The female condom (FC), also known as the femidom, has been on the market since 1993, however, its use remains limited in many parts of southern Africa, including in Eswatini (formerly Swaziland). There is a dearth of literature on the reasons for the limited use of the FC, especially from the perspective of health science students who would otherwise be expected to be knowledgeable about and have favourable attitudes to it. The aim of this study was to explore and describe the barriers to FC use among undergraduate health science students at a selected tertiary institution in Eswatini. A qualitative, exploratory descriptive study was conducted among nine conveniently sampled, unmarried undergraduate students at a selected tertiary institution in the Hhohho region in Mbabane, Eswatini. Responses to an unstructured interview guide, using in-depth interviews were analysed thematically following Creswell’s steps of qualitative data analysis. Five themes emerged from the data: (1) inadequate knowledge about the FC, (2) the FC hinders sexual pleasure, (3) insertion of the FC is time-consuming and uncomfortable, (4) the FC is bigger than the vagina, and (5) fear of being labelled “promiscuous”. Generally, the participants stated that they did not use the FC because of societal myths. Therefore, there is a need to strengthen health education campaigns for the femidom to clear the myths and misconceptions that limit its use.

Background: Testicular torsion is an organ-threatening surgical emergency with a limited timeframe for intervention. Objective: To identify the delays to surgical exploration of patients with an acute scrotum in a tertiary hospital to prevent adverse outcomes associated with this time critical emergency. Methods: A retrospective review of medical records for all patients who underwent scrotal exploration for acute scrotal pain in a tertiary hospital in regional New South Wales between January 2008 to December 2018 was performed. Results: Retrospective review identified 242 patients, of whom 161 had testicular torsion and 56 resultant orchidectomies. No statistically significant difference in pre-hospital delays between paediatric or adult populations was found. The average time from presentation to theatre was 4 h 36 min. Patients who were delayed > 6 h from presentation to surgical exploration had significantly increased rates of orchidectomy. Delays that significantly affected rates of orchidectomy were transfer from peripheral sites, late presentation, misdiagnosis and representation after discharge. Conclusion: The most common reason for delay was diagnostic error with the patient later re-presenting to hospital. Patient transfer from the primary hospital to a tertiary institution and subsequent delayed surgical exploration also contributed to significantly higher rates of orchidectomy. Level of Evidence: 3

Abstract Background Fomites have long been known to play a key role in the spread of disease causing agents. Hospital-associated fomites in particular have been linked with transmission of members of the Enterococci sp which are key human pathogens. Few studies have explored the role non-hospital door handles might play as potential sources of these isolates. This study therefore set out to explore this role. Results A total of hundred toilet and office door handles in a tertiary institution in Rivers State, Nigeria, were sampled using the swab and rinse method. The presence and drug susceptibility of Enterococcus was determined using the selective bile esculin agar (BEA) and standard microbiological methods. Growth on BEA was observed in 71% of cases, with more growth (38/50, 76%) observed from toilet door handles. Only 35% of samples produced the characteristic black pigmentation associated with Enterococcus sp. Six different bacterial groups were identified from this subset with Enterococcus sp. making up only 14% (5/35) of the isolates. All (100%) Enterococci were isolated from toilet door handles. Antibiotic susceptibility testing revealed very high levels of resistance (80–100%) against 75% of the test antibiotics. An analysis of the antibiotic resistance pattern of each isolate revealed 11 unique antibiogram patterns. Only 2 of these patterns were associated with the enterococci, with majority (4/5) exhibiting resistance to Augmentin (AUG), Ceftazidime (CAZ), Ceftriaxone (CTR), Cefuroxime (CRX), Cloxacillin (CXC), Erythromycin (ERY), Gentamicin (GEN), Ofloxacin (OFL) (antibiogram of AUG–CAZ–CRX–CTR–CXC–ERY). None of the enterococci, however, was resistant to vancomycin. Conclusion This study reports low level contamination of door handles by enterococci. Identical antibiogram patterns linked with majority of the enterococci could however point at the occurrence of a single clone perhaps indicating single source contamination. Reports of high levels of ampicillin resistance among these isolates are problematic as ampicillin–gentamicin combination is the treatment of choice for nosocomial enterococci pathogens.
