Ultraviolet water sterilizer is a long cylindrical device used for the sterilization of water into  drinkable water. Ultraviolet water sterilizer has two main parts: an adapter and UV tubes. The  adapter serves to drain ration electricity in accordance with the voltage and current required by  ultraviolet water sterilizer. Tubes UV is containing lamps germicides where light is equipment  major unruk make the water drinkable and the water will flow along the ultraviolet rays, so that the  water flowing in the tube is passed through the sterilization process with ultraviolet rays so that the  water unfit for consumption. The problems that the authors take in this study are the factors that  affect the declining quality of sterilization, the impact of what happens when the quality of  sterilization decreased, and what efforts made to keep the quality of sterilization is not decreased.  The method used is the method of fault tree analysis, a method to find the root of the problem  by using a fault tree and simple logic gates to help determine the basic problem. Fault tree analysis  method consists of a top event, intermediate event, and basic event where all three events are  interrelated to form the fault tree that can be seen cause and effect. Data collected through  observation, interviews, and literature directly to the subject related to the factors, the impact, as  well as the efforts of the decline in the sterilization process to the sterilizer in order to improve the  quality of drinking water.  The cause of the decline to the sterilizer sterilization process in order to improve the quality  of drinking water in MT. Dragonaria that dirty filters, dirty water tubes, and the dirty germicidal  lamp or berdampaknya resulting in less fresh water, complaint against the company, as well as the  ship operational cost is reduced. Therefore made some efforts to improve the quality of the results  of sterilization including cleaning filters, cleaning water tubes, and also clean the germicidal lamp,  as well as auxiliary engine checks periodically so that the quality of sterilization can be increased.

This paper examines the reliability of the operation of cableway systems in Slovenia, which has major impact on the quality of service in the mountain tourism, mainly in wintertime. Different types of cableway installations in Slovenia were captured in a sample and fault tree analysis (FTA) was made on the basis of the obtained data. The paper presents the results of the analysis. With these results it is possible to determine the probability of faults of different types of cableways, which types of faults have the greatest impact on the termination of operation, which components of cableways fail most, what is the impact of age of cableways on the occurrence of the faults. Finally, an attempt was made to find if occurrence of faults on individual cableway installation has also impact on traffic on this cableway due to reduced quality of service. In case of fault tree analysis of large complex system, the probability of bottom event in dynamic fault tree is uncertain in some cases. To address the problem, the paper presented a dynamic fault tree analysis method based on fuzzy set computation. The method separates logic attributes and timing attributes of dynamic logic gates. It can convert dynamic fault tree into static fault tree not considering timing constraints and obtain minimum cut set of static fuzzy fault tree with set operations, then the concept of minimum cut set is extended to dynamical minimum cut sequence. Thus, the dynamic fault tree was analyzed in both qualitative and quantitative aspects, which solve the problem that it is difficult to assign value of event probability in previously process.

Concepts and methods used to analyze is a statistical process control (SPC) which includes Pareto diagram, fraction nonconformities, flow charts and fishbone diagrams and management tools (fault tree analysis). Statistical Process Control (SPC) is one of the methods, which includes Pareto charts, fraction nonconformities, flow chart, and fishbone diagram and also management tools (fault tree analysis). SPC is useful to find the facts from the problems and factors that affect the quality of the products, while fault tree analysis is useful to analyze each of the production process.

In this paper, a fault diagnosis method for spacecraft based on telemetry data mining and fault tree analysis was proposed. Decision trees are constructed from the history telemetry data of the spacecraft, and are used to classify the current data which is unknown whether it is fault. If there is a fault, the fault tree method will be used to analyze the fault reason and the impact on the spacecraft system. This method can effectively solve the problem of diagnostic knowledge acquisition. We design and construct a fault diagnosis expert system for spacecraft based on this diagnosis method. An experiment is presented to prove the effectiveness and practicality of the expert system.

The approach taken by this paper is expected to motivate the Furan No Bake (FNB) sand casting foundries to use a standardized system of classification to show unwanted casting defects for highly effective failure analysis. It will also motivate foundries to develop new systems to quantify process parameters relating to the defects. Any reduction in the scrap and rework also highly influences the impact on environment of industry. This paper covers the various aspects of a systematic approach for understanding and quality cost system development in cast iron foundries through Fault Tree Analysis (FTA).

Abstract. Quality control is activities to be able to improve, maintain and improve the quality of the products produced. Because it controls the quality, it is very important to improve the quality of the products produced. Can improve the quality expected by all elements of the company can be obtained as expected. CV. X is a company established in the city of Bandung in 2003, Jakarta, Bogor and Central Java. The company is engaged in the production of various laboratory equipment such as erlenmayer, test tubes, drip pipettes, measuring flasks and measuring cups. The number of pumpkin production in 2016 was 2218, in 2017 as many as 2429 and in 2018 as many as 2871. Of the total production, the level of defect of pumpkin products measured an increase in 2016 of 3.5%, in 2017 amounted to 4.3% and in 2018 of 5.3%. The level of product defect exceeds the target or determined by the company for smoothness of 2%, it is necessary to improve the quality of product defects in the flask. Fault Tree Analysis (FTA) and Failure Mode and Effect Analysis (FMEA) can be used to analyze and provide solutions to improve the quality of defective products. Based on the processing and analysis of the data obtained on the type of defective form is not appropriate is the operator who is not careful with the value of RPN 120, the cause of screen printing defects that consider cleanliness, less careful, dirty work area, the user uses the operator to reverse the setting with the RPN value 144, causing damage back / broken ie operator settings change, less careful and less tidy work station with a value of RPN 240. Abstrak. Pengendalian kualitas merupakan aktivitas-aktivitas untuk dapat menjaga, mempertahankan dan dapat meningkatkan kualitas dari produk yang dihasilkan. Oleh karena hal tersebut pengendalian kualitas memiliki peranan sangat penting dalam terciptanya produk yang berkualitas. Sehingga tujuan yang diharapkan oleh seluruh elemen perusahaan dapat tercapai. CV. X merupakan perusahaan yang didirikan di Kota Bandung pada tahun 2003, berbasis usaha kecil menengah dengan pangsa pasar meliputi wilayah Bandung, Jakarta, Bogor dan Jawa Tengah. Perusahaan ini bergerak dalam produksi berbagai macam alat-alat laboratorium seperti erlenmayer, tabung reaksi, pipet tetes, labu ukur dan gelas ukur. Jumlah produksi labu ukur pada tahun 2016 adalah 2218, pada tahun 2017 sebanyak 2429 dan pada tahun 2018 sebanyak 2871 . Dari jumlah produksi tersebut, tingkat kecacatan produk labu ukur mengalami peningkatan yaitu pada tahun 2016 sebesar 3.5%, tahun 2017 sebesar 4,3% dan tahun 2018 sebesar 5,3%. Tingkat kecacatan produk sudah melewati ambang batas dari yang diharapkan perusahaan sebesar 2 % sehingga diperlukan upaya unuk mengatasi masalah kecacatan produk pada labu ukur . Fault Tree Analysis (FTA) dan Failure Mode and Effect Analysis (FMEA) dapat digunakan untuk menganalisis dan memberikan solusi terhadap perbaikan kualitas produk cacat. Hasil dari data yang telah diolah dan dianalisa, faktor penyebab kecacatan bentuk tidak sesuai yaitu operator kurang hati-hati dengan nilai RPN 120, penyebab cacat sablon yaitu perhatian terhadap kebersihan kurang, kurang hati-hati, stasiun kerja kotor, intensitas penggunaan alat tinggi, kurangnya pengawasan dan operator mengabaikan setting dengan nilai RPN 144, penyebab cacat retak/pecah yaitu operator mengabaikan aturan setting, kurang hati-hati dan stasiun kerja kurang rapi dengan nilai RPN 240.
