COMPONENT ALLERGY DIAGNOSTICS AS A TOOL FOR THEMANAGEMENT OF PATIENTS WITH ALLERGIC DISEASES

Molecular diagnostics makes it possible to determine true and cross reactivity differentially, which is of great clinical importance not only for the diagnosis of the true spectrum of sensitization, but also for the reasonable choice of pathogenetic therapy, prediction of the effectiveness and risks associated with allergenspecific immunotherapy. A number of clinical cases are presented as examples demonstrating the clarifying nature of molecular allergodiagnostics.

Shellfish allergy affects 2% of the world’s population and persists for life in most patients. The diagnosis of shellfish allergy, in particular shrimp, is challenging due to the similarity of allergenic proteins from other invertebrates. Despite the clinical importance of immunological cross-reactivity among shellfish species and between allergenic invertebrates such as dust mites, the underlying molecular basis is not well understood. Here we mine the complete transcriptome of five frequently consumed shrimp species to identify and compare allergens with all known allergen sources. The transcriptomes were assembled de novo, using Trinity, from raw RNA-Seq data of the whiteleg shrimp (Litopenaeus vannamei), black tiger shrimp (Penaeus monodon), banana shrimp (Fenneropenaeus merguiensis), king shrimp (Melicertus latisulcatus), and endeavour shrimp (Metapenaeus endeavouri). BLAST searching using the two major allergen databases, WHO/IUIS Allergen Nomenclature and AllergenOnline, successfully identified all seven known crustacean allergens. The analyses revealed up to 39 unreported allergens in the different shrimp species, including heat shock protein (HSP), alpha-tubulin, chymotrypsin, cyclophilin, beta-enolase, aldolase A, and glyceraldehyde-3-phosphate dehydrogenase (G3PD). Multiple sequence alignment (Clustal Omega) demonstrated high homology with allergens from other invertebrates including mites and cockroaches. This first transcriptomic analyses of allergens in a major food source provides a valuable resource for investigating shellfish allergens, comparing invertebrate allergens and future development of improved diagnostics for food allergy.

Timothy grass pollen is a source of potent allergens. Among them, Phl p 1 and Phl p 5 are thought to be the most important, as a majority of timothy grass-allergic individuals have IgE antibodies directed against these two allergens. The profilin from timothy grass (Phl p 12) has been registered as a minor allergen, with up to 35% of individuals in populations of grass pollen allergic patients showing IgE binding to Phl p 12. Profilins are primarily minor allergens and are known for a high likelihood of co-sensitization as well as cross-reactivity situations caused by their sequence and structure similarity. The crystal structure of Phl p 12.0101 was determined and it revealed that this allergen may form an unusual dimer not previously observed among any profilins. For example, the Phl p 12 dimer has a completely different geometry and interface when compared with the latex profilin (Hev b 8) dimer that has its crystal structure determined. The structure of Phl p 12.0101 is described in the context of allergenic sensitization and allergy diagnostics. Moreover, the structure of the Phl p 12.0101 dimer is discussed, taking into account the production of recombinant allergens and their storage.

The article presents two clinical cases of microscopic polyangiitis in patients with symptoms of glomerulonephritis with renal failure, which were preceded by such nonspecific symptoms as: abdominal syndrome, high blood pressure, arthralgia, myalgia, weight loss, uveitis, shortness of breath, general weakness. Clinical and laboratory-instrumental aspects of diseases are analyzed. Emphasized the feasibility of early diagnosis, adequate therapy appointment. The aim of the article is to show that only with timely prescribing of pathogenetic therapy it is possible to achieve clinical and laboratory remission and, even, to cease hemodialysis sessions. It was described two clinical cases of microscopic polyangiitis in patients with symptoms of glomerulonephritis with renal failure. Approaches to complex treatment of patients with the use of pathogenetic and the possibility of using renal replacement therapy were discussed. After verifying the diagnosis, all patients started immunosuppression with corticosteroids and cytostatics. It is shown that only with timely prescribing of pathogenetic therapy it is possible to achieve clinical and laboratory remission. Clinical examples demonstrate to physicians that systemic vasculitis can often hide under the «mask» of other diseases and require timely diagnosis and immediate pathogenetic treatment.

Cross-reactivity between allergens and human proteins could have a clinical impact in allergic diseases. Blo t 13 is an allergen from the mite Blomia tropicalis, which belongs to the fatty acid binding protein (FABP) family and has structural homology with human FABPs. This work aimed to map B cell epitopes on Blo t 13 and to identify epitopes involved in cross-reactivity with human heart FABP (FABP3) and adipocyte FABP (FABP4). Sera from 25 patients with house dust mite (HDM) allergy that were sensitized to Blo t 13 were used for testing the reactivity of immunoglobulin E (IgE) and IgG to FABP. The epitope mapping of Blo t 13 was performed using overlapping peptides, and cross-reactivity between Blo t 13 and human FABP was analyzed using human sera and anti-Blo t 13 monoclonal antibodies. IgE antibodies to all FABPs were detected in 14/25 serum samples, and IgG was detected in 25/25 serum samples. The cross-reactivity of Blo t 13 was 42% with FABP3 and 48% with FABP4. Two IgE-binding regions were identified in Blo t 13; one between residues 54 and 72 (the main cross-reacting region) and another between residues 111 to 129. Our results suggest that exposure to the Blo t 13 allergen could induce an auto-reactive response to endogenous FABP in allergic patients sensitized to Blo t 13.

SENSITIZATION TO PET ALLERGENS AMONG THE PATIENTS OF ALLERGOLOGIC CLINIC M. A. Lykova Shupyk National Healthcare University of Ukraine, Kyiv, Ukraine KIA «Forpost», Kyiv, Ukraine Abstract. The aim: to study the prevalence and structure of sensitization to pet allergens among patients of the allergy clinic. Material and methods of research. The results of a multiplex study (determination of specific IgE to 112 allergenic proteins using the ImmunoCap ISAC test system) in 553 patients who came to the immunology and allergology clinic "Forpost" with complaints of respiratory and / or skin manifestations of allergopathology were analyzed. Results. Sensitization to animal allergens was found in 46.1 % of children of group 1 (0–6 years), 62.8 % of children of group 2 (7–18 years) and 42.6 % of adults. Most patients of all ages (96 %) were also sensitized to other (food and / or inhalation) allergens. In all groups of subjects, sensitization to cat allergens prevailed (44.6 % of all animals sensitized to allergens in the group of children 0–6 years, 43.4 % in the group of children 7–18 years, and 42.7 % in the group of adults) and dogs 12.3 %, 6.6 % and 17.7 % respectively). In all age groups, sensitization to the main cat allergen Fel d1 (uteroglobin) prevailed, in children age groups lipocalin also caused a high frequency of sensitization, in particular Can f1 and Fel d4, while in the adult group the frequency of sensitization to prostatic falkrelin canine was in second place. Conclusions. Animal allergens, especially cats and dogs, play an important role in the development of allergic diseases. Most patients with sensitization to one animal species have specific IgE to cat or dog proteins, suggesting that sensitization to horse and mouse allergens may be more frequent due to cross-reactivity. In the structure of sensitization, the most common allergen in all age groups is the major cat allergen Fel d1. In the younger age group, serum albumin is also important, and in adults — prostatic kallikrein Can f5, which should be considered when choosing the appropriate allergen molecules for diagnosis in patients with hypersensitivity to animal allergens before allergen-specific immunotherapy. Allergic disease is a serious problem in practical healthcare. Over the last 40 years there has been exponential growth in the prevalence. According to the world health organization information, allergic diseases are at the 2nd place in prevalence the children, behind the viral infections. Their frequency and severity are increasing. In this regard, the relevance of timely and skilled diagnostic allergopathology is most important. In this study the current state of the question of allergy diagnostics is considered, the international experience is summarized and the approach to the allergy diagnosis based on use of step-by-step identification of a causal and significant factor of allergic reactions is offered. On the basis of the analysis of relevance and the importance for patients of one or the other allergens (taking into account a source of allergens and age of patients) use of a step-by-step allergy diagnostics algorithm is offered. The first step is definition of clinical implications of an allergy. It means direct contact of the phisition with the patient, clarification of its complaints, clinical symptoms, medical history disease. The second step is the confirmation of IgE-dependent mechanism. It involves the using of screening tests that are selected depending on the clinical symptoms and seasonality manifestations (the screening module). The third step is to identify the source of the allergens that are most meaningful for the patient with using test panels (modules). The panels include the most common and clinically relevant triggers of allergic reactions. The fourth step is the search for an individual significant allergens, which were not included in the diagnostic modules. On the fifth step, we plan to conduct component-divided diagnostics and detect the antibodies to unique components of significant allergens. The developed diagnostics algorithm, corresponds to needs of both the adult, and children’s population and provides the personalized approach to the patients.
