Abstract Objective To investigate experimental eggshell powder (ESP) solution application effect on shear bond strength (SBS) test timing of bulk-fill resin composite to bleached enamel. Methods Eighty bovine-extracted anterior teeth were alienated to eight groups (n = 10), representing shear bond strength test timing periods (immediate or delayed) and the remineralizing agents investigated (MI paste Plus and ESP solution). Enamel surfaces were bleached using in-office chemical bleaching agent and the two remineralizing agents were applied then self-etch adhesive and bulk-fill resin composite were performed at 24 h and 14 d before SBS test was done. One-way ANOVA followed by Tukey post hoc test was performed for statistical analysis. Results The control groups showed the highest mean values, while the bleached enamel groups recorded the least mean values. ESP bleached enamel group recorded a significantly higher mean SBS values than MI paste Plus bleached group. Delayed SBS groups showed higher mean values than immediate SBS groups except for control group. Conclusions ESP had a positive and significant effect on SBS of bulk-fill resin composite to bleached enamel surface. Delaying the bonding procedure and restoration of the bleached teeth using resin composite restorations results in a better bond durability.

Objetivos: Evaluar los efectos de agentes remineralizantes a base de fosfato de calcio en la obliteración de túbulos dentinarios y dispersión de energía en la dentina cervical. Material y métodos: Treinta especímenes de dentina cervical bovina fueron previamente erosionados (ácido clorhídrico al 0,6 %, pH 2,3; 5 min) y divididos aleatoriamente en 3 grupos (n = 10): Control – sin tratamiento, NP – Desensibilize Nano P (FGM®) y RD – MI Paste Plus (RecaldentTM). Los tratamientos fueron aplicados en cuatro sesiones con intervalos de 7 días. Los especímenes fueron analizados por microscopia electrónica de barrido (MEB) y espectroscopia de rayos X de energía dispersiva (EDX). Resultados: Los grupos NP e RD mostraron mayor obliteración de túbulos dentinarios que el grupo control (p < 0,05). Las concentraciones de Ca/P fueron similares entre todos los grupos evaluados. Conclusiones: Los agentes remineralizantes a base de fosfato de calcio demostraron ser una alternativa promisoria en la obliteración de túbulos dentinarios.

Introduction: White spot lesions (WSLs) occurring after orthodontic treatment lead to patient dissatisfaction and aesthetic problems. The role of calcium-phosphate demineralization systems and the Er:YAG laser in the treatment of these lesions has recently been taken into account. This study aimed to investigate the effect of the Er:YAG laser and MI Paste Plus on the treatment of WSLs. Methods: A total of 65 premolars extracted due to orthodontic treatment were studied in this research. To create enamel lesions, the teeth were placed in a demineralizing solution. The teeth were then randomly divided into five groups (n=13) as follows: first group, control; second group, saliva; third group, MI Paste Plus; fourth group, Er:YAG laser; and fifth group, MI Paste Plus together with the Er:YAG laser. The teeth were kept in artificial saliva between treatment processes. Artificial saliva was replaced daily with fresh artificial saliva. The teeth were sectioned longitudinally by a disc from the middle of the exposed enamel and each section was mounted in polyester resin. The surface of the samples was serially polished and the microhardness of the teeth was measured at depths of 0, 50, 100, and 150 µm. Results: The microhardness was significantly higher in the fifth group than other groups at depths of 50 and 150 µm (P<0.005). Using the laser or MI Paste Plus alone did not significantly increase the microhardness. Conclusions: The combined application of the Er:YAG laser and MI Paste Plus is effective in the treatment of WSLs.

Background. Enamel demineralization around orthodontic brackets is an important clinical problem. This study sought to compare the efficacy of sodium fluoride (NaF), casein phosphopeptide amorphous calcium phosphate fluoride (CPP-ACP-F; MI Paste Plus) and a water-based cream (Remin Pro), which contains hydroxyapatite and fluoride for prevention of enamel demineralization. Methods. Fifty-six sound human premolars extracted for orthodontic purposes were collected. After cleaning, the crowns were mounted in acrylic resin and all the surfaces were coated with nail varnish except for a 3×4-mm window on the buccal surface. The samples were randomly divided into four groups of 14 and subjected to pH cycling for 14 days, during which the teeth were immersed in artificial saliva for 21 hours and in demineralizing agent for three hours daily. Before transferring the samples from the saliva to the demineralizing solution, the remineralizing agent (0.05% NaF, MI Paste Plus or Remin Pro Paste, depending on the group) was applied on the samples once a day for five minutes. No remineralizing agent was used in the control group. Surface microhardness of samples was measured by Vickers microhardness tester at baseline and after the intervention. The data were analyzed using one-way ANOVA, ANCOVA, Bonferroni test and Tukey test. Statistical significance was set at P<0.05. Results. The mean microhardness was significantly different between the test and control groups (P<0.0001). Other differences were not significantly different (P>0.05). Conclusion. The results showed that 0.05% NaF was more efficient than Remin Pro and MI Paste Plus for prevention of white spot lesions (WSLs). Remin Pro and MI Paste Plus were not significantly difference from the control group in this regard.

BACKGROUND: Erosion is a widespread phenomenon with higher predilection in primary dentition. AIM: The aim of the present study is to assess the remineralising effect of Er,Cr:YSGG laser application combined with CPP-ACPF after erosive demineralisation by Coca-Cola in primary teeth. METHODS: Fifty teeth (n = 10) were divided into; Group I: Artificial saliva, (Saliva natural, Medac, UK), Group II: CPP-ACPF (MI Paste Plus, GC Corp, USA), Group III: Er,Cr:YSGG (Waterlase iPlus, USA), Group IV: CPP-ACPF + Er,Cr:YSGG, Group V: Er,Cr:YSGG + CPP-ACPF. Teeth were immersed in Coca-Cola for 10 min, 5 times/day for 5 days. DIAGNOdent (DD) measurements were taken before and after the experiment. RESULTS: There was a significant increase in DD readings after erosive-treatment cycles in all test groups. The highest reading was in samples immersed in artificial saliva, and the lowest was in those subjected to combined CPP-ACPF and Er,Cr:YSGG laser application, regardless of the sequence used. There was no significant difference between samples immersed in artificial saliva, and after CPP-ACPF application. Similarly, there was no significant difference between samples treated by combined treatment of CPP-ACPF and Er,Cr:YSGG application. However, there was a significant difference between samples immersed in artificial saliva or treated with CPP-ACPF application and those subjected to combined treatment CPP-ACPF along with Er,Cr:YSGG. CONCLUSION: Combining Er,Cr:YSGG laser and CPP-ACPF paste significantly increased enamel remineralisation, regardless of the sequence implemented. Saliva naturally and CPP-ACPF application had a comparable effect on remineralisation.

Актуальность. В настоящей статье дан анализ показаний к проведению реминерализирующей терапии согласно отечественным и зарубежным классификаторам кариеса. Выработка оптимального подхода для стабилизации начальных кариозных поражений у детей является основой внедрения концепции минимальной интервенции, возможность избежать инвазивных методов лечения кариеса особенно важна в практике детского стоматолога. Цель на основе анализа источников литературы: выделить основные тенденции в проведении реминерализирующей терапии в комплексном лечении начальных форм кариеса у детей. Материалы и методы: было изучено 483 источника литературы из базы данных Pubmed, а также 82 источника отечественной литературы. В общем объеме изученных работ 27 являются систематическими обзорами, 2 метаанализа, 83 рандомизированных клинических исследования, 6 учебников. Результаты: приведен обзор современных препаратов, используемых для лечения начальных кариозных поражений, выделены основные ключевые пункты для создания индивидуальных протоколов реминерализации с использованием домашних курсов GC Tooth Mousse и GC MI Paste Plus. Выводы: на основе проведенного обзора выделены ключевые пункты в современных тенденциях к неинвазивному лечению начальных форм кариеса у детей, особый акцент сделан на практических аспектах назначения домашних курсов реминерализирующей терапии.

ABSTRACT Objectives: To determine the effect of Clinpro 5000, Clinpro Tooth Crème, and MI-Paste Plus on the formation of white spot lesions in patients undergoing orthodontic treatment. Materials and Methods: Three prospective groups with 40 patients undergoing orthodontic treatment in each group were evaluated (total recruitment = 120 subjects). The selected product was brushed on for 2 minutes twice daily for 4 months. Subjects were reviewed for 4 months on a monthly basis. The Enamel Decalcification Index (EDI) was used to determine the number of white spot lesions per surface at each visit. Results: 100 subjects (35 using Clinpro 5000, 32 using Clinpro Tooth Crème, and 33 using MI Paste Plus) completed the study. The data lend strong support for Clinpro 5000 providing superior protection against enamel decalcification when compared to Clinpro Crème, and mixed support when compared to MI Paste Plus. Conclusions: The use of Clinpro 5000, Clinpro Crème, and MI paste Plus all have a reduction effect on white spot lesions when compared to studies reported previously. Clinpro 5000 has a marginally better effect than the two other test pastes. The results of this study can be used by clinicians when deciding the effectiveness of using fluoride dentifrice products to prevent white spot lesions in their orthodontic practice (ClinicalTrials.gov ID: NCT03440996).

ABSTRACT The objective of this study was to report the clinical treatment of a child with Incisor Molar Hipomineralization. A 5-year-old Brazilian child, male gender, was diagnosed with Molar Incisor Hypomineralization, reporting high teeth sensitivity. After anamnesis and clinical examination, treatment was conducted with three weekly applications of fluoride varnish containing 5% CPP-ACP complex. Also, it was advice to the patient for using a toothpaste containing fluoride and CPP-ACP (MI Paste Plus). After that, molars with great tooth structure loss were restored with resin modified glass ionomer cement. Prior to the first topical application of varnish with CPP-ACP and fluoride toothpaste containing CPP-ACP, a sensitivity test was conducted using thermal stimulus and facial pain scale. It was observed relative sensitivity decrease between sessions, reporting no sensitivity at the last session before the restoration. The treatment of Molar Incisor Hypomineralization teeth with CPP-ACP complex associated with fluoride varnish can be an alternative to reduce sensivity.
