Imagine working in an environment where all patients undergoing mechanical ventilation are alert, calm, and delirium free. Envision practicing in an environment where nonvocal patients can effectively express their need for better pain control, repositioning, or emotional reassurance. Picture an intensive care unit where a nurse-led, interprofessional team practices evidence-based, patient-centered care focused on preserving and/or restoring their clients’ physical, functional, and neurocognitive abilities. A recently proposed bundle of practices for the intensive care unit could advance the current practice environment toward this idealized environment. The Awakening and Breathing Coordination, Delirium Monitoring and Management, and Early Mobility (ABCDE) bundle incorporates the best available evidence related to delirium, immobility, sedation/analgesia, and ventilator management in the intensive care unit for adoption into everyday clinical practice.

ABSTRACTObjective: to analyze nurses' knowledge and practice about "quality nursing care" in the Intensive Care Unit. Method: this is a qualitative, descriptive, observational study carried out with ten effective ICU nurses. A semi-structured interview script was applied, and the data analyzed through Content Analysis. Results: it is revealed that the majority is female, with a mean age of 44 years and a mean age of 16 years. Two categories were generated, one of which refers to the knowledge of nurses about the quality of nursing care and another about the practice of nurses. Conclusion: it was observed that nurses attributed the quality of care to patient-centered care in a humanized, holistic and safe way. In terms of applicability in practice, routine activities were considered as processes that guarantee quality care. It contributes, through the results, to planning and improvement of Nursing care. Conclusão: observou-se que os enfermeiros atribuíram a qualidade da assistência ao cuidado centrado no paciente de forma humanizada, holística e segura. Consideraram-se, quanto à aplicabilidade na prática, as atividades desenvolvidas na rotina como processos que garantem uma assistência de qualidade. Contribui-se, pelos resultados, para planejamentos e melhoria da assistência de Enfermagem. Objetivo: verificar a percepção dos acompanhantes sobre dispositivos invasivos em Unidade de Terapia Intensiva Pediátrica. Metodologia: estudo do tipo descritivo com abordagem qualitativa realizado em Unidade de Terapia Intensiva Pediátrica de um hospital de referência em emergência traumato-ortopédico de Fortaleza, com dezessete acompanhantes, no período de julho a agosto de 2016. Utilizou-se para coleta de dados o formulário, analisados por meio da análise de conteúdo. Resultados: foram organizados em três categorias, intituladas caracterização dos acompanhantes participantes da pesquisa, sentimentos de estar acompanhante: ansiedade e medo, e significação dos dispositivos invasivos para o acompanhante. Conclusão: conclui-se que os acompanhantes que receberam informes sobre os dispositivos invasivos souberam identificar e discorrer sobre sua utilidade de forma parcial ou completa. Results: were organized in three categories, entitled characterization of accompanying participants of the research, feelings of accompanying: anxiety and fear, and significance of invasive devices for the companion. Conclusion: it was concluded that the companions who received reports on the invasive devices were able to identify and discuss their usefulness partially or completely. Since the onset of the COVID-19 pandemic, telehealth technologies have become critical to providing family and patient-centered care. Little is known about the impact of these technologies on parent stress levels in the Neonatal Intensive Care Unit (NICU). We sought to determine the impact of bedside web cameras on stress levels of parents in the NICU in order to work toward interventions that might improve family-centered care. A validated survey, the Parental Stress Scale NICU, was administered to parents of babies admitted to the Children’s Hospital of Philadelphia Neonatal/Infant Intensive Care Unit on days 7 to 10 of hospitalization. Parents were also asked if they used the available AngelEye Camera while their baby was hospitalized. Stress levels were analyzed for associations with the use of the bedside cameras. Parents who reported using the bedside camera also reported lower levels of stress relating to being separated from their babies. Bedside web camera interventions may hold potential for reducing parent stress related to separation from their babies, especially in the setting of a global pandemic.

Objective To investigate management and implementation of the “awakening and breathing trials, choice of drugs, delirium management, and early exercise/mobility” (ABCDE) bundle in the pediatric intensive care unit (PICU) in southwestern China Methods A self-designed questionnaire for determining implementation of the ABCDE bundle was distributed to healthcare professionals in the PICU. Multiple linear regression was used to analyze results. Results A total of 270 questionnaires were collected. There was no significant difference in the awareness of the ABCDE bundle rate among Sichuan, Guizhou, and Yunnan workers. Only dynamic adjustment of drug dose accounted for more than half (55.5%) of “frequent implementation” and “general implementation”, followed by implementation of sedation assessment, pain assessment, and spontaneous breathing trials (46.4%, 39.3%, and 35.6%, respectively). A total of 80.4% of healthcare professionals never performed screening of delirium. Multivariate analysis showed that the healthcare professionals’ scores of ABCDE bundle behavior significantly differed regarding awareness of the ABCDE bundle, years of work at the hospital, the region of hospitals, and occupational category. Conclusion Implementation of the ABCDE bundle in the PICU in southwestern China is not sufficient. Existing problems need to be identified and a standardized sedation and analgesia management model needs to be established.
