Realgar, an arsenic-containing traditional Chinese medicine of As2S2, has significant therapeutic effects for hundreds of years. NiuHuangJieDu tablets (NHJDT) is one of the most commonly prescribed realgar-containing preparations for the treatment of sore throat, swelling, and aching of gums. However, realgar-containing TCMs raise great safety concerns due to the adverse effects reported by arsenic poisoning. In this study, the arsenic-related health risk assessment of NHJDT was conducted in healthy volunteers after single and multiple doses oral administration. Blood, plasma, and urine samples were collected after dosing at predetermined time points or periods. Simple, rapid, and sensitive methods were established for the quantification of total arsenic and arsenic speciation in biological samples. The total arsenic and arsenic speciation were determined by hydride generation-atomic fluorescence spectrometry (HG-AFS) and high-performance liquid chromatography–hydride generation–atomic fluorescence spectrometry (HPLC-HG-AFS), respectively. No significant fluctuation of total arsenic was observed in human blood, and no traces of arsenic speciation were found in human plasma. Dimethylarsenic acid was detected as the predominated arsenic species in human urine after dosing. Therapeutic dose administration of NHJDT was relatively safe in single dose for the limited blood arsenic exposure, but long-term medication may still pose health risks due to the accumulation of arsenics in blood and its extremely slow excretion rate. Therefore, arsenic exposure should be carefully monitored during realgar-containing TCM medication, especially for long-term regimen. The results obtained in this study will provide scientific references for the clinical application of realgar and its-containing TCMs.

Background. Fruit and fruit products are important part of our daily diet. In addition to the ingredients necessary for the proper functioning of the body, these products can also be a source of intake elements harmful to human health. Objective. Analysis of the results of monitoring studies conducted in Poland in 2015 concerning contamination of lead (Pb), cadmium (Cd), arsenic (As), mercury (Hg) and tin (Sn) in fruit and fruit products and exposure assessment. Material and methods. Approximately 600 samples of fresh, frozen, dried fruits, fruit preserves and canned fruits were tested. The laboratories of State Sanitary Inspection were involved in testing. Accredited and validated analytical methods were used. The test samples were prepared in accordance with the standard PN-EN 13804:2013. The contents of lead, cadmium and tin were determined by using flame atomic absorption spectrometry (FAAS) or flameless with electrothermal atomization spectrometry (GFAAS); arsenic by using hydride generation atomic absorption spectrometry method (HGAAS) and mercury the "cold vapor" atomic absorption spectrometry (CVAAS) method. Results. Contamination of investigated samples with elements harmful to human health (Pb, Ca, As, Hg and Sn does not rise concern to human health and was in most cases comparable with contamination reported in other European Union countries. The estimated average exposure of adults and children to lead intake with fruit and fruit products ranged from 0.5% to 14.6% of the respective Benchmark Dose Lower Confidence Limit (BMDL) established by the European Food Safety Authority (EFSA). The mean intake of cadmium with these groups of foodstuffs was in the range 0.3 - 8.5% of the TWI value (Tolerable Weekly Intake) established by EFSA for adults and children (0.1 - 3.6% of the PTMI (Provisional Tolerable Monthly Intake) established by JECFA. In case of arsenic mean intake was in the range 0.5 - 1.6% BMDL whilst for mercury (inorganic form) intake represented 0.5 - 1.4% of TWI value established by JECFA and then confirmed by EFSA. Conclusions. Based on the obtained results, it was found that content of Pb, Cd, As, Hg and Sn in the tested samples of fruit and their products does not pose a risk to consumer health. Estimated exposure of consumers does not exceed tolerable doses established by EFSA and JECFA for these elements.

Abstract Background The microbial ecosystem in the human gut varies between individuals with differences in diet. Selenium is one of most common trace elements in everyday diet, and selenium intake affects the human gut microbiota. We studied the effect of selenium intake on the gut microbiota in regions of Enshi with different distributions of selenium. Methods One hundred elderly subjects (>65 years) were recruited from high-selenium and low-selenium areas in Enshi and blood, nail, and fecal specimens were obtained. The selenium contents in these samples were determined in triplicate by hydride generation atomic fluorescence spectrometry. DNA was extracted from fecal specimens and the microbial diversity was analyzed by 16 S RNA. Results The selenium contents in the blood and nails were significantly different between the high- and low-selenium areas, and the composition of the intestinal microbiota, including abundance and extent of intestinal flora, was altered. The function and metabolic pathways of the gut microbiota showed clear differences. Conclusions As a trace element in human diet, selenium intake is an important factor that affects the intestinal microbiota and is likely involved in many human diseases. This study provides new clues and ideas for studying the correlation between selenium and human health.

Objective: Over the years, scientific investigations have proven the importance of selenium as an essential element for mammals, emphasizing its activity against many diseases and even its prophylactic effects. It is also established now that a malconsumption of selenium can be harmful. Therefore, the nature and the concentration of selenium and its derivatives found in the diet, the body, and even in the environment, for example, in the soil, should be determined carefully. Methods: In this review, analytical methods for speciation and determination of selenium concentrations in biological samples are summarized. Results: Methods ranging from routine to cutting-edge are explored, focusing on their analytical characteristics, such as specificity for discrete selenium species, sensitivity, accuracy, reproducibility, and skills required. Conclusion: There are already numerous studies regarding the analysis of selenium species. Beyond the method employed for actual measurements, we propose to review the preanalytic steps for sample handling in biological matrices, which directly affect results that will be more accurate with careful pretreatment. Furthermore, to reach better outcomes in terms of the identification of selenium species, different combinations of techniques might be the answer. We highlight here the last and the cutting-edge methods to identify and quantify selenium such as, high-performance liquid chromatography combined to inductively coupled plasma mass spectrometry (HPLC-ICP-MS), hydride generation atomic absorption spectrometry (HG-AAS), hydride-generation combined to atomic fluorescence spectrometry (HG-AFS), or to inductively coupled plasma optical emission spectrometry (HG-ICP-OES). This review emphasizes the importance of such investigations and the need to achieve reliable, safe, and effective quantification and methods of determination.
