✓ Acquired nontraumatic (spontaneous) encephaloceles of the middle fossa are not common. Rupture of an encephalocele frequently leads to a cerebrospinal fluid fistula. Tension pneumocephalus consequent to rupture of an encephalocele has not been previously reported, but conceivably occurs by means of a ball-valve mechanism in the ensuing fistulous tract. An unusual case is presented of an elderly man who suffered acute life-threatening neurological symptoms from a tension pneumocephalus that likely developed from rupture of an acquired nontraumatic encephalocele of the left middle fossa. The symptoms correlated with the location of the intracranial abnormalities. The literature is reviewed and the pathophysiology of the lesion is discussed.

A male 3-year-old child was referred to our service due to increasing head size and mild headache. On physical examination, head circumference was 55 cm (bigger than 97th percentile), the child was conscious, alert and oriented to person, place, time and event, his pupils were isochoric, the light reflex was bilaterally positive, eye movements showed a slight palsy on the abduction of the left eye, and a partial vision loss on the left eye was detected. Gait was also impaired. MRI presented a cystic lesion in the suprasellar region suggesting arachnoid cyst, associated with hydrocephalus. An endoscopic ventricular approach was performed through a straight incision over Kocher’s point. The cyst was easily identified, coagulated with cautery and incised with endoscopic scissors. A ball-valve mechanism was well visualized. Prepontine cistern was also sharply opened and the whole cyst wall was coagulated. The patient was discharged on the second postoperative day showing a clinical improvement. Late head CT presented also a radiologic improvement.

✓ A diverse collection of unverified theories as to the etiology of extradural meningeal cysts have been previously proposed. One case of intraspinal extradural meningeal cyst of the thoracolumbar region is presented in which a ball-valve mechanism involving an idiopathic dural rent and a herniated segment of an underlying dorsal rootlet was suggested by the operative findings. Closure of the dural rent with marsupialization of the meningeal cyst obliterated this extradural lesion. The ball-valve mechanism of formation and other previously proposed theories are discussed.

Objective: The prostate volume is an objective due to BPH diagnosed, however it is not always concerned with the LUTS degree. LUTS is a syndrome of BOO which marked by obstructive and irritable symptoms. The ball valve mechanism in bladder neck results in increasing of BOO. The aim of this study is to prove the relation of intravesical prostatic protrussion (IPP) with prostate volume, Q max, and international prostate symptom score (IPSS) in non complicated BPH LUTS patients. Materials and method: This research is an analysis observational that involved 85 male patients aged 50 to 91 years old (average 63,98 years old) in Urology Outpatient Clinic RSUD Dr. Soetomo, starting November 2007 until April 2008. Such IPSS evaluation, prostate volume measurement, uroflowmetry examination, and IPP examination with TAUS were applied to all of these patients. Results: The calculation shows the increasing of IPP degree followed by the decreasing of Q max value, where Spearman rank correlation indicates r=-0,465 with p=0,001 (smaller than 0,05), which shows the relation between IPP and Q max. In Spearman rank correlation, with the IPP degree increasing followed by IPSS value increasing indicates r=0,456 and p=0,001, which is smaler than 0,05. Also shows the increasing of IPP degree followed by prostate volume increasing, with r=0,230 and p=0,034 is smaller than 0,05 in Spearman rank correlation. Conclusion: The research reveals a connection between IPP degree and Q max, where the higher IPP degree the lower Q max value, the higher IPP degree the bigger IPSS value, and the higher IPP degree the bigger prostate volume.

Background: Valves are an important part of nuclear power plants and are the control equipment used in nuclear power plants. It can change the cross-section of the passage and the flow direction of the medium and has the functions of diversion, cutoff, overflow, and the like. Due to the earthquake, the valve leaks, which will cause a major nuclear accident, endangering people's lives and safety. Objective: The purpose of this study is to synthesize the existing valve devices, summarize and analyze the advantages and disadvantages of various devices from many literatures and patents, and solve some problems of existing valves. Methods: This article summarizes various patents of nuclear-grade valve devices and recent research progress. From the valve structure device, transmission device, a detection device, and finally to the valve test, the advantages and disadvantages of the valve are comprehensively analyzed. Results: By summarizing the characteristics of a large number of valve devices, and analyzing some problems existing in the valves, the outlook for the research and design of nuclear power valves was made, and the planning of the national nuclear power strategic goals and energy security were planned. Conclusion: Valve damage can cause serious safety accidents. The most common is valve leakage. Therefore, the safety and reliability of valves must be taken seriously. By improving the transmission of the valve, the problems of complicated valve structure and high cost are solved.

Abstract Background The larynx has multiple composite functions which include phonation, airway protection, and sensory control of respiration. Stenosis of the larynx and trachea were first recorded by O’Dwyer in 1885 and by Colles in 1886, respectively. Initially, the aetiology of laryngotracheal stenosis was predominantly infective. Currently, the leading cause is iatrogenic injury to the laryngotracheal complex secondary to prolonged ventilation in an intensive care unit. Main body Laryngotracheal stenosis is a complex and diverse disease. It poses a major challenge to the surgeon and can present as an airway emergency. Management typically demands the combined involvement of various disciplines including otorhinolaryngology, cardiothoracic surgery, anaesthesiology, interventional pulmonology, and radiology. Both the disease and its management can impact upon respiration, voice, and swallowing. The incidence of iatrogenic laryngotracheal stenosis has reflected the evolution of airway and intensive care whilst airway surgery has advanced concurrently over the past century. Correction of laryngotracheal stenosis requires expansion of the airway lumen; this is achieved by either endoscopic or open surgery. We review the relevant basic science, aetiopathogenesis, diagnosis, management, and treatment outcomes of LTS. Conclusion The choice of surgical procedure in the management of laryngotracheal stenosis is often dictated by the individual anatomy and function of the larynx and trachea, together with patient factors and available facilities. Regardless of how the surgeon chooses to approach these lesions, prevention of iatrogenic laryngotracheal damage remains of primary importance.
