Introduction: The aim was to compare root canal wall cleanliness, amounts of apically extruded debris and irrigant during retreatment using the ProTaper Universal Retreatment (PT) system or balanced force technique (BFT). Materials and Methods: Root canals of extracted mandibular premolar teeth (n=44) were endodontically prepared and obturated, then the teeth were divided into two groups: BFT and PT. Apically extruded material was collected into preweighed Eppendorf tubes using the Myers and Montgomery method. The root halves were photographed under a stereomicroscope. Area of the remnant filling material (coronal, middle, apical thirds and total canal surface) was measured by software. Data were analyzed by Mann-Whitney U test. Results: The BFT and PT system provided similar degrees of canal wall cleanliness (p > 0.05) and amounts of extruded irrigant (p>0.05). The PT system caused less debris extrusion (p<0.05). Conclusions: Although the BFT was shown to cause less apical extrusion in primary root canal treatment compared to other manual instrumentation techniques, in the present study, BFT caused more debris extrusion than the PT group.   How to cite this article: Sarıçam E, Altunkaynak B, Kayaoğlu G. Aim: The aim of this case report is to present a series of anatomical variations and endodontic treatments in four two-canal mandibular premolar teeth and three rooted three-canal maxillary second premolar teeth with root canal treatment indications identified via clinical and radiographic examinations. The success of root canal treatment is achieved with a thoroughly examined root canal morphology that has been accurately determined radiographically and clinically before adequate shaping, irrigation, and hermetic filling procedures. Root canals that are not found or not adequately disinfected can cause root canal treatment failure and complications, such as pain, swelling, or persistent fistula, also known as flare-up, after treatment. Canal variations in the teeth were detected via periapical radiographs during the root canal instrumentation stage. Methodology: The endodontic treatments of four two-canal mandibular premolar teeth and one triple-rooted three-canal maxillary second premolar with root canal treatment indications were described. The goal of endodontic treatment is to remove all the vital and necrotic tissues, microorganisms and microbial byproducts from root canal system. This goal can be achieved through chemical and mechanical debridement of root canals. This article narrates the specifics and requirements of the irrigation solutions. Sodium hypochlorite is proposed as the primary irrigant by virtue of its organic tissue dissolution capacity and broad antimicrobial properties. On the other hand, chelation solutions are recommended as auxiliary solutions to remove the smear layer or to hinder its formation on dentin surface. Thus, it's hoped that sealers and root canal fillers can penetrate to dentin tubules and obturate the canals hermetically. There are new studies on traditional irrigants especially on some irrigants that can replace sodium hypoclorite. This article reviews the new irrigants which can be used in future endodontic practice, and their advantages and limitations. The obturation quality of GuttaFlow Bioseal in curved root canals is not commonly investigated although there has been a current approach toward utilizing this material in extracted molars in recent years. This study assessed the obturated surface area, extrusion of root filling material beyond the apical foramen and duration of obturation procedure in curved root canals using monocone obturation technique. Access cavity was prepared in 20 human mandibular molars. Root canals with curvature of more than 10 as determined according to Schneider’s method were included. Samples were prepared using Hyflex CM rotary files and divided into two groups (n=10): Group 1 [gutta-percha cone and GuttaFlow Bioseal] and Group 2 [gutta-percha cone and RoekoSeal Automix root canal sealer]. The duration of obturation procedure was recorded and obturation radiographs were taken. Samples were bisected and the mesial roots were sectioned horizontally to obtain 3 root segments; apical, middle and coronal. All resected roots were mounted on brass stubs, sputter-coated with thin platinum coating and observed under scanning electron microscope (SEM) at 70x magnification. The SEM images were transferred to the SketchAndCalc Area Calculator software. No statistically significant differences in the obturated surface area and extrusion of root filling material were observed between Group 1 and 2, irrespective of the status of root canal curvature. Duration for obturation in severe root canal curvatures between Group 1 and 2 were statistically significant. Obturated surface area and extrusion of root filling material were not affected by the root canal curvature, however duration for obturation using GuttaFlow Bioseal in severe root canal curvatures was slightly longer.

Abstract Aim The focus of this study was to examine the staining potential of calcium hydroxide (Ca(OH)2) on tooth structure following the removal of AH26 root canal sealer. Methods and Materials Fifty maxillary anterior teeth were prepared and obturated with AH26 and gutta percha. The sealers were then removed 24 hours later and the teeth were randomly divided into two groups. Ca(OH)2 was then placed in the root canals of the first group of teeth as a medicament and camphorated monochlorophenol (CMCP) was placed in the second group of teeth after the filling material was removed. The color of the external tooth surfaces was determined before tooth preparation and two weeks after the placement of the medicaments. The Z test was used for statistical analysis. Results All experimental teeth showed varying degrees of coronal discoloration with the Ca(OH)2 group showing more discoloration than the CMCP group (p<0.05). Conclusion Using Ca(OH)2 as a medicament after removing AH26 caused progressive discoloration of the teeth, whereas using CMCP caused only slight discoloration. Clinical Significance To avoid staining of the treated tooth, AH26 root canal sealer must be completely removed from the dentin walls before using a medicament. Canal filling must be well adapted to the walls of the root canal to prevent bacterial infiltration. Endodontic seals play an essential role in ensuring tightness, without which the canal filling would suffer infiltrations. This study aimed to evaluate the areas occupied by the two components of the canal filling, as well as the sealer/gutta-percha ratio in the root canals of the maxillary central incisors after their filling using the cold lateral condensation technique with gutta-percha. Thirty extracted upper central incisors were rotatably prepared with ProTaper Universal up to F3 and sealed using the cold lateral condensation technique with gutta-percha. After setting the sealer, the roots of the teeth were sectioned perpendicularly to 1 (L1), 3 (L3), 6 (L6), and 8 (L8) mm from the apex. The surface of the sections was analyzed with a Leica EZ4D stereomicroscope and photographed at two magnification orders: 10x and 25x. The areas corresponding to the gutta-percha, sealer, gaps, and root canal were expressed in pixels using the ImageJ software, version 1.50i. The difference in the representation of sealer areas, gutta-percha and voids was statistically significantly different for all four sections analyzed. The best adaptation of the canal obturation was observed in L1 and L3. The gutta-percha area was statistically significantly higher than that of the sealer for the L1, L3, and L6 levels, while the sealer/gutta-percha ratio recorded the lowest value at the L3 level (0, 30) and the highest at its L8 (0.70) level, without registering statistically significant differences regarding the area at the four analyzed levels. The voids were mostly absent or recorded a minimal percentage area (<1%). Cold lateral condensation of gutta-percha has led to a good adaptation of gutta-percha to the root canal wall, with a small amount of sealer, especially to the sections made at 3 mm from the apex. Given the limitations of this study, we noted that the voids were few – observed in the 6 and 8 mm sections – and were negligible in many cases.

Objective. The objective of this study was to evaluate the efficacy of two retreatment NiTi systems (protaper universal retreatment files, R-Endo), when compared to manual technique in removing Gutta-percha obturated with two sealers. Study Design. Sixty extracted single-rooted premolars were instrumented with Protaper rotary files till F3. The specimens were divided into six groups. Groups 1, 2, 3 were obturated with Gutta-percha and zinc oxide eugenol and Groups 4, 5, 6 were obturated with Gutta-percha and AH-plus. The retreatment was carried out in groups 1 and 4 with H-files and GGdrills, groups 2 and 5 with R-endo retreatment files and groups 3 and 6 with Protaper retreatment files. The roots were sectioned and evaluated under optical stereomicroscope. Statistical analysis was performed with one-way ANOVA and Newman-Keul's test at . Results. The manual technique resulted in cleaner canal walls when compared with both rotary retreatment systems. Conclusion. NiTi rotary retreatment files can be used to remove the filling material quickly, but it should be followed by hand instruments to obtain better canal wall cleanliness.

Abstract Background To evaluate the retreatability of curved root canals obturated with gutta-percha (GP) and iRoot SP using three different Ni-Ti systems through micro-computed tomographic (micro-CT) analysis. Methods A total of 51 curved root canals of permanent molars with angles of curvature ranging from 20° to 40° were used. The root canals were prepared with the ProTaper Next (PTN) rotary system to size 25, .06 taper and filled with iRoot SP and GP using the single cone technique. Samples were randomly divided into three groups (n = 17 each) according to the retreatment system used: Group I. ProTaper Universal Retreatment (PTR) system + ProTaper Next system up to PTN X2; Group II. Waveone Gold (WOG) system (WOG25); Group III. Reciproc Blue (RB) system (RB25). The ability to regain apical patency, the volume of residual filling material, and dentinal microcrack were assessed using micro-CT imaging. Data were analyzed statistically using two-way analysis of variance and chi-square test with a significance level of 5%. Results There were no significant differences among the systems tested in the reestablishment of apical patency and reduction of filling materials (P > 0.05), but RB system exhibited better reestablishment root canal ability in the upper portion of curvature and the reduction of filling materials in the apical third (P < 0.05). Intergroup analysis indicated that PTR system induced significantly more dentinal microcracks than other two systems (P < 0.05). Conclusions All three tested Ni-Ti instruments showed the similar retreatability of curved root canals obturated with gutta-percha and iRoot SP. RB system may be more effective and safer in curved root canal retreatment.

Introduction. Root canal filling material may be extruded during retreatment through the apical foramen and cause flare-up or chronic infection. The aim of this study was to compare the apical extrusion of gutta-percha and resilon filling materials during retreatment using hand and rotary instruments. Methods. Sixty extracted single-rooted teeth with single, straight canal were selected. Canals were prepared with ProTaper Universal rotary system to a size F2. Two groups (30 teeth in each) were filled with gutta-percha or resilon points, respectively. In both groups teeth were randomly divided into the three subgroups (10 teeth in each), based on the instruments used for retreatment: Hedstrom hand files and two rotary groups- ProTaper and Twisted File instruments. Apical extrusion was detected visually, using a 4-degree scoring system. Mean scores were calculated and analyzed statistically (t-test and ANOVA). The level of significance was set at p < 0.05. Results. Under tested experimental conditions, the type of canal filling material did not have significant effect on the results of apical extrusion during retreatment. Significantly more material was extruded in the resilon group when manual, Hedstrom file was used (1.80 ? 1.13) than rotary ProTaper (0.60 ? 0.70) and Twisted File (0.50 ? 0.71). Conclusions. The use of a rotary technique is recomended to minimize apical extrusion, especially when resilon obturation material is removed during retreatment.

OBJECTIVE: The aim of this study was to evaluate the effectiveness of gutta-percha and Resilon in filling lateral root canals in cervical, middle, and apical third using a thermomechanical technique. MATERIAL AND METHOD: Root canals of artificial teeth were prepared using a standard preparation. The lateral canals were fabricated using a 0.3-mm-diameter bur at 3 parts of each root. By using Tagger's hybrid technique with a McSpadden thermomechanical compactor, the root canal was filled using the following filling materials: Dentsply gutta-percha, Endopoint gutta-percha, and Resilon cones. The root canal fillings were evaluated using digitized radiographs and the Image Tool software. The percentage of filled area of each lateral canal was determined. The data were subjected to analysis of variance (ANOVA) and Tukey tests at a 5% significance level. RESULT: Resilon showed better effectiveness as a filling material. When the three thirds were compared, Resilon was more effective in the apical third than in the cervical third (p < 0.05). CONCLUSION: Resilon is an effective filling material for lateral root canals using a thermomechanical technique.

The objective of this study was to evaluate the efficacy of reciprocating instruments in the removal of bioceramic and epoxy resin-based sealers using micro-CT analysis. Root canals of 40 extracted human teeth were instrumented with a size R25 Reciproc instrument. Specimens were randomly divided into two groups (n = 20) according to the root canal filling material. In the first group, root canals were obturated with AH Plus sealer and Reciproc R25 gutta-percha. In the second group, a combination of bioceramic gutta-percha (TotalFill BC) and bioceramic sealer (TotalFill BC) was used. After one week, the retreatment of all specimens was performed using Reciproc instruments. To analyze the differences in the filling remnants, specimens were scanned in a micro-CT device after obturation and after the retreatment procedure. Statistical analysis was performed using the Mann–Whitney U test (p < 0.05). A combination of bioceramic sealer and bioceramic gutta-percha was more effectively removed from canals using a reciprocating instrument, with a filling remnants volume of 4.01 ± 3.13 mm3, in comparison to the combination of epoxy resin-based sealer and gutta-percha (6.96 ± 2.70 mm3) (p < 0.05). A reciprocating instrument was more effective in removing bioceramic sealers than epoxy resin-based sealers, although none of the root canal filling materials were completely removed from the root canals.
