ObjectivesTo evaluate the impact of a patient-specific national programme targeting older Australians and health professionals that aimed to increase use of emollient moisturisers to reduce to the risk of skin tears.DesignA prospective cohort intervention.ParticipantsThe intervention targeted 52 778 Australian Government’s Department of Veterans’ Affairs patients aged over 64 years who had risk factors for wound development, and their general practitioners (GPs) (n=14 178).Outcome measuresAn interrupted time series model compared the rate of dispensing of emollients in the targeted cohort before and up to 23 months after the intervention. Commitment questions were included in self-report forms.ResultsIn the first month after the intervention, the rate of claims increased 6.3-fold (95% CI: 5.2 to 7.6, p<0.001) to 10 emollient dispensings per 1000 patients in the first month after the intervention. Overall, the intervention resulted in 10 905 additional patient-months of treatment. The increased rate of dispensing among patients who committed to talking to their GP about using an emollient was six times higher (rate ratio: 6.2, 95% CI: 4.4 to 8.7) than comparison groups.ConclusionsThe intervention had a sustained effect over 23 months. Veterans who responded positively to commitment questions had higher uptake of emollients than those who did not.

AbstractMany studies raise doubts about the effectiveness of the institutions, programmes and instruments that shape the Chinese national innovation system. This article scrutinizes central–local interactions in the national Torch Programme that has governed a large group of high-technology zones since 1988. The Torch Programme's procedural practices challenge widely shared assumptions about the dirigiste character of Chinese innovation policy. It combines centralized definition of programme objectives with extensive local implementation experiments. As three case studies demonstrate, bottom-up policy innovations are effectively fed back into national programme adjustments and into horizontal policy diffusion. The array of organizational patterns and promotional instruments that emerges from competitive “experimentation under the shadow of hierarchy” (ESH) goes way beyond what could have been initiated from top down. We hypothesize that the procedural strengths displayed in the Torch Programme may provide better indicators of future innovative potential in China's high-technology zones than retrospective statistical indices and benchmarks that are derived from OECD experience.

A cross-sectional population-based survey on the occurrence of lymphatic filariasis was carried out in the municipality of Cabo de Santo Agostinho, Pernambuco, Northeast Brazil. 7,650 individuals of both sexes were examined (from 1,416 households) of whom six tested positive for microfilaria according to the thick blood diagnostic test. The age of the individuals examined varied from 0 to 98 (averaging 26.6 years), whilst the age of the microfilaria-positive individuals varied from 11 to 29, averaging 22.5 years. Five of the six positive cases were male. These cases were residents of the following areas: Pista Preta (one case); Ponte dos Carvalhos (four cases); and Pontezinha (one case). This last case from Pontezinha was autochthonous. Of the individuals examined, 109 (1.4%) cited complaints relating to filariasis. These results suggest that filariasis is being transmitted in the municipality of Cabo de Santo Agostinho, a finding that establishes the need to carry out disease control activities, and to collaborate with the planning of the national programme for the elimination of filariasis.

Airports similarly to other companies are certified in compliance with the International Standardization Organization (ISO) standards of products and services (series of ISO 9000 Standards regarding quality management), to coordinate the technical side of standardizatioon and normalization at an international scale. In order for the airports to meet the norms and the certification requirements as by the ISO they are liable to undergo strict audits of quality, as a rule, conducted by an independent auditing organization. Focus of the audits is primarily on airport operation economics and security. The article is an analysis into the methodology of the airport security audit processes and activities. Within the framework of planning, the sequence of steps is described in line with the principles and procedures of the Security Management System (SMS) and starndards established by the International Standardization Organization (ISO). The methodology of conducting airport security audit is developed in compliance with the national programme and international legislation standards (Annex 17) applicable to protection of civil aviation against acts of unlawful interference.
