ABSTRACT Complications and pitfalls are unavoidable sequel of any surgical procedure. Although advances have been made in surgical techniques and fixation devices, these are far from ideal. The aim of trauma surgeon is to restore the patient to pre-injury form and function, but it may not be possible in each and every case. Treatment failure and poor outcome in a case of severe maxillofacial trauma is not uncommon. This is mainly dependent upon the extent of injury, delay in management, improper diagnosis, wrong treatment plan, poor execution of a treatment plan, wrong choice of fixation devices, inexperience of the surgeon and inability to seek help from other specialists, etc. The objective of this article is to discuss and analyze common complications and pitfalls along with the various methods and modalities to prevent and manage it. ABSTRACT Aim: To describe an approach for the diagnosis and management of patients presenting with crooked nose. Background Patients with crooked nose suffer from functional ailments, most significant nasal obstruction, as well as esthetic concerns which may impact their self-image as well as others’ perception of them. As such, management of the crooked nose is an especially challenging task in that it demands careful attention to both nasal function and appearance. There are a plethora of surgical techniques which may be utilized, and the surgeon's approach must be tailored to each individual patient's presentation. Technique Accurate diagnosis is imperative for a successful outcome and relies on a thorough history and careful physical examination. Surgical intervention may be addressed by either an endonasal or open approach based on the location and severity of the deformity. It is helpful to break down the nose in vertical thirds when planning your surgical approach. Different techniques are used to address the upper third comprising the nasal bones, middle third composed of the upper lateral cartilages and septum, and the lower third composed of the lower lateral cartilages and caudal septum. Conclusion Management of the crooked nose is a technically challenging endeavor that highlights the need to address both form and function for a successful outcome. Effective treatment can significantly improve a patient's quality of life leading to a high level of satisfaction for both the patient and the surgeon. One of the main advance in orthopaedic surgery domain has a goal to investigates the safest and harmless method in surgical procedures. Less complications means a better outcome of surgery. One of the commonest risks at orthopaedic surgery is central and peripheral nerve injury. The modality of Intraoperative Neurophysiologic Monitoring (IONM) which act to limit the risk of nerve injury during operative procedure through the evaluation of nerve integrity and function enable the surgeon to decrease injury to the nerve associated with orthopaedic surgical procedure in the operating room. This article aims to explain and describe the latest modality of IONM, its basic concept and its function at surgery. The last part of the article discussed about orthopaedic surgical techniques which use IONM. The authors hope that this article will enhance the knowledge of all the readers about IONM. This article was written based on literature study searched at Google Scholar, Medline and PubMed. The references were taken from a relatively up to date study ranging from 2013-2018. The article was selected according to the authors inclusion criteria and six articles was chosen as the references for this review. As a conclusion, IONM has an important role to increase successful rate of surgery through minimizing nerve injury risk during surgical procedure.

The increase in the occurrence of LDA in cows stems from the increase in the milk yields from cows. The treatment is based on a surgical method, and its effectiveness has an economic effect. The article below presents progress made in identifying the nature of LDA over the last 50 years. Also included is a critical assessment of contemporary and accepted LDA treatments. The surgical procedure has two aims: repositioning and fastening the abomasum directly to the abdominal wall, which prevents the recurrence of this disease. Over the course of the last 50 years LDA surgery has evolved from the laparotomy method to laparoscopy. This latest method has become more readily available and popular. It is characterized by less invasive surgical techniques and faster recovery, as well as lowering the cost of treatment. However, laparoscopy cannot treat every case. Complications or technical difficulties during an operation sometimes force doctors to finish the surgery by laparotomy. To avoid the problem of having to change the technique during the operation, doctors should choose the most appropriate method for each particular case.

Abstract Background The B-Lite® lightweight breast implant (LWBI), weighs ~ 30% less than traditional silicone implants, while maintaining an equivalent size, form and function. The LWBI thus places less stress on breast tissues, preserves tissue stability and integrity over time, reducing weight-related complications and reoperation rates. Objectives To assess the long-term (over 5 year) safety and performance of the LWBI in primary and revision augmentation procedures. Methods Retrospective single-center, single surgeon analysis of prospectively collected data, was performed on 827 consecutive primary and revision augmentation patients operated between December 2013 and January 2019. 1653 implants (250-835 cc, mostly round, textured, extra high-profile) were implanted using standard surgical techniques. Direct physician-to-patient follow-up ranged from 6 to 67 months. Chart data on reoperations and overall complications as well as patient and surgeon satisfaction, were analyzed. Results The 5 year Per Patient Kaplan–Meier reoperation free rate was very high (97.1%). Only 2 out of 5 total cases of capsular contracture grade III required reoperation (KM rate 0.2%, CI- 0.1-1.0). No cases of rupture or BIA-ALCL were recorded. 94.9% of patients rated the aesthetic outcome, and 95.5% of patients rated the natural look and feel of their breasts, at 4-5 (Satisfied-Very Satisfied). Similarly, the surgeon rated 4-5 on 95.4% of the patients’ aesthetic outcomes. Conclusions The extremely favorable safety profile, high patient and surgeon satisfaction, and inherent benefits of reduced weight, in the largest known study of B-Lite® implant surgeries, should make the LWBI a strongly considered strategic alternative to traditional implants.

Various surgical techniques have been described for the release of syndactylized fingers. In our experience, the omega flap technique, which includes a dorsal truncated flap and an anchor incision on the volar side, stands out as a good technique to release syndactyly. Incidentally, in symbrachydactyly also, the fused digits can be released using this technique. Despite this, we could find no reference in the recent years. We would like to stress the ease and importance of this technique, hoping many practicing hand surgeons will benefit from this. Our purpose was to revisit this technique and expose it to the younger generation of hand surgeons. We have operated on 20 cases of syndactyly of different types—simple, compound, and complex—and 5 cases of symbrachydactyly. In all cases, the omega flap on the dorsum and anchor incision on the volar aspect of the finger forming 2 lateral palmar flaps were used. The release of syndactyly was satisfactory in all patients. There was no flap necrosis. None of these cases have required secondary surgery because the primary releases were adequate. Release of syndactyly had been a problem for centuries. Awareness of the disability was insufficient in earlier days; currently, they seek early medical care. The release should be complete. These children must be able to achieve the form and function of the hand, and additionally precision to work. We believe that the use of omega flap and anchor flap is a good procedure for syndactyly release.

ABSTRACT Intraoral trauma may occur in isolation or in combination with injury of the other parts of the body. Once the life-threatening phase of trauma is over, the primary need for the patient is to restore the form and function of the tissues lost due to trauma. In the oral and maxillofacial region, esthetic consideration does sometimes supersede the need of function. Background: Each organ serves a specific function in the human body. Congenital absence or loss of even minor parts of it can pose extreme psychological trauma in addition to the functional impairment and esthetic issues encountered by the individual. Amputation of complete or a part of phalange of hand is frequently encountered either due to trauma or necrosis. Restoration of such defects become mandatory to improve general form and function combined with the esthetic requirements of the patient. It can either be done by microvascular reconstruction or prosthetic rehabilitation. The latter becomes the means of choice in cases where the former is either not possible, unavailable, unsuccessful, or unaffordable. Case Report: A case report has been presented of a 60 years old patient with an old traumatic partially amputated index finger. Treatment Plan: A custom-made glove-type prosthesis was fabricated using silicone elastomer. Conclusion: A ring is provided for better esthetics and special adhesive is recommended for retention of the prosthesis.

Television drama is an important tool to present hypothetical scenarios and imagine various ways to deal with them, while testing the viability of ethical theories that could guide moral judgements and practical decisions made in real life. Buffy the Vampire Slayer (1997-2003) left an important legacy in Popular Culture captivating viewers worldwide and still being relevant 20 years later. The aim of this article is to revisit Buffy’s Season 4 and analyze it through a libertarian perspective. Over this season, a great number of relevant subjects are discussed, such as the form and function of the state, its relationship with society, the subversion of public authorities and the morality of law and punishment. It is expected that the successful adoption of libertarian ethics and principles to understand this TV show might bring out Libertarianism as a valuable philosophical alternative to be taken into account when looking for solutions to current issues.

ABSTRACT Replacing a missing tooth in a maxillary anterior esthetic zone displaying a Seibert's class III ridge defect is a big challenge in the field of perioesthetics. In order to achieve maximum esthetics, form and function, an ideal pontic should have a natural emergence profile and support the labial soft tissue as well as the adjacent papillae. This is of paramount importance, especially if the patient has a high smile line. Augmentation protocols differ based on the treatment plan for implants or fixed prosthesis. The most popular techniques for soft tissue ridge augmentation for Seibert's Class III ridge defects include the roll technique, the wedge technique and the pouch technique among others. This article presents a case of an 18-year-old female patient with a challenging Seibert's Class III ridge defect treated for pontic site development using a combination of the pouch and the roll technique followed by a fixed prosthesis with ovate pontics. Research on gecko-based adhesion has become a truly interdisciplinary endeavour, encompassing many disciplines within the natural and physical sciences. Gecko adhesion occurs by the induction of van der Waals intermolecular (and possibly other) forces between substrata and integumentary filaments (setae) terminating in at least one spatulate tip. Gecko setae have increasingly been idealized as structures with uniform dimensions and a particular branching pattern. Approaches to developing synthetic simulacra have largely adopted such an idealized form as a foundational template. Observations of entire setal fields of geckos and anoles have, however, revealed extensive, predictable variation in setal form. Some filaments of these fields do not fulfil the morphological criteria that characterize setae and, problematically, recent authors have applied the term ‘seta’ to structurally simpler and likely non-adhesively competent fibrils. Herein we briefly review the history of the definition of squamate setae and propose a standardized classificatory scheme for epidermal outgrowths based on a combination of whole animal performance and morphology. Our review is by no means comprehensive of the literature regarding the form, function, and development of the adhesive setae of squamates and we do not address significant advances that have been made in many areas (e.g. cell biology of setae) that are largely tangential to their classification and identification. We contend that those who aspire to simulate the form and function of squamate setae will benefit from a fuller appreciation of the diversity of these structures, thereby assisting in the identification of features most relevant to their objectives.
