Proximal Focal Femoral Deficiency (PFFD) is a term used to describe a condition where the femur is short and associated with an apparent loss of continuity between the shaft and neck. PFFD is reported as an uncommon but complex clinical condition. This was commonly grouped with other disorders like coxa vara and short bowel tumours. This has a female to male ratio of 2:1 and an incidence of 1 case per 50,000 population to 1 case per 200,000 population. This is a case report of a three-year-old female that was referred for plain radiograph of the lower limbs on account of limb-length discrepancy and abnormal gait. The plain pelvic and lower limb radiograph showed a markedly shortened femur with non-demonstration of the femoral head and neck. There was a vertically oriented acetabula fossa with altered acetabula and iliac angles. The patient is currently managed conservatively mainly by reassurance and use of crutches.

Context: Spondylodiscitis is a term that includes vertebral osteomyelitis, spondylitis and discitis. Among the vertebrae, the most affected are the lumbar (45%), followed by the thoracic (35%). Adults present themselves progressively, with a predominant complaint of low back pain and pain on palpation of the affected site, with significant limitation of movement due to muscle spasms. Report a clinical case with an emphasis on the possibility of early diagnosis and correct treatment aimed at the recovery of patients with neurological sequelae. Analysis of medical records in a patient admitted to the neurology ward of Santa Casa de Belo Horizonte. Clinical Case report: A.I.S. patient, 45 years old, with chronic low back pain due to asymmetry of the left lower limb. History of wear of the femoral head diagnosed in adolescence. Evolving for 2 months with progressive weakness in the lower limbs, associated with paresis and paresis in the left lower limb. He performed abdominal USG which showed hepatosplenomegaly with collateral circulation, increased caliber of the portal, splenic and superior mesenteric veins. Tomography of the lumbosacral spine with osteolytic lesions in the joints of L2-L3, L3-L4 and L4-L5., With almost total osteolysis of the L4 vertebral body, retropulsion of much later at this level, suggesting spondylodiscitis. The resonance of the lumbar spine performed with acute spondylodiscitis L2-L3 and L3-L4, compressing the roots of the equine tail with a comprehensive potential, remains as the emerging emerging roots. Liquid filling of the L3- L4 intervebral disc compatible with acute spondylodiscitis. Staphylococcus aureus and enterobacteria are responsible for more than half of the cases of non-tuberculosis. Conclusion: The diagnosis of discitis can be quite difficult, due to the rarity of the disease, the insidious symptoms and the high prevalence of low back pain in the general population. It is considered an important morbidity factor, as it causes an important neurological sequel. In addition, it points to the importance of differential diagnosis of low back pain in the population.

AbstractWe present a case of a 14-year-old girl who was first treated when she was 4 years old. She had progressive limping of the left lower limb from the age of 3 years. She was diagnosed to have developmental coxa vara of the left hip and treated by a subtrochanteric valgus osteotomy of the left femur. Later, she developed hypertrophy of the left upper and lower limbs. There were port-wine stains over the left lower limbs with multiple superficial varicosities. Her diagnosis was Klippel–Trénaunay syndrome (KTS). She is asymptomatic at the final follow-up. Both developmental coxa vara and KTS are rare conditions. We present this case to demonstrate the rarest combination of two rare conditions occurring in the same limb.

AbstractProximal femoral focal deficiency (PFFD) is a rare congenital anomaly of the femur ranging from mild shortening to total agenesis. If left untreated, it could lead to impaired child growth, abnormal gait, vertebral abnormalities, cosmetic problems, and behavioral changes. We report on an Egyptian female, presenting to us at the age of 15 days with a short left lower limb. Both upper limbs and the right lower limb were normal and she was not dysmorphic. The skeletal survey showed an isolated short left femur. At the age of 11 months, clinical reexamination confirmed left femur shortening. Echocardiogram and abdominal sonography revealed no abnormality. Magnetic resonance imaging (MRI) of the pelvis and lower limbs revealed isolated left proximal femoral focal deficiency. Her developmental milestones were normal. She was referred to pediatric orthopaedics for early intervention and proper management. To our knowledge and after reviewing the literature, this patient represents the first case of unilateral isolated proximal focal femoral deficiency in Egypt. In conclusion, PFFD diagnosis needs a systemic skeletal–facial–genital phenotyping to differentiate from overlapping genetic disorders. Early diagnosis and radioimaging are highly important for proper management.

INTRODUCTION Transverse myelitis (TM) is a focal inflammatory disorder of the spinal cord, often associated with infectious disease, which can lead to permanent paraplegia or quadriplegia. CASE REPORT: A 56-year-female ,presentedwithb/l lower limb weakness since 2 days,b/l lower limb loss of sensations since two days,inability to walk since one day,and urinary retention requiring catherisation and fecal incontinence since one day. By history there was also , intermittent fever and night sweats. There was past history of respiratory tract infection two weeks back which partially subsided with antibiotics.Patient is known hypertensive and hypotyroid. Vitals:-normal General physical examination showed moderate general condition, and presence of crackles in both lungs. Neurological examination :- arreflecticlowerlimbs,paraplegia inboth lower limbs (strength 0/5 according to the Medical Research Council grade), and a positive Babinski sign,loss of all sensations completely below umbilicus. CONCLUSION- Tuberculous transversal myelitis is rare but should be consid- ered in the differential diagnosis of non-compressive myelopa- thies, in high burden TB countries or in the presence of clinical symptoms suggestiveofTB

A 39-year-old woman with restriction of movement and macrodactyly of the great toes was found to have severe juxta-articular multiple osteophytosis of the lower limbs. The pathogenesis of this unusual condition is unknown. It may be a variant of diffuse idiopathic skeletal hyperostosis.

Background: This study compares different lower-limb length measurements using tests of lower-limb upright full-length radiography and anteroposterior radiography of load-bearing hips. Methods: Forty-seven consecutive individuals aged 17 to 61 years (mean ± SD, 31.47 ± 11.42 years) voluntarily took part in the study; 23 (48.9%) were women and 24 (51.1%) were men. All individuals presenting a difference of 5 mm or greater between both lower limbs quantified with a tape measure were included. All of the participants signed an informed consent form to take part in the study. Two anteroposterior load-bearing radiographs were taken: one of the hip and an upright full-length radiograph of the lower limbs. Lower-limb–length discrepancy was quantified by taking different reference points. Interobserver and intraobserver reliability was assessed for each radiographic measurement. Any correlation between the different measurements were also verified. Results: Interobserver and intraobserver reliability was high for all of the measurements because the intraclass correlation was greater than 0.75 in all of the cases. There was a strong and positive correlation between the different measurements because when performing bivariate correlations with the Pearson correlation coefficient, positive values close to 1 were found. Conclusions: In this study, the different reference points reported in the upright full-length radiograph in addition to the hip radiographs are useful for assessing lower-limb–length discrepancy. The results showed that there is a correct correlation between the different measurements.

Background: Hypoplasia of the external iliac artery with persistent sciatic artery (PSA) is rare. Patients with this condition seek medical treatment mainly due to the formation of persistent sciatic aneurysm or aneurysm complicated with distal ischemia. Case presentation: In the present study, we report a case of a patient with thrombosis of PSA without aneurysm in the left lower limb that underwent endovascular treatment. Conclusion: Thrombosis of PSA may be associated with sedentary or other unhealthy lifestyles. Endovascular treatment of sciatic arterial diseases could be performed through the brachial artery approach, especially for those with hypoplasia of bilateral iliofemoral arteries.

AbstractA specimen of Sirenomelia from the department of Anatomy, Mahatma Gandhi Medical College and Research Institute, Pondicherry is presented here. This specimen was examined in detail and X-Ray was taken. On examination, the specimen showed i) Fused lower limb like a stump, ii) Undifferentiated genital organs, iii) Widely spaced nipples, iv) Receded chin and webbed neck, v) Low set large ears, vi) Epicanthal fold, vii) Flattened nose, viii) Absence of anal orifice. Fused lower limbs with associated undifferentiated genital organs and absence of anal orifice was found interesting to study and reported here as a case. The presence of Fused lower limb like a stump is suggestive of Sirenomelia and presence of i) Widely spaced nipples ii) Receded chin and webbed neck, iii) Low set large ears, v) Epicanthal fold, vi) Flattened nose is suggestive of Potter's Facies. It is presented here due to rarity of occurrence of such anomaly.

In the presented case report, we evaluated the mechanism of the external iliac, the common femoral and the superficial femoral arterial thrombosis secondary to total hip arthroplasty. A 75-year-old female sufferd from 5.5 cm shorter left lower limb and same sade coxarthritis. Next day after arthroplasty and eqalisation of the lower limbs, an acute ishemia of the treated leg was presented. Multyscan CT angiography revealed the presence of the external iliac, the common femoral and the superficial femoral arterial thrombosis. From the best of our knowledge, it seems to be the only case of this arterial segment thrombosis after total hip arthroplasty and equalistaion of the lower limbs reported.

Fractures of the shaft of the tibia and fibula are common. Complications are minimal following careful application of principles during treatment by the Orthopaedic Surgeon. However, in our environment, not all patients present to the Orthopaedic Surgeon as they opt for some form of unconventional treatment, usually with the traditional bone setters. Such treatments sometimes lead to clinically unacceptable outcomes. This is a case report of a 42-year old female Nigerian with a 4-year history of post traumatic anterior angulation of the distal right tibia with severely contracted Achilles tendon following traditional bone setter’s treatment for open fractures of the distal tibia and fibula. There was 7 cm of right lower limb shortening and 45 degrees anterior angulation of a tibial malunion. She had a two- stage operation to correct the deformities.
