ABSTRACT Treatment of respiratory disorders using a metered-dose inhaler (MDI) with spacer becomes one of the important points. Pharmacists at pharmacies are the final key in therapy to ascertain how the use of inhalation tools is correct. This study aims to determine the completeness of each step of information on how to use MDI with spacer given by pharmacists at pharmacies in East Surabaya. The research design used is cross-sectional study with the subject of research is pharmacist in pharmacy. The research variable is the explanation of how to use MDI inhaler with spacer. An assessment method of 8 steps on how to use MDI with spacer and each number gets a value. Research subjects were 22 pharmacists. Most pharmacists describe each step orally by using the tool directly, ie at step: 1a (77.27%), 1b (77.27%), 2 (81.81%), 3 (31.81%) , 4a (59.09%), 4b (13.63%), 5a (100%), 5b (63.63%), 6a (90.90%), 6b (40.90%), 8a (9 , 09%), and 8b (9.09%). Most of the pharmacists explain verbally with a demonstration tool but only one pharmacist can explain all the steps completely. Therefore, pharmacists need to improve IEC services related to the use of MDI with a spacer for respiratory treatment.   Keywords :   information on how to use inhaler, Metered-Dose Inhaler (MDI), spacer, pharmacist, pharmacy

Background: Nowadays, ionizing radiation is increasingly used in medicine. One of the most frequent X-ray examinations is pelvic radiography. Gonads are susceptible in the pelvic area. Gonadal shielding (GS) is a useful method to reduce the received dose by gonads. Despite the benefits of using gonadal shielding, it is rarely used by radiographers. Methods: This cross-sectional study was carried out in ten governmental hospitals with 300 radiographs. Results: The radiographers’ knowledge of using GS had a value equal to 59.1%. However, the radiographers did not have enough information on the subject, although their awareness about the significance of GS was acceptable. Conclusions: Although the radiographers believed in the necessity of using GS for pelvic, abdominal, and spine examinations, they used no shields.

Background: Inhaler technique errors can affect the disease outcome through reduced deposition of medication in the lungs with certain errors were more likely to deter the optimum drug from reaching the lung. The objective of this study was to evaluate the pressurized metered-dose inhaler (pMDI) critical technique errors among asthma and COPD patients, and to investigate the predictors of those errors. Methods: A cross-sectional, observational, and multi-centered study conducted in 3 major hospitals in the state of Penang, Malaysia. Asthma and COPD patients aged 18 years or older consented to the study were consecutively recruited if they were at least on 1 pMDI without mouthpiece spacer or facemask space. Direct observation of the patient’s technique was recorded using a standard checklist and certain steps in the checklist were pre-determined as critical steps. The number of patients missed or incorrectly performed the steps including the critical steps were documented. Using logistic regression, the factors associated with the critical errors were determined, and the correlation was evaluated between age and pMDI knowledge score with critical inhalation steps score. Results: Of 209 patients observed, only less than half (40.7%) could perform all the critical pMDI steps correctly. Low education level was the only factor associated with critical inhaler errors. The increased knowledge of their inhaled medication was found to positively correlate with the correct critical inhalation steps. The critical errors also increase with the increased age of patients. Conclusion: The majority of the study population make inhalation error particularly in critical steps. Training and education intervention programs may help patients with lower education level, as well as those of advanced age and lack of knowledge of their inhaled medication because those populations are at a greater risk of committing critical errors in their use of pMDI.

This experiment centred in metered dose inhaler (MDI) device which used routinely by asthma patient as controller therapy. MDI used ask patient’s skill more when using their medication. MDI technique informnation provided by pharmachist are important to achive optimal medication in asthma therapy. By a non experimental study, mysterious patient collect MDI technique gived by pharmacist in Tuban pharmacy using observacional and cross sectional method. Information about MDI technique comprehensive collected by two ratter as mysterious patient which assessed with checklist that got measseured by kappa to know ratter aggrement percentage. The result showed from a total sampling under 41 pharmachist which represent each 41 pharmacy in Tuban, by purposive sampling Pharmacists as pharmaceutical personnel responsible for information services at pharmacies, including information on how to use MDI. Most of the apothecary's ability level in providing information is still lacking, then continued with adequate technique, and optimal technique in providing complete information of MDI use for asthma patient. Pharmacists know how to use MDI and no pharmacist is unexpected or not familiar with MDI as an asthma therapy. Pharmacists in Tuban district have a tendency to inform only four stages of the nine stages of MDI use from Osman et al (2012), ie at the 1st, 3rd, 5th, and 6th stages of MDI use

Background: Asthma is still a major health problem in global population, including Indonesia. Antiasthma drugs available in various dosage forms, including inhaler. However, several problems related to inhalation route were found due to its unique device form and spesific use technique. One of the major problems related to inhalation route is inappropriate use technique of inhaler device, which could lead to treatment failure. Therapy outcome can be measured through Asthma Control Test (ACT). Objectives: This study was aimed to evaluate correlation between metered dose inhaler (MDI) use technique and asthma control level in patients. Material and Methods: A cross-sectional analytic study was conducted in May - June 2021. Thirty patients who met inclusion criteria were enrolled in this study. Patients’ MDI use technique and asthma control level were evaluated using a valid and standardized questionnaire. Statistics analysis was performed to determine the correlation between MDI use technique and asthma control level. Results: This study showed that most of asthmatic patients were women in older age, with mild asthma severity for more than 10 years. The most prevalent medication used was Fenoterol HBr, followed by salbutamol, and salmeterol/fluticasone combination. Inappropriate MDI use was found in 70,0% patients, with major problem found in patients’ breathing technique before and during MDI use. Asthma control test was performed and showed that 90,0% of asthmatic patients involved in this study have an uncontrolled asthma. Statistical analysis using Pearson product-moment correlation test showed a positive correlation between proper use of MDI and asthma control level (r=0.425, p<0.05). Conclusions: Patient who properly use MDI may have a higher score in asthma control test, thus have a better control of asthma. This study emphasized pharmacist role as patient educator in ensuring appropriate inhaler use in order to achieve therapeutic goals.

Introduction: Metered-dose inhalers (MDI) are the most used inhalers worldwide. However, improper MDI techniques remain a significant problem and cause uncontrolled asthma. Studies worldwide have reported that the causes of inefficient inhaler technique are multifactorial. However, this topic has been less well studied in the Asian population. This study aimed to evaluate the MDI technique and associated factors among adults with asthma in a primary care setting in Malaysia. Method: This was a cross-sectional study with universal sampling conducted between July and October 2017. A total of 146 patients with asthma aged 18 years and older in a primary care clinic in Putrajaya, Malaysia were recruited. Logistic regressions were used for statistical analysis to examine the association between improper MDI techniques and their related factors. Results: The majority (83%) of respondents were female with a median age of 37 (IQR = 30.75–49.25) years, and the median duration of asthma of 20 (IQR = 10–30) years. An improper MDI technique was observed in 100 (71.9%) patients. The most frequently missed step was exhaling gently and fully before inhalation (51.4%). Respondents who were not on an MDI preventer (adjusted OR: 2.487, 95% CI: 1.121–5.519, p = 0 .025) or had used an MDI 5 years or fewer (adjusted OR: 3.369, 95% CI: 1.425–7.964, p = 0.006) were more likely to employ an improper MDI technique. Conclusion: There was a high proportion of improper MDI techniques among patients with asthma. Patients not using an MDI preventer or who had used an MDI less than 5 years were at higher risk of improper MDI technique.
