We describe a 34-year-old woman with isolated tuberculosis (TB) uveitis who had unsuccessful resolution of her ocular surface squamous neoplasia after multiple shave excisions with a positive purified protein derivative (PPD) test and interferon-gamma (IFN-γ) release assays (IGRAs) proven tuberculosis. She was managed with wide local excision of the lesion. Histopathology confirmed conjunctival intraepithelial neoplasia (CIN) grade II. Topical interferon alpha-2b (4 times daily) and anti-tuberculous therapy was planned for her, and there was no sign of recurrence of the lesion at six months follow-up.

We report a case of conjunctival intraepithelial neoplasia (CIN) in a patient presenting with the pigmented conjunctival lesion. This study involved a 56-year-old woman that presented with right eye irritation for 1 month. She noticed brownish pigmentation arising from her right nasal conjunctiva and growing slowly over time. Biomicroscopic examination showed a gelatinous pigmented conjunctival mass with feeder vessels. Conjunctival impression cytology (CIC) was done and reported as CIN. Treatment was started with 0.02% mitomycin-C eye drops. The conjunctival lesion responded well to medication. This report shows that CIN can manifest as a pigmented tumor, resembling melanoma. CIC plays a role in the diagnosis of this condition. This tumor responded well with 0.02% mitomycin-C eye drops.

A 68-year-old Malay male with no known medical illness presented with progressive growth of right conjunctival mass over a few months. Anterior segment examination of the right eye showed an inferior perilimbal elevated gelatinous conjunctival mass measuring 6 mm vertically x 14 mm horizontally with 360° corneal vascularisation. Excision biopsy and histopathological examination revealed areas of dysplastic cells involving the full epithelial thickness, suggestive of conjunctival intraepithelial neoplasia. The patient defaulted follow-up and presented later with recurrence involving the superior two-thirds of the cornea. Pulsed dosing of topical 5-fluorouracil 1% was initiated 4 times daily for a week with 21-day breaks for a total of 4 cycles. Regression of the lesion was noted after two cycles of 5-fluorouracil.
