Purpose:  To study the surgical outcomes of Dacryocystorhinostomy (DCR) in fistulous dacryocystitis. Study Design:  Quasi Experimental study. Place and Duration of Study:  The detailed study was carried out in the Institute of Ophthalmology, Liaquat University Hospital Jamshoro, between September 2018 to August 2020. Material and Methods:  We analyzed the histories of 30 patients taking the DCR procedure and noted their mean age, standard deviation, follow-up time, complications and other details. We also reported the intraoperative anatomical results, postoperative analysis, and variable groups vs. outcomes post-surgery using SPSS Version 20. Results:  The mean patient's age was 44.2 ± 4.13years, where males to female percentages were 27% to 73%. We noted significant changes in patients with a success rate of 87% displayed by no relieved epiphora and lacrimal patency in 1 month, 3 months, and 6 months. The mean time of the patients was 4 months varying between 1 to 8 (months) and the variable group values vs. surgical outcomes showed no significant association between the variables (p-value ranging from 0.195 to 0.935). Conclusion:  Complications resulting in some patient’s post-surgery are manageable and the surgical technique has a good success rate. Abstract Background Acute acquired comitant esotropia (AACE) is a type of strabismus characterized by a sudden onset of large angle esotropia with diplopia, which often occurs in children after infancy, teenagers, and young adolescents. However, studies on the surgical outcomes of only adults are rare. The purpose of this article is to analyze the surgical outcomes for adult patients diagnosed with AACE. Methods Medical records of 24 patients who had undergone surgery for AACE were retrospectively analyzed. The main outcome measures were the final motor and sensory success rate after surgery and factors affecting motor and sensory outcomes. Motor success was considered alignment within 8 prism diopter (PD) at both near and distance and sensory success was stereoacuity ≥ 60 sec/arc. Results The preoperative mean esodeviation angles were 33.1 ± 10.4 PD at distance and 33.3 ± 11.2 PD at near. The mean period of postoperative follow up was 7.5 ± 4.5 months (range 1–8 months). The postoperative mean esodeviation angles at final follow-up time were 3.4 ± 6.1 PD at distance and 3.8 ± 6.7 PD at near. The surgical motor success rate at final follow-up was 79.2% (19/24). The sensory success rate at final follow-up was 50.0% (12/24). The factor affecting the motor outcome was the type of surgery (p < 0.05). The factor affecting sensory outcome was postoperative follow-up time (p < 0.05). Conclusions Surgery type appears to affect surgical motor outcomes in adults with AACE. Although the sensory outcome was favorable, it seems that regaining bifoveal fixation takes time.

Objective: To determine the effect of husband's support in the implementation of oxytocin massage by midwife on the production and time of colostrum secretion among post C-Section delivery women.Methods: A quasi-experimental study with a post-test only group design at hospital in DIY in April-June 2020. The samples consisted of 32 respondents, 16 post C-Section delivery women with oxytocin massage who received husband's support and 16 respondents with oxytocin massage without husband’s support. The preparation of husband’s support was conducted through training by midwife to the husbands using a booklet for 80 minutes. Data analysis in this study used univariate, Independent t-test, and MANOVA with a significance level of 95%.Results: Evaluation showed that 68.75% of post C-Section delivery women received good husband’s support. The mean colostrum production in the oxytocin massage group by midwife with husband's support was 3.61 cc and the mean time of colostrum secretion was 4.13 hours better than oxytocin massage without husband's support.Conclusion: Husband's support in the implementation of oxytocin massage by midwife had an effect on the production and time of colostrum secretion among post C-Section delivery women with p-value <0.0001.

Purpose: To compare surgical outcomes according to the distance between the preplaced suture and the muscle insertion in lateral rectus (LR) recession for exotropia. Methods: We retrospectively reviewed 64 patients who had undergone unilateral LR recession by one right-handed surgeon and divided the patients into 2 groups: right LR recession (R group, 26 patients) and left LR recession (L group, 38 patients). In the R group, the preplaced suture was behind the hook, far from the insertion; in the L group, the suture was in front of the hook, near the insertion. The main outcome measures were postoperative alignment and surgical success rate and the secondary outcome measure was effect/dose ratio. Surgical success was defined as an alignment between 10 prism diopters (PD) of exodeviation and 5 PD of esodeviation both at distance and at near. The effect/dose ratio was defined as the corrected angle of deviation at postoperative 6 months divided by the amount of LR recession. Results: The mean angle of deviation at distance showed a significant difference between the groups at postoperative 3 months (p = 0.022), but not at final follow-up (p = 0.163). There was no difference between the groups in the mean angle of deviation at near postoperatively. Surgical success was achieved in 73.1% of the R group and 71.1% of the L group at final follow-up, which was not significant (p = 0.860). Conclusions: The distance between the preplaced suture and the muscle insertion in LR recession did not affect the surgical success rate or the postoperative alignment at final follow-up.

Purpose. To evaluate a 12-year follow-up of myopic patients after iris-fixated phakic intraocular lenses (IF pIOLs) implantation. Setting. Ophthalmology Department, Military University Hospital in Prague (Czech Republic). Design. Single-center retrospective cohort study. Methods. We describe the results of a cohort study that included 85 eyes of 46 myopic patients who underwent implantation of Verisyse myopia, Veriflex, and Verisyse myopia toric (all Abbott Medical Optics, Inc.) intraocular lenses. Refractive functions and adverse events were assessed preoperatively, at 6 months, and 1, 2, 5, and 12 years after IF pIOL implantation. Results. Mean spherical equivalent was measured as −9.37 ± 2.87 D, 0.14 ± 0.61 D, and −0.42 ± 1.08 D, preoperatively, at 6 months and 12 years postoperatively, respectively. There was a significant reduction in the cylinder after surgery. At 12 years postoperatively, 90% of eyes had uncorrected distance visual acuity (UDVA) of 20/40 and 64% of 20/20. The safety index was 1.10 for the whole postoperative follow-up period. We found cataract formation in 3 eyes (3.5%). The endothelial cells loss (EC loss) directly caused by IF pIOL implantation was 6.0%, 8.10%, 12.8%, and 11.9%, at 1, 2, 5, and 12 years, respectively. In our cohort, 95% of eyes lost a higher percentage of EC than would be expected from a physiological loss at 12 years postoperatively. We found a significant negative interaction between preoperative pachymetry and EC loss, indicating that the lower pachymetry leads to a faster decline in endothelial cells density (ECD). IF pIOL re-enclavation was found in 28% of eyes. 7% of subluxations were caused by trauma. The mean time of nontraumatic re-enclavation was 6 years postoperatively. Conclusions. The study confirmed the advantages of IF pIOL implantation due to rapid visual recovery and stable visual function over the 12-year follow-up and also showed the influence of lower corneal pachymetry regarding EC loss.

Purpose. To report the epidemiological and clinical data as well as surgical outcomes of canalicular lacerations with Mini-Monoka insertion at a tertiary center in Taiwan and to discuss differences in traumatic pattern, pathogenesis, and surgical outcomes between Taiwan and other countries. Methods. From 2009 to 2018, all 48 patients who underwent canalicular laceration repair with Mini-Monoka stent at a tertiary center in Taiwan were retrospectively analyzed. Demographic and clinical data and surgical outcomes were recorded. Results. The mean age of the 48 patients was 38 years. Single lower canaliculus was involved in 37 (77.1%) patients, upper canaliculus in 10 (20.8%) patients, and both in 1 (2.1%) patient. The most common etiology was motorcycle accident (41.7%), and all traffic accident injuries accounted for 68.75% of cases. Subgroup classification revealed 64.6% of patients (n=31) were categorized in the deep laceration group, and lower anatomical and functional outcomes were noted in deep laceration. The mean follow-up time was 14.5 months. Overall, the anatomical success rate was 87.5%, and the functional success rate was 91.7% after stent removal. Conclusion. Canalicular laceration caused by traffic accidents occurred with a relatively high frequency in Taiwan. Affected patients tended to be middle-aged, and deep laceration accounted for 64.6% of patients. These were contributed by the avulsive eyelid injury mechanism caused by traffic accidents. Furthermore, the deeper lacerated site was located, and the lower anatomical and functional success rates were observed. Early repair after trauma with Mini-Monoka stents achieved good eyelid position (100%) as well as good anatomical (87.5%) and functional (91.7%) success without serious complication.

Methods: Medical records of 21 patients who had undergone surgery for AACE were retrospectively analyzed. The main outcome measures were the final motor and sensory success rate after surgery and factors affecting motor and sensory outcomes. The postoperative mean esodeviation angles at final follow-up time were 4.1 ± 5.7 PD at distance and 3.9 ± 5.6 PD at near. The surgical motor success rate at final follow-up was 76.2% (16/21). The sensory success rate at final follow-up was 55.5% (11/21). The factor affecting the motor outcome was the type of surgery ( p < 0.05). The factor affecting sensory outcome was postoperative follow-up time ( p < 0.05). Conclusions: Surgery type appears to affect surgical motor outcomes in adults with AACE. Although the sensory outcome was favorable, it seems that regaining bifoveal fixation takes time.

Objectives: For shoulders of artistic gymnasts, stability against various forces such as weight bearing, torsion, and traction is required as well as wide range of motion. The prevalence of shoulder instability in gymnasts has been reported to be high, particularly in female gymnasts; however, there has been no report on its pathology and the surgical outcomes. The purpose of this study was to retrospectively investigate the pathology and surgical outcomes of anterior traumatic shoulder instability in gymnasts. Methods: The subjects of this study were 18 shoulders of 16 gymnasts that underwent arthroscopic surgery for anterior traumatic shoulder instability. They consisted of 4 male and 14 female shoulders with a mean age of 18 years (range, 16-20). The mean follow-up was 30 months (range, 10-66 months). All surgeries were performed arthroscopically, and procedures were determined according to the intraoperative findings. Patients were immobilized with a brace for 3 weeks and started range of motion exercise after the immobilization period. Hand-stand or hanging were normally allowed at 3 month after surgery according to patients’ functional recovery. We investigated injury mechanism, intraoperative findings, surgical procedures, times to start hand-stand or hanging and to start giant swing, time to complete return to gymnastics, and recurrence of instability. Results: Three shoulders experienced dislocations that self-reduction was impossible. The remaining 15 shoulders had self-reducible dislocations or subluxations, and 10 of 15 shoulders were injured with the shoulder hyper-flexed: e.g., pulling up hands during somersaults; pushing off the vault (Figure). Bankart lesion was identified during surgery in 14 shoulders including one bony Bankart lesion. Capsular tear was found in 5 shoulders, and only one lesion was concomitant with Bankart lesion. All lesions were arthroscopically repaired. We additionally performed rotator interval closure in 10 shoulders, superior labrum repair in 13, and rotator cuff repair in 2. One patient retired from gymnastics immediately after surgery, and 2 patients were lost to follow-up. The remaining 13 patients returned to gymnastics. The mean time to start hand-stand or hanging was 4 months (range, 3-10 months), and that to start giant swing was 6 months (range, 3-15 months). The mean time to complete return was 9 months (range, 5-17 months). Two shoulders experienced recurrence (1 dislocation and 1 subluxation) after return to gymnastics. Both shoulders underwent revision surgery and returned to gymnastics. Conclusions: Capsular tear without Bankart lesion was more common (4 shoulder, 22%) in gymnasts than general population. The sport-related characteristics might be associated with the high incidence of capsular tear. The outcomes of arthroscopic stabilization for gymnasts was good with the high complete return rate.

The aim of this study to evaluate the success rate in one-surface ART restorations placed in permanent molars using a glass ionomer cement especially developed for ART in a community with high caries experience; to evaluate the operator influence on its success, post-operative sensitivity and technique acceptance by patients. Prior to placement of the restorations, the Gingival Bleeding Index, Visible Plaque Index, DMFT, dmft indices and treatment needed were assessed. One hundred and fifty five one-surface restorations were placed in permanent molars of school children, according to the WHO ART manual, 1997. After 6 months, the success rate was verified clinically and by slides. At baseline, the mean DMFT was 2.56 (±1.08) and the mean dmft, 2.53 (±2.33). Operators A and B have placed 102 and 53 ART restorations respectively. The mean time of placement was 16 minutes and 25 seconds to operator A and 14 minutes and 43 seconds to operator B. At 6-month follow up, 152 ART restorations were evaluated and 97.3% were assessed to be successful. Two restorations were excluded from the sample because the ART restorations were replaced by amalgam fillings. Only 4 restorations have failed, in which 2 due to caries, 1 due to fistula presence and the other because the restoration was lost. Four patients reported postoperative sensitivity. At this study period, ART treatment seems to be suitable for its purpose. Further evaluations are necessary to the new glass ionomer cements especially developed for ART technique provided good results at 6-month follow up in a community with high DMFT and dmft indices. The success rate for one-surface cavities in permanent molars was 97.3%, and the studied indices seemed to have no influence in this early evolution period.

Purpose:  To determine the mean change in refractive status after Levator muscle resection in patients of simple congenital ptosis. Study Design:  Quasi Experimental study. Place and Duration of Study:  This study was carried out at Institute of Ophthalmology, King Edward Medical University, Mayo Hospital Lahore, from 2014 to 2015. Material & Methods:  Fifty patients were selected under the age of 12 years with no sex predilection presenting with simple congenital ptosis of pupil sparing type. Patients with neurogenic, aponeurotic ptosis, mechanical ptosis, Blepharophimosis syndrome, patients with previous failed ptosis surgery and patients with corneal anomalies were excluded. After history and examination, Cycloplegic retinoscopy was done, which was repeated after surgical intervention. All interventions were done by a single ophthalmic surgeon. The patients were examined and discharged on the second post operative day. All data including preoperative and postoperative recordings of spherical and cylindrical values were collected on a self-designed proforma. Follow up period was extended to 3 months. At 3rd month cycloplegic retinoscopy was done and change in character of refractive status was analyzed. Mean change in refractive status was calculated by difference of preoperative and postoperative sphere and cylinder values. Results:  Mean age of patients was 4.26 ± 3.78 years. Mean change in spherical value after treatment was 0.447 ± 0.339. This difference was statistically significant with p-value = 0.000. While mean change in cylindrical value after treatment was 0.640 ± 0.580. This difference in cylinder value after treatment was also statistically significant with p-value = 0.000. Conclusion:  Levator muscle resection has a significant effect on refractive status of the patients with congenital ptosis.

Introduction: Since Longo First described it in 1998, Stapled Hemorrhoidectomy has been emerging as the procedure of choice for symtomatic hemorrhoid. Several studies have shown it to be a safe, effective and relative complication free procedure. The aim of this study was to determine the suitability of (SH) as a day cas procedure at Hue University Hospital. Methods: From Decembre 2009 to April 2012, 384 patients with third- degree and fourth-degree hemorrhoids who underwent Stapled Hemorrhoidectomy were included in this study. Parameters recorded included postoperative complications, analegic requirements, duration of hospital stay and patient satisfaction. Follow-up was performed at 1 month and 3 months post-operative. Results: Of the 384 patients that underwent a Stapled Hemorrhoidectomy 252 (65,7%) were male and 132 (34,3%) were female. The mean age was 47,5 years (range 17-76 years. Duration of hospital stay: The mean day was 2,82 ± 1,15 days (range 1-6 days). There were no perioperative complications. There was one case postoperative complication: hemorrhage; Follow-up after surgery: 286 (74,4%) patients had less anal pain, 78 (20,3%) patients had moderate anal pain, 3 (0,8%) patients had urinary retention; Follow-up after one month: good for 325 (84,6%) patients, average for 59 (15,4%) patients; Follow-up after three months: good for 362 (94,3%) patients, average for 22 (5,7%) patients. Conclusion: Our present study shows that Stapled Hemorrhoidectomy is a safe, reduced postoperative pain, shorter hospital stay and a faster return to unrestricted daily activity
