The theory of humours (akhlat) is one of the fundamental pillars of the Unani System of Medicine (USM). The concept of health and disease depends on the quality (kaifiyat) and quantity (kammiyat) of humour (khilt). Health (sehat) lasts when humours remain in equilibrium and the main determinant of health is the balance in six essential factors (asbab-e-sittah zarooriya). These factors are highly modifiable and deviation in any of them leads to disequilibrium in humours either qualitatively or quantitatively which ultimately manifests in the form of the disease. Elimination (istifragh) of these morbid humours from the body becomes mandatory to treat the diseases or to restore health. One of the effective methods of elimination is purgation (ishal). It is a method by which morbid humours from the body are eliminated through the anal route. Before the elimination of any pathological humour especially in chronic diseases, it is mandatory to make the humour easily eliminable. This process of making the pathological humour eliminable is known as concoction (nuzj). The process of concoction is a regular and continuous process of the tabiyat (mediatrix naturae) of the body. In case of a minor deviation in humour, tabiyat itself eliminates it from the body after concoction. When the causative pathological humours are in abundance or grossly deviated from normalcy, tabiyat needs help from outside the body. This help of tabiyat can be done with some humour specific drugs which are known as concoctive medicines (munzij advia). Once, the humours become eliminable, the process of evacuation can be started. Classical Unani literature and published papers were explored to find the rationale of purgation therapy. Purgation is found to be advisable in the treatment of many chronic diseases. Tabiyat is the ultimate healer in the body and purgation helps it to overcome the diseases. Keywords: Istifragh; Munzij; Nuzj; Akhlat; Humours; Concoction

UNSTRUCTURED Approach: This is a theorical reflection about Next-Generation Medicine, which is the first step to begin with an exponential medical healthcare and break with past models. Findings: In the past, the medical healthcare relied on an evidence-based practice to provide the best treatment options for patients, however, since 2010 a strong economic wave has shaped the perspective into a value-based medicine framework. We are facing new social dynamics and megatrends in our society. The emergence of 4.0 technologies is leading us to a pathway where a next-generation medicine will create an exponential value for the overall healthcare ecosystem. Originality: Next-Generation Medicine (NGM) integrates healthcare into digital ecosystems linked by innovative interfaces, advanced analytics, centric customer models and digital epidemiology surrounding a new concept of health and disease management. NGM is based in four core capabilities of physicians: Creativity, Collaboration, Communication and Critical thinking added to advanced digital operations that creates a systemic risk management. This integration is developed using bidirectional and integrative digital platforms operated by AI/ML connected to IoT and data collection in the cloud or in the edge computing. It is time for healthcare visionaries to set prejudice aside and start contemplating the amazing landscape that Next-Generation Medicine could offer.

 O texto parte de uma conferência de Canguilhem sobre o estatuto do conceito de saúde. Trata-se de uma exegese da citação de Epicteto nas Entrevistas (Livro II, 17) que busca explicitar a dupla dimensão da saúde como questão filosófica e como conceito vulgar. Busca-se articular esta formulação tardia e singular da obra de Canguilhem em seu projeto de uma história das ciências da vida compreendida como tarefa filosófica. Neste sentido cabe evocar questões centrais de sua tese Normal e Patológico – especialmente a disjunção entre o par antinômico saúde e doença e a dupla cromática fisiologia-patologia. Enquanto conclusão é oferecida uma articulação do problema com a crisecovid em curso, entendida a partir de impasses do campo biomédico e respostas insatisfatórias das ciências sociais e da filosofia.Palavras-chave: Canguilhem. It is an exegesis of the quote from Epictetus in the Interviews (Book II, 17) that seeks to explain the double dimension of health as a philosophical issue and as a common concept. The aim is to articulate this late and singular formulation of Canguilhem's work in his project for a history of the life sciences understood as a philosophical task. In this sense, central issues of his Normal and Pathological thesis should be evoked – especially the disjunction between the antinomic pair health and disease and the chromatic physiology-pathology pair. As a conclusion, an articulation of the problem with the ongoing crisis is offered, understood from the impasses in the biomedical field and unsatisfactory answers from the social sciences and philosophy.Keywords: Canguilhem.
