The paper describes a family school for learning how to live with schizophrenia, which was founded in 1986 in Vienna, Austria, and is still running today. It was established in cooperation between professionals and the Austrian self-help association HPE of the relatives of persons with mental disorders. It addresses the needs of 10 families at a time, in cases where a son or a daughter was diagnosed with schizophrenia and had already experienced one or several episodes of the illness. The course lasts one and a half years and is organized according to the model of a weekly boarding school, where 10 children, the residents, stay in the school overnight from Sunday evening to Friday and take part in a structured program on cognitive, social and practical life skills. Ambulatory psychiatric treatment is taking place concurrently outside the school through local routine services. On weekends residents stay with their parents since the school is closed. Parents visit the school regularly to take part in joint activities with the residents. They also undertake night shifts in the school and attend a weekly parents group. In the regular encounters during everyday activities in the school, learning by doing occurs parents get to know the daughters and sons of other families and can learn to distinguish between disease-related and personality-related behavior. Residents can have similar learning experiences in relation to the parents of other residents. The main aim of the school is that parents learn to provide protected autonomy for the daughters or sons in question, in order to assist them after the end of the course in leading a life characterized by as much autonomy as possible after the end of the school.

Objectives To evaluate the extent to which our current provision of diagnostic and interventional radiology services matches existing clinical demand and future government proposals as set out in the Royal College of Radiologists published guidance on providing seven-day acute care. Methods In June 2018, all UK radiology department audit leads were sent a questionnaire designed to assess compliance for each standard of the Royal College of Radiologists published guidance on providing seven-day acute care. Results 135 hospitals (68%) responded. Of those that responded, 96% of departments have a diagnostic radiologist rota for clinicians to discuss acute cases and review imaging and 48% of departments do not have a fully staffed consultant rota 24 h a day, seven days a week for interventional radiology. There is significant variance in MRI radiographer availability within departments, ranging from 18.8% during Saturday/Sunday evening/overnight up to a maximum of 63.9% during Saturday daytime. 11% of departments participate in a regional out of hours cross-organisation reporting rota. 40% of departments have no 24/7 RIS technical support and 34% have no PACS technical support out of hours. Conclusion There is a wide variation in practice across radiology departments in the UK. Although there are some standards that the majority of hospitals are achieving, there is a significant short-fall in fundamental aspects of providing acute seven-day care. The multifactorial nature in which these problems have arisen means there is no easy solution to combat these issues. There is a requirement for significant investment and political commitment to improve staffing and infrastructure in order to address the current situation. Advances in knowledge A UK wide evaluation of the current provision of seven-day working in radiology showing 54% of hospitals do not have a UK working-time regulations compliant Interventional radiology rota, severe lack of availability of acute MRI out of hours and significant deficiencies in providing technical support out of hours. A sustainable and efficient seven-day service is currently not being provided.

Ed Sullivan dominated Sunday night primetime television for a quarter of a century with his extremely popular and inclusive variety show (first called Toast of the Town, then The Ed Sullivan Show). A former (and concurrent) Broadway columnist, Sullivan adored the mythology of Broadway and promoted its history—in song and performance—on practically every broadcast. Expanding on the reach of radio, television allowed for millions of Americans to get their first glimpse of Broadway magic through Sullivan’s promotion of current shows and tributes to giants of the past. The musical Bye Bye Birdie paid homage to the great impresario of the television age by writing him into the show. Sullivan’s domination of the airwaves also nearly parallels the so-called Golden Age of the Broadway musical.

Background There are cases in the headache literature described as “cluster-migraine,” but none of stereotyped cases of cluster headache evolving into status migrainosus. We believe this is the first documented case of “cluster-status migrainosus”. Case A 54 year-old female hospital administrator presented with headaches with a unique periodicity and semiology that were acutely responsive to high-flow oxygen. She experienced cluster headache attacks every Thursday morning at 3–4 am, which would evolve into status migrainosus lasting through Sunday evening. These attacks were preceded by prodromal depressed mood changes and fluid retention, and later followed by postdromal euphoria and auto-diuresis. These attacks initially occurred every other week and progressed to weekly attacks for 1.5 years. These headaches did not respond to trials of propanolol, sodium valproate, topiramate, amitriptyline, gabapentin, and carbamazepine for preventive treatment or to oral sumatriptan and butalbital-acetaminophen-caffeine for acute treatment. We started her on high-flow 100% oxygen for cluster headache, which successfully aborted greater than 80% of her weekly cluster headache attacks and prevented them from evolving into status migrainosus. Conclusion We believe this is the first case of “cluster-status migranosus” described in the medical literature. High-flow oxygen both aborted the cluster headaches and prevented the ensuing status migrainosus.

Among villagers of Jambu, in Mlonggo subdistrict of Jepara regency, there is iriban activity that has become a tradition in their daily lives. This routine includes neighborhood meeting on the date of 15th and 30th, participated by men and women as members of the neighborhood association (Rukun Tetangga); the other activities are only participated by the men such as dhiba'an or berjanjen held every Sunday evening, the manaqiban held on the date of 11th and 27th as well as tahlilan held every Thursday night. Iriban activity has become a tradition that is carried out from generation to generation. Yhe reason for the implementation of iriban tradition is to draw the spirit and sympathy of the people to participate in social activities. The implementation of iriban philosophy includes some elements, such as paguyuban (gemeinschaft) and patembayan (gesselschaft) concepts, reciprocity, exchange theory, and togetherness.
