepared enough to manage an emergency. Irrespective of their perceived abilities, 100% showed keen interest in the field of improvement through continuing education in management of medical emergency. Conclusion: Medical emergencies are common in dental practice and a substantial proportion of dentists feel incompetent in handling such situations. This requires better medical emergency management courses and training in undergraduate dental programs as well as continued dental education and training.

The management of medical emergencies is clearly of vital importance in dental practice. The starting point of training in this area is the undergraduate dental degree programme. The aim of this study was to examine the opinions of final-year dental students at Newcastle School of Dental Sciences in relation to their undergraduate training in medical emergency management. Overall the responses were positive but the need for further practical training in the use of emergency drugs and equipment was highlighted.

ABSTRACT Aims More than 18,000 patients need medical emergencies management in dental offices in Iran annually. The present study investigates medical emergencies management among Iranian dentists. Materials and methods From the list of the cell phone numbers of the dentists practicing in the city of Tehran, 210 dentists were selected randomly. A self-administered questionnaire was used as the data collection instrument. The questionnaire requested information on personal and professional characteristics of the dentists, as well as their knowledge and selfreported practice in the field of medical emergency management, and availability of required drugs and equipments to manage medical emergencies in their offices. Results Totally, 177 dentists (84%) completed the questionnaire. Less than 60% of the participants were knowledgeable about characteristics of hypoglycemic patient, chest pain with cardiac origin, and true cardiopulmonary resuscitation (CPR) practice. Regarding practice, less than one quarter of the respondents acquired acceptable scores. In regression models, higher practice scores were significantly associated with higher knowledge scores (p < 0.001). Conclusion The results call for a need to further education on the subject for dentists. Continuing education and changing dental curriculum in the various forms seems to be useful in enhancement of the self-reported knowledge and practice of dentists. Clinical significance To successful control of medical emergencies in the dental office, dentists must be prepared to recognize and manage a variety of such conditions. In addition to dentist's knowledge and skill, availability of necessary equipments and trained staff is also of critical importance. Medical emergencies are one of the most stressful situations the staff in a dental practice might encounter. The duty of care toward the attending patients obligates suitable preparedness to provide the necessary care if such emergencies ensue. Unfortunately, we found that 22% of the investigated dental clinics had no emergency kit available. Only 38% of the interviewed dentists felt confident to perform CPR, and 18% had no confidence to manage any medical emergency. An MCQ test of 20 questions examining the dentists’ knowledge in medical emergencies was distributed, and the level of knowledge was found to be suboptimal. The average score of the interviewed dentists was 10.87 out of 20. Experience and specialty training had a negligible effect on the level of knowledge.

This study explored South Australian volunteers' motivations for volunteering, their satisfaction with volunteering, and their need for supportive structures. Primary data were collected from volunteers working for a self-funded charitable organisation. A self-enumerated questionnaire was distributed by mail; 214 were completed and returned (66.9% response rate). Of the responding volunteers, 82% () were female. All were aged over 60 years (range: 63–76 years; mean: 68 years). Numerical data were entered into SPSS and analysed descriptively. Content analysis was applied to narrative data. Volunteers reported that they volunteered most often to help others, because they had spare time and because of what they received in return. Difficulties experienced included accessing continuing education and training. Organisations engaging volunteers have a responsibility to ensure that volunteers are adequately supported both in their day to day work and in training or education. Volunteers are able to become valued partners in service delivery.

In the dental practice or in the ambulatory of oral and maxillo-facial surgery, there may be real medical emergencies, emergencies in which the speed of effective measures is essential. A good knowledge of the things to be done in the practice, as first representatives of the medical shield in defence against the inexorable secures the winning of precious seconds. Medical care in dentistry and dentoalveolar surgery involves two seemingly different aspects: a well-defined technical one, of strict specialty, and another less well defined, which practically includes all the particularities of the patient coming to the dental practice. However precise the dental technique and the practitioner�s skill are, if the specific conditions of each patient are not taken into account, the medical benefit cannot rise to a high level, and the final result may be compromised by complications with unforeseen risks. The study included 7,996 patients resolved in the Oral and Maxillo-facial Surgery Clinic (Ambulatory), in the period from 1.02.2014 to 31.12.2018. The superficiality of a seemingly healthy patient approach may lead to the disregard of some important aspects with a predictive role in the triggering of a medical emergency, as an informational history must be more than an orderly list of symptoms. You always gain something by listening to patients and observing the way they talk about their symptoms. The crossed statistical deductions on the explored data revealed, based on the majority percentages obtained on each studied index, that the following have an extremely important aspect for the dental practice and dentoalveolar surgery: the dental anxiety level as well as the patient�s general status are the landmarks that require the greatest attention from the practitioner for the purpose of preventing medical emergencies.

ObjectivePhysical activity (PA) counselling has been shown to raise awareness of the importance of PA and to increase the rate of PA engagement among patients. While much attention has been paid to examining the knowledge, attitudes and practice of general practitioners in relation to PA counselling, there is less literature examining such issues in hospital-based doctors in Ireland and further afield. This study aimed to explore doctors’ PA counselling practices and to analyse how this related to their level of PA knowledge, training and attitudes.MethodsAn invitation to participate in an online survey was sent to 4692 members of the Royal College of Physicians of Ireland who were listed as having an address in Ireland. Descriptive and explorative analyses of the data were performed using IBM SPSS V.22.0.ResultsA total of 595 valid responses were included (response rate 12.7%; 42.7% male, 42.6±12.1 years). The majority reported enquiring about PA levels (88.0%) and providing PA counselling (86.4%) in at least some of their patients. Doctors who saw it as their role and those who felt more effective/confident in providing PA counselling were significantly more likely to do so. A perceived lack of patient interest in PA and patient preference for pharmaceutical intervention were significant barriers to undertaking PA counselling.ConclusionThis study demonstrates the need for further education and training in PA counselling in Ireland with a particular focus on improving the attitudes and self-efficacy of doctors in this area at both undergraduate and postgraduate levels.
