Aims. A major clinical concern is the continuous increase in the number of patients diagnosed with advanced coronary artery disease, ischemic heart failure, and refractory angina, and one of the most promising treatment options for these conditions is stem cell-based therapy. The aim of this study was to assess the functional improvement following intramyocardial injection of adipose-derived stromal cells, using cardiac magnetic resonance. Methods and Results. Thirteen patients with ischemic heart failure, reduced left ventricular ejection fraction, refractory angina, and who have been disqualified from any form of direct revascularization were enrolled in the study with transthoracic autologous adipose-derived stromal cell implantation. All patients underwent cardiac magnetic resonance prior to the procedure and after 12 months of follow-up. A significant increase in stroke volume ( 83.1 ± 8.5   mL vs 93.8 ± 13.8   mL , p = 0.025 ) and stroke volume index ( 43.3 ± 7.6   mL / m 2 vs 48.7 ± 9.1   mL / m 2 , p = 0.019 ), a statistical trend toward an increase in left ventricle ejection fraction ( 36.7 ± 13.2 vs 39.7 ± 14.9 , p = 0.052 ), and cardiac output improvement ( 5.0 ± 0.7 vs 5.5 ± 0.9 , p = 0.073 ) was observed in the patient postprocedure. Enhanced relative regional thickening was noted in the segments with adipose-derived stromal cell implantation. Conclusions. Intramyocardial adipose-derived stromal cell implantation is a promising therapeutic option for selected, symptomatic patients with ischemic heart failure, who have preserved myocardial viability despite being unsuitable for direct revascularization.

Zusammenfassung Einleitung Mesenchymale Stromazellen sind aufgrund ihres regenerativen Potentials schon in zahlreichen Fachgebieten der Medizin im klinischen Einsatz. In der Plastischen Chirurgie stehen vor allem Fettstammzellen (Adipose derived stromal cells = ASCs) aus Lipoasopirat im Mittelpunkt der rezenten Forschung. Die Fähigkeit zur Ausschüttung von regenerativen Zytokinen ist der Schlüsselmechanismus im therapeutischen Einsatz von ASCs. Es sind zurzeit eine Vielzahl an Methoden und Geräten zur Lipoaspiratgewinnung in der klinischen Anwendung. Bei den meisten dieser Geräte sind die Auswirkungen auf die regenerativen Eigenschaften der ASCs noch weitgehend ungeklärt. Ziel dieser Studie ist der Vergleich der Expression von wesentlichen regenerativen Zytokinen von ASCs, gewonnen mittels dreier Standardverfahren der Plastischen Chirurgie: Abdominoplastik, Suction assisted Liposuction (SAL) und Ultrasound assisted Liposuction (UAL). Patienten und Methoden UAL und SAL Lipoaspirate wurden jeweils von 3 gesunden weiblichen Probanden gewonnen, be vor eine Abdominoplastik an denselben Patienten durchgeführt wurde. ASCs wurden aus UAL und SAL Lipoaspiraten sowie aus den Abdominoplastikresektaten isoliert und die RNA-Expression von wichtigen regenerativen Zytokinen (HGF (hepatocyte growth factor), FGF-2 (basic fibroblast growth factor), MCP-1 (monocyte chemotactic protein 1), SDF-1 (stromal cell-derived factor 1) und VEGF (vascular endothelial growth factor) verglichen. Resultate Es zeigte sich kein Unterschied hinsichtlich der Expression von HGF, FGF-2, SDF-1 und VEGF zwischen den einzelnen Proben, jedoch eine signifikant erhöhte Expression von MCP-1 in ASCs gewonnen aus UAL-Lipoaspirat. Conclusio UAL stellt bezüglich Einfluss auf das ASC Expressionsprofil von Schlüsselzytokinen der Gewebserneuerung eine mit SAL zumindest vergleichbare Methode dar. Sowohl UAL als auch SAL Lipoaspirate sind geeignet zur Gewinnung von funktionellen ASCs vergleichbar mit minimal manipulierten Zellen aus Abdominoplastikresektaten.

Abstract Alterations of the BM microenvironment have been shown to occur after chemoradiotherapy, during aging, and after genetic manipulations of telomere length. Nevertheless, whether BM stromal cells adopt senescent features in response to these events is unknown. In the present study, we provide evidence that exposure to ionizing radiation (IR) leads murine stromal BM cells to express senescence markers, namely senescence-associated β-galactosidase and increased p16INK4a/p19ARF expression. Long (8 weeks) after exposure of mice to IR, we observed a reduction in the number of stromal cells derived from BM aspirates, an effect that we found to be absent in irradiated Ink4a/arf-knockout mice and to be mostly independent of the CFU potential of the stroma. Such a reduction in the number of BM stromal cells was specific, because stromal cells isolated from collagenase-treated bones were not reduced after IR. Surprisingly, we found that exposure to IR leads to a cellular nonautonomous and Ink4a/arf-dependent effect on lymphopoiesis. Overall, our results reveal the distinct sensitivity of BM stromal cell populations to IR and suggest that long-term residual damage to the BM microenvironment can influence hematopoiesis in an Ink4a/arf-dependent manner.
