Corneal topography and wavefront data changes in dry eye

Purpose.To conduct a comparative analysis of the effectiveness of assessing the stability of the tear film using corneotopography and the classic Norn test in patients with myopia and myopic astigmatism before keratorefractive surgery. Material and methods. The study included 26 patients planned for keratorefractive surgery for myopia and myopic astigmatism, aged 18 to 47 years. 7 people there was a mild degree of myopia, in 14 people – medium degree, in 5 people – high degree. The average degree of corneal astigmatism was 2.25 diopters. The stability of the pre-corneal tear film was assessed using computed keratotopography and Norn's test – the tear film rupture time (TFRT) was deter- mined. A standard questionnaire was also used to assess the severity of symptoms of dry eye syndrome. A comparison was made of the indicators of the stability of the tear film obtained by the methods of corneotopography and the Norn's test, with their subsequent comparison with the data of the questionnaire. Results. In 17 patients (65.4%), the TFRT index corresponded to the age norm. In the remaining 9 people (34.6%), indicators of stability of the tear film were reduced both by corneotopography (12±4 sec) and by Norn's test (13.5±4.5 sec), and there were also subjective signs of dry eye syndrome according to data questionnaire. The data turned out to be comparable. Conclusions. 1. Among the total population of patients, 34.6% were persons with impaired tear film stability and subjective manifestations of dry eye syndrome. 2. Assessment of the stability of the tear film, determined using the method of corneal topography, showed a high degree of comparability with the results of Norn's test. A significant advantage of the keratotopography method is its non-invasiveness. 3. It is necessary to pay attention to the condition of the ocular surface and identify risk groups to avoid chronicity or aggravation of existing disorders. Background. Today, the urgent problem is coronavirus disease 2019 pandemic in the whole world. Safety measures such as the use of masks, distance learning have been implemented. However, ophthalmologists, as well as the general population, should know that a face mask together with prolonged use of digital devices leads to an increase in the number of dry eye cases in many patients. The purpose was to study the frequency of dry eye disease in students with myopia. Materials and methods. The study involved 96 medical students diagnosed with myopia taken by random sampling, who were examined by an ophthalmologist. Apart from the standard ophthalmologic examination, all patients underwent the evaluation of the stability of the precorneal tear film by means of corneal topography, and Norn test (tear break-up time (TBUT)). There was also used a standard questionnaire designed to assess the severity of dry eye disease symptoms (Ocular Surface Disease Index). Results. The corneal topography of the 47 patients did not demonstrate any reduction in TBUT. The average time of the concentric ring remained stable for 20.2 ± 3.0 seconds. But 49 individuals exhibited signs of tear film instability over time compared to 47 patients without TBUT impairment. In 47 people without impairments, corneal topography demonstrated that this indicator in Norn test varied from 25 to 18 seconds, i.e. was within the age norm (21.5 ± 3.5 seconds on average). In 3 patients with a minimum TBUT on keratotopography, its value during the Norn test was 9 seconds. In 36 patients with reduced TBUT according to both methods, there was a weak degree of dry eyes, in 10 — moderate, in 3 — severe. The most common complaints were a sandy, gritty sensation in the eyes, visual discomfort when working at a computer and in windy weather, and with prolonged use of a protective mask. Conclusions. Among students who were on distance learning, 51.04 % of individuals have objectively confirmed dry eye disease. People wearing contact lenses have a more pronounced degree of dry eye, so it is mandatory for ophthalmologists to prescribe lubricating eye drops.

Abstract Purpose: To investigate the effect of Botulinum neurotoxin-A (BTX-A) treatment on dry eye symptoms, tear meniscus, corneal topography and corneal aberrometry in patients with benign essential blepharospasm (BEB) and hemifacial spasm (HFS).Materials and Methods: This prospective study comprised of 32 eyes from 8 BEB and 24 HFS patients. Tear meniscus height (TMH) and depth (TMD), tear break-up time (TBUT), corneal fluorescein staining score (CFSS), Schirmer I test, ocular surface disease index (OSDI) score, corneal topography [corneal power of flat axis (K1), corneal power of steep axis (K2), mean corneal power (Km), astigmatism and thinnest pachymetry] and anterior corneal aberrometry [spherical aberration (SA), vertical coma (vcoma), horizontal coma (hcoma), higher order root mean square (hRMS) and total RMS] were evaluated before BTX-A treatment and 3 weeks after BTX-A treatment.Results: 3 weeks after BTX-A injection TMH (265.9±116.4 µm vs. 347.1 ± 172.6 µm; p < 0.001) and TMD (178.1 ± 57.7 µm vs. 221.4 ± 90.3 µm; p < 0.001) showed a significant increase. TBUT, CFSS, Schirmer I test values were similar (p > 0.005). OSDI scores decreased significantly from 27.8 ± 26.1 to 18.4 ± 19.8 (p = 0.04). K2 (44.1 ± 1.7 D vs. 43.9 ± 1.7 D p = 0.009), Km (43.7 ± 1.6 D vs. 43.6 ± 1.6 D p = 0.025) and astigmatism (0.9 ± 0.6 D vs. 0.7 ± 0.5 D; p = 0.02) values improved significantly at 3 weeks. Pachymetry and aberrometric values did not change significantly.Conclusion: BTX-A injection increases tear meniscus and decrease symptoms related to dry eye disease in BEB and HFS patients. It decrease astigmatism and keratometry values but it does not cause a significant change in corneal aberrations.

Purpose: To compare the corneal epithelial thickness profile in patients with dry eyes and keratoconus suspect with normal healthy eyes. Methods: The study involved 120 eyes with an age range from 19 to 30 years. Forty eyes had normal corneal topography and no dry eyes. Forty eyes had dry eyes but had normal corneal topography. The last 40 eyes were keratoconus suspect and had no symptoms or signs of dry eyes. Results: Central epithelial thickness was not different statistically for all eyes. ( p-value: 0.1). The superior epithelial thickness was 53.5 µm ±3.1 in the control group, 53.4 µm ±3.5 in the dry eye group, and 53.6 µm ±2.8 in the keratoconus suspect group. No statistically significant difference was found ( p-value = 0.7). The inferior epithelial thickness was 55.7 µm ±3.5 in the control, 57.2 µm ±3.19 in the dry eyes, and 52.2 µm ±3.12 in the KC suspects. There was inferior thickening in the dry eyes and thinning in the KC suspects and this was statistically significant ( p-value < 0.01). Minimum epithelial thickness was 52.8 µm ±2.91 in the control and 53.2 µm ±3.51 in the dry eyes and it was located superiorly for both groups. In the KC suspects, the minimum thickness was 52.3 µm ±3.19 and was located inferiorly. Conclusion: In our study the epithelium appears to be thicker inferiorly in dry eyes and thinner in KC suspects. Displacement of thinnest location on epithelial map may be a helpful early sign of keratoconus. However, follow-up study is necessary to confirm the thinnest location displacement helped in this diagnosis.
