Developmental coordination disorder (DCD) refers to the motor skill difficulty without any explainable medical conditions. The prevalence rate of DCD is estimated to be 5–12% in Taiwan. Balance deficit is one of the main characteristics of children with DCD. The aim of this study was to examine the effect of a self-developed balance training program on the balance performance in children with DCD. Twenty four children aged 5–10 years old with DCD and 12 age-matched typically developing (TD) children participated in the study. Children with DCD were randomly divided between an experimental group (DCD-exp) who received treatment with the iBalance training program and the control group (DCD-con) who did not received treatment. TD children also did not receive treatment as a comparison group. All the children were tested with the movement assessment battery for children-2nd edition (MABC-2) and iBalance assessment system before and after the intervention period. A significant improvement of the MABC-2 balance component score, time duration of one-leg standing and the number of success trials with dynamic balance tasks were noted in the children of DCD-exp but not in the children of DCD-con and TD. The results suggest that the iBalance training program is effective for a short time treatment effect for children with DCD. The long term effect and transfer effect need to be examined.

Background and Purpose. Traditionally, rehabilitation programs emphasize training balance under single-task conditions to improve balance and reduce risk for falls. The purpose of this case report is to describe 3 balance training approaches in older adults with impaired balance. Case Descriptions. Three patients were randomly assigned to 1 of 3 interventions: (1) single-task balance training, (2) dual-task training under a fixed-priority instructional set, and (3) dual-task training under a variable-priority instructional set. Outcomes. The patients who received balance training under dual-task conditions showed dual-task training benefits; these training benefits were maintained for 3 months. The patient who received variable-priority training showed improvement on novel dual tasks. Discussion. Older adults may be able to improve their balance under dual-task conditions only following specific types of balance training. <p style="text-align: justify;"><strong>Background </strong>and Purpose: Balance is controlled through a complex process involving sensory, visual, vestibular and cerebral functioning which get affected by various neurological disorders such as fall. The purpose of the study was to compare the efficiency of three different balance training strategies in an effort to understand the mechanisms underlying training-related changes in dual task balance performance of older adults with balance impairment.</p> <p style="text-align: justify;"><strong>Methods:</strong> 45 older adults with balance impairment were recruited and randomly assigned to three groups. Group one received single task balance training, group two received dual task training balance training under fixed priority, group three received dual task balance training under variable priority. Subjects received one-hour individualized training sessions, five times in a week for two weeks. Berg balance scale and time up and go test were the outcome measure and their scores for all groups were taken prior and after the training.</p> <p style="text-align: justify;"><strong>Results:</strong> One-way analysis of variance was used to analyse the difference among the balance improvement in Group one, two and three. And the results revealed that post intervention scores were highly significant (p£ 0.05) in group two and group three performed better than group one.</p> <p style="text-align: justify;"><strong>Conclusion:</strong> In conclusion, dual task training is effective in improving balance under dual task context in older adults with balance impairment, and single task training may not generalize to balance performance under dual task conditions.</p>

BackgroundDual-task, performing a balance task and a cognitive task simultaneously, has been used as sport-related concussion assessment tool. However, the effect of dual task on balance performance remains controversial. Also, the fluctuation of hormone across menstrual cycle on balance and dual-task performance has not been fully examined.ObjectiveTo examine (1) the effect of dual task on balance performance, (2) the interaction of female sex hormonal level, balance, and dual-task performance.DesignA cohort study.SettingA laboratory of concussion research and services.ParticipantsA group of 49 healthy female college-aged students (age = 21.6 ± 2.0 years).Main outcome measuresThe Stability Evaluation Test (SET) protocol, Balance error scoring system (BESS) test and postural sway velocity (deg/sec) by using VSR SPORT force plate. Three auditory mathematics questions were given for each condition of the SET test. Two single-task conditions were provided, including balance only and math only. Balance test and math questions were given simultaneously during the dual-task condition.ResultsThere was no significantly statistically difference in BESS or sway velocity between single and dual task. However, the performance of math calculation is better in dual-task comparing to the single task (math only), with an 8%–12% higher rate of accuracy, but it didn't reach a statistical significance. The rate of accuracy during the dual task didn't vary among menstrual phases.ConclusionsBalance performance was maintained under dual-task conditions and fluctuation of female sex hormones across the menstrual cycle may not affect working memory and executive function. These findings are potentially clinically applicable to detecting balance deficits and cognitive alterations in female concussed individuals. Future studies are needed to investigate the underlying mechanism of balance maintenance and attention dividend under dual-task conditions.

Purpose Prior investigations suggest that simultaneous performance of more than 1 motor-oriented task may exacerbate speech motor deficits in individuals with Parkinson disease (PD). The purpose of the current investigation was to examine the extent to which performing a low-demand manual task affected the connected speech in individuals with and without PD. Method Individuals with PD and neurologically healthy controls performed speech tasks (reading and extemporaneous speech tasks) and an oscillatory manual task (a counterclockwise circle-drawing task) in isolation (single-task condition) and concurrently (dual-task condition). Results Relative to speech task performance, no changes in speech acoustics were observed for either group when the low-demand motor task was performed with the concurrent reading tasks. Speakers with PD exhibited a significant decrease in pause duration between the single-task (speech only) and dual-task conditions for the extemporaneous speech task, whereas control participants did not exhibit changes in any speech production variable between the single- and dual-task conditions. Conclusions Overall, there were little to no changes in speech production when a low-demand oscillatory motor task was performed with concurrent reading. For the extemporaneous task, however, individuals with PD exhibited significant changes when the speech and manual tasks were performed concurrently, a pattern that was not observed for control speakers. Background and Purpose This case report describes how a strength (muscle force-generating capacity) training program was associated with changes in muscle strength, motor function, and proprioceptive position sense in a young child with poor body awareness and a diagnosis of developmental coordination disorder. Case Description Assessment of a prekindergarten child referred for physical therapy because of behaviors compatible with poor body awareness revealed muscle weakness, poor performance on the Bruininks-Oseretsky Test of Motor Proficiency, and poor proprioception. Physical therapy testing done when the child was 5 years of age contributed to a pediatrician-assigned diagnosis of developmental coordination disorder. A 12-week strength training program was initiated. Outcomes Improvements were noted in muscle strength, gross motor function, and proprioception. Discussion Research indicates that muscles provide information about joint position. Evidence suggests that muscle strength gains seen in children are the result of neuromuscular learning and neural adaptations; therefore, a structured strength training program may have contributed to proprioceptive changes in this child.

Context The tandem gait test is a method for assessing dynamic postural control and part of the Sport Concussion Assessment Tool, versions 3 and 5. However, its reliability among child and adolescent athletes has yet to be established. Objective To examine the test-retest reliability of the single-task and dual-task tandem gait test among healthy child and adolescent athletes. Design Descriptive laboratory study. Setting Sports injury-prevention center. Patients or Other Participants Uninjured and healthy athletes between the ages of 9 and 18 years. Intervention(s) Tandem gait measures repeated 3 times across the period of approximately 1 month. Main Outcome Measure(s) Participants completed the tandem gait test under single-task and dual-task (ie, while simultaneously executing a cognitive task) conditions. Our primary outcome measure was completion time during the single-task and dual-task conditions. We also assessed cognitive accuracy and response rate while participants completed the dual-task tandem gait test. Results Thirty-two child and adolescent athletes completed the study (mean age = 14.3 ± 2.4 years; females = 16). Single-task tandem gait times were similar across the 3 testing sessions (14.4 ± 4.8, 13.5 ± 4.2, and 13.8 ± 4.8 seconds; P = .45). Dual-task tandem gait times steadily improved across the test timeline (18.6 ± 6.9, 16.6 ± 4.5, and 15.8 ± 4.7 seconds; P = .02). Bivariate correlations indicated moderately high to high agreement from test 1 to test 2 (single-task r = .627; dual-task r = 0.655) and from test 2 to test 3 (single-task r = 0.852; dual-task r = 0.775). Both the single-task (intraclass correlation coefficient; ICC [3,1] = 0.86; 95% confidence interval [CI] = 0.73, 0.93) and dual-task (ICC [3,1] = 0.84; 95% CI = 0.69, 0.92) conditions demonstrated high reliability across testing sessions. Conclusions Tandem gait outcome measures demonstrated high test-retest reliability in both the single- and dual-task conditions. The overall reliability was within the acceptable range for clinical practice, but improvements across tests suggested a moderate practice effect. Tandem gait represents a reliable, dynamic, postural-control test that requires minimal space, cost, and time.
