The aim: The aim of our research is to study the features of toxic osteomyelitis in drug addicts, their diagnosis and comprehensive treatment, aimed at strengthening motivation for the suspension of the use of psychoactive substances and the elimination of the pathological process in the lower jaw . Materials and methods: The features of toxic osteomyelitis, complicated by abuse, have been studied on 46 patients in the department of oral and maxillofacial surgery of Lviv regional clinical hospital during 2013-2019. Psychoactive substances, used by the patients, varied from homemade drugs “Screw” taken by 32 men (69.5%), synthetic drug amphetamine consumed by 10 men (21,7%) to Subutex used by 4 patients (8,7%) for their pleasure. All the patients underwent comprehensive examination, which included clinical, laboratory, radiological, pathohistological studies. Almost all patients – 41 ( 89.1% ) underwent sequestrectomy. Results: Clinical picture progressed quite rapidly in the form of diffuse destructive-necrotic osteomyelitis of the mandible, which was characterized by a severe, atypical course of the pathological process with permanent inclusion of other additional areas of the lesion of the mandible. The X-ray at this stage showed an increase in destructive processes in the bone. Conclusions: Appropriate surgical tactics and pathogenetic therapy are of great importance for toxic osteomyelitis in drug addicts. Due to the treatment, despite the total destruction of the mandible, it was possible to stop the destructive bone processes and to preserve life for such patients.

Objective This investigation evaluated the results of alveolar bone grafting in patients with complete clefts, comparing secondary alveolar bone grafting (during the mixed stage of dentition) versus tertiary bone grafting (after completion of the second stage of dentition). Design This was a retrospective study. Of 140 osteoplasties, which all were operated according to the same standardized surgical technique, a clinical and roentgenological follow-up investigation was carried out in a collective of 85 osteoplasties. Clinically we searched for oronasal fistulae, assessed the periodontal status, determined the extent of the gingival attachment in the area of the osteoplasty, and searched for vertical growth disturbances in the area of the osteoplasty. Roentgenologically the height of the alveolar bone in the former clefted area was ascertained. Setting Records were obtained from a clinical and radiological study of the Department of Oral and Maxillofacial Surgery of the Medical University of Hannover (Germany). Interventions All patients were operated with the same standardized surgical method. The osteoplastic bridging of the alveolar cleft was performed via a vestibular gingival marginal incision and exclusively by grafting of cancellous bone from the iliac crest. Results The best results of alveolar bone grafting in cases of secondary osteoplasty were obtained when the lateral incisor or canine had grown into the transplant and had led to a functional stress of the transplanted bone. Approximately good results were to be found in tertiary osteoplasty when the transplanted bone had been stressed functionally through a dental implant. Comparing the secondary with the tertiary osteoplasty, there was a trend of lower resorption in secondary osteoplasty. Conclusions Secondary osteoplasty should represent an integral component of any concept for the comprehensive treatment of patients with cleft lip and palate.

Of the total number of animal diseases, 70–80 % are non-communicable diseases, and more than 50 % are surgical diseases, which cause significant economic damage to the livestock industry due to reduced productivity and premature culling of livestock. Irrational, unbalanced in terms of macro- and microelements, vitamins, carbohydrates feeding leads to metabolic disorders. At the same time, deep destructive-degenerative changes in bones, joints, ligaments and tendons are registered, horn formation in the hooves is disturbed and a less stable horn is produced. Omission in feeding unsatisfactory housing conditions and inadequate care of animals. When such conditions are often supplemented, the outer cover of animals, in particular the hooves, is affected first of all. In the event of inflammatory and dystrophic processes at the base of the skin of the hooves there is a violation of tissue (tubular) and cellular structure. The horn of such hooves undergoes changes in the form of parakeratosis and deformation; this contributes to the spread and accumulation of fungal flora, which shows its pathogenic action mainly due to enzymatic activity and toxic products, which characterizes these microorganisms as a factor in the destructive processes of the hoof horn. With reduced mechanical stability and destroyed by microscopic fungi of hoof horn is more likely to develop aseptic or purulent pododermatitis. Here, the fungal flora is not only a contributing factor in the occurrence of diseases, but also as one of the factors complicating the course of the disease. That is why timely detection and effective treatment of ungulomycosis prevents the complication of the pathological process, which can even lead to culling of animals. The essence of treatment of superficial ungulomycosis is reduced to functional cleaning of hooves and their antiseptic treatment in the form of fungicidal powders, sprays, hoof baths. In deep ungulomycosis, and, moreover, in combination with purulent pododermatitis, treatment includes local anesthesia, orthopedic treatment, antiseptic treatment with isolation, pathogenetic therapy. Prevention of ungulomycosis is based on knowledge of the etiology of the disease and the pathogenetic mechanisms of its development.  Its essence is to form in animals a strong hoof horn, prevent its destruction and eliminate favorable conditions for the reproduction of keratomycetes. This includes proper feeding, proper housing conditions and appropriate animal care.

Reconstruction of the mandible after severe trauma is one of the most difficult challenges facing oral and maxillofacial surgery. The mandible is an essential element in the appearance of the human face that gives the distinctive shape of the face, holds. This paper aims to propose a methodology that allows the surgeon to perform virtual surgery by investing engineering programs to place the implant by default and with high accuracy within the mandible based on the patient's medical data. The current study involved a 35-year-old man suffering from a traffic accident in the mandible with multiple fractures of the facial bones. Basically, an identification of the steps required to perform virtual surgery and modeling images from the CBCT technology has been done by using the software proposed in the research. The implant model is designed as a mesh model, allowing the patient to return to a normal position. Moreover, an application of FEA procedures using the Solidworks simulation software to test and verify the mechanical properties of the final transplant.
