The use of mechanical restraints for the management of disturbed or violent psychiatric patients continues to be a controversial issue. A survey of their use was carried out by means of a questionnaire sent to 370 psychiatric facilities in Canada to which there was a 62.2% response. General hospital psychiatric units, psychiatric hospitals and mental retardation centres accounted for 83.0% (191) of the returns and were the only facilities analyzed. The study showed that the vast majority still use mechanical restraints. The types of restraints in order of frequency of use included posey belts or shirts, isolation room, straps, sheets, strait jackets, wet or dry packs, hydrotherapy, and others. Violent behaviour is the main reason for which restraints are used. Medical orders, recording procedures and staff training in techniques of managing disturbed behaviour are reviewed. An important finding is the significant majority of facilities that have no stated policy permitting or forbidding the use of restraints. The findings are briefly related to Tuke's work in 1882 pointing to similarities in practice. Finally, the authors emphasize the need to deal with this contentious issue in an enlightened manner reflecting modern day demands.

This article is devoted to the 55th anniversary of the beginning of broadcasting radio “Mayak”. Due to a number of circumstances, among which the format of the radio station and the professional staff of its employees played a decisive role, “Mayak”, successfully working and having a large audience in the Soviet period, remained in the post-Soviet period, without losing its importance and weight. Having received in 2000 FM frequency, he was able to compete in the air and became an integral part of the new broadcasting system. The author identifies and analyzes the main innovations that have arisen in the air of this station, subsequently perceived by private commercial radio stations that have appeared in the post-Soviet space. The relevance of the article in determining the role of radio “Mayak” in the development and activities of domestic broadcasting, as well as in increasing the prestige and importance of the work of radio journalists. When writing the article, the author, who worked on the air of “Mayak” for more than ten years, who was both a witness and a participant in a number of innovative projects, used both conversations with veterans of the station and his personal experience, and the empirical method of research of the radio station “Mayak”.

The movement of people with intellectual disabilities into the community is increasingly endorsed by public policy. Whilst staff are critical to a successful transition to the community, there is only scattered research on their role in supporting people to move. In this study, 32 staff and the 16 people with intellectual disabilities they supported to move from congregated settings to group homes or personalized living arrangements in the community were interviewed on two occasions, before (time 1) and after (time 2) the move. In congregated settings, staff steered the move to community living, they helped them to settle in community group homes and supported them to increase control over their lives in personalized community arrangements, where they support became increasingly more personalized and geared towards community participation and development of social relationships. Implications for staff training are drawn.

Limited data exist analyzing the role of gender in workplace violence in health care settings. This study examined whether different types of threatening incidents with patients (physical, verbal, sexual, or posturing) were salient to male versus female staff across psychiatric settings (inpatient forensic, inpatient acute/chronic psychiatric, and outpatient psychiatric). Results indicated that although women disproportionately experienced sexualized threats, they were not more likely to report such incidents as salient and threatening. The study also assessed the extent to which situational variables contributed to staff’s feelings of threat. Results showed that rapport with the patient, quality of relationships with coworkers, and presence of coworkers in the area were not significantly related to how threatened staff felt in a recent threatening incident. Findings are discussed within the context of staff training and organizational benefits.

Local Communities Participation in Ecotourism DevelopmentAbstractThe community forest Mandiri Kalibiru manages of protected forests one of activities is ecotourism management. The involvement of local communities as ecotourism actors is a major feature in the service business. This study aims to present the type of local communities participation in developing ecotourism, and the role of HKm institution in encouraging the local communities to participate in ecotourism. The concept of participation at the planning, implementation and utilization level is illustrated in three periods of tourism development: the first period-pioneering, the second-developing and the third-developed. Data collection uses in-depth interview techniques and document review. Data analysis was performed descriptively and theoretical propositions. There are differences in local communities participation in the three periods. Participation in the planning level is very dependent on the companion during periods 1 and 2, but when the third period, the planning can be done independently. Participation in the implementation level in periods 1 and 2 was carried out cooperatively, but in the third period, the implementation of the development uses professional staff. Participation in utilization rates began in the second period involving limited HKm members and youth leaders, and the 3rd period the Kalibiru community involved took advantage of work and business opportunities which reached 85%. ABSTRACT Risk for complications and even death is inherent to anesthesia. However, the use of guidelines, checklists, and training can decrease the risk of anesthesia-related adverse events. These tools should be used not only during the time the patient is unconscious but also before and after this phase. The framework for safe anesthesia delivered as a continuum of care from home to hospital and back to home is presented in these guidelines. The critical importance of client communication and staff training have been highlighted. The role of perioperative analgesia, anxiolytics, and proper handling of fractious/fearful/aggressive patients as components of anesthetic safety are stressed. Anesthesia equipment selection and care is detailed. The objective of these guidelines is to make the anesthesia period as safe as possible for dogs and cats while providing a practical framework for delivering anesthesia care. To meet this goal, tables, algorithms, figures, and “tip” boxes with critical information are included in the manuscript and an in-depth online resource center is available at aaha.org/anesthesia.
