Information technology is growing rapidly in the development of even all walks of life. Every development makes it easy for the public to receive information quickly. An example in the health sector is in the medical record information system. A medical record is a unit of data from facts or evidence about a patient's history, patient's condition and previous treatments, and written by medical staff who provide health services to patients. Recording medical records by recording in the patient's book is an old method used, and a problem often encountered when using the old medical record system is found to be difficult in managing patient files. This medical record will later be used by medical personnel to add or view patient health records that have been treated. In this study a medical record system was designed using the waterfall method. Waterfall is a systematic and sequential model for information system development. Therefore the researchers developed the system using the waterfall method. The purpose of this medical record information system is to design a medical record information system that can manage patient history data and monthly reports. The system designed is very important because to prevent errors in processing patient data. This application is made based on web using the programming language HTML, PHP, and MYSQL database. So as to produce a web medical record that can be used to make the most disease reports every month, and the number of monthly patient visits using the web-based waterfall method.

Puskesmas is a level 1 health facility. More than 40% of Indonesia's population uses health services at the Puskesmas. It is interesting that the Puskesmas is the health care provider that is closest to the community. Recording medical records of patients at the Kuningan Health Center is still done manually. Data search has time constraints. This happens because the same data is often found. Ineffective management of medical records will become a major problem in health services at the Puskesmas. This problem must be overcome so that the puskesmas has good data and information. One way to overcome this problem is to build a computerized medical record information system. Medical Record is a compilation of facts about the health and illness of a patient. Medical Records become a very important thing in the delivery of health services. Because the importance of a medical record, the author is interested in conducting research with the title "Information Systems for Outpatient Medical Records in UPTD Puskesmas Kuningan Web-Based". The medical record information system is expected to help improve the function of the Puskesmas as a place of health care. With the existence of a medical record system, each patient visit can be taken in a database making it easier for officers in the process of finding medical record data when needed. With the database, the compilation of patients forgetting to bring a treatment card can be done by searching the patient's data by the electronic officer. Making a report will be easier because it retrieves data that is done through the request system so as to facilitate the process and minimize errors in data management.Keywords: php, mysql, medical record, outpatient

Chio Medical Center is one of the services in the health sector that is engaged in special medical check-ups for prospective employees who will work and permanent employees in a company. Currently, the system that processes medical check-up report data has not been computerized properly, so that data or files are stored conventionally and it is difficult to find the required data. The research method used in this study is the System Development Life Cycle (SDLC) method and system development using the Waterfall model which starts from analysis and observation of the system running in the company. The results of the observations show that the system that runs at the Chio Medical Center clinic has not been computerized properly, namely in recording examination reports using excel and word files so that they have not been stored in a structured database. This system is designed using the Dreamweaver application as a web-based editor tool and uses the PHP and MySQL programming languages ​​as the database. The result of this research is a web-based medical check-up report information system which is expected to assist employees in inputting data and improve the process of providing information services to clients so that it is faster and more accurate.

Medical record clerk Public Hospital in South Solok still trouble in the process management of inpatient data because it is still using manual systems that haven't computerized. Then needed a medical record information system Web-based inpatient care can help medical record officers in the process of the patient data management. Objectives; To produce a medical record information system Web-based inpatient Public Hospital in South Solok. Method; This system design method using the system development life cycle SDLC (Systems Development Life Cycle) which consists of the stages of planning, analysis, design, implementation, and usage. Results; Produce a medical record information system web-based inpatient public hospital in south solok. which can be used for computerized medical record officers by the management of the medical record in inpatient services unit. Conclusion; The presence of medical record information system Web-based Inpatient Public Hospital in Solok Selatan, can facilitate the attendant medical record medical record data in the management of patient hospitalization, because the system information it generates various reports information medical record of patients required inpatient management for decision making.

The purpose of this research is to know the geographic information system of tourism that is in Dinas Kepemudaan dan Olahraga dan Pariwisata Kebumen Regency and develop it Research methods used by doing observationin Dinas Kepemudaan dan Olahraga dan Pariwisata Kebumen Regency, then proceed with the system development method. the research method used is by the method of SDLC (System Development Life Cycle). By using the Software Notepad ++ to build tourism Geographical Information System web-based. PHP as a programming language, MySQL as the database server and the design of the map using the Google Maps API. The object-oriented approach that is used UML (Unified Modeling Language) can explain the flow of the existing system. Dinas Kepemudaan dan Olahraga dan Pariwisata Kebumen Regency has been doing promotion through mass media such as newspapers and brochures in the delivery of information. But the way is not enough to inform tourism and places of attractions. It is therefore through the design of Geographical information system of tourism was able to resolve the issue. After the results of the study of geographic information systems is expected delivery of tourism information becomes more widespread, and the tourists could be quick and precise in finding information a tourist want to visit.
