Using a contemporary Kleinian psychoanalytic model, this paper explores some important theoretical issues relevant to understanding infidelity for the couple psychotherapist. The main focus of this paper is on the problems that one or both of the partners in a couple may have with mourning past or impending loss, and the consequent infidelity that may result from a manic attempt to replace depression or psychic pain with excitement. The concepts of the oedipal situation, triangulation, mourning, and manic defence are outlined in terms of the implications for understanding infidelity. The importance of viewing infidelity as possibly reflecting a range of psychological dilemmas across the developmental spectrum is emphasised. Clinical material is used to illustrate the role of unresolved pre-oedipal and oedipal issues, manic defences, and the ensuing triangulations that may develop, providing the psychic structures for infidelity.

The onus on therapists to seek the consent of their patients before publishing clinical material may be one reason why so few decide to write about their experience. There are inevitable and unavoidable tensions in balancing the duty of care to patients with other ethical responsibilities, including the needs of the professional community for education and scientific advancement. In this paper, we explore the context and dynamics of seeking consent from couples and families to publish material relating to their therapy and propose a way to manage some of the ethical dilemmas involved in writing about patients that is in keeping with the contemporary analytic literature on the interpersonal unconscious between patient and therapist, and the interpsychic/interpersonal dimensions of therapeutic action. Throughout this paper, the term “patient” is used to designate couples and families as well as individuals.

Few books in the burgeoning field of couple psychoanalysis have garnered as much admiration as James Fisher's The Uninvited Guest: Emerging from Narcissism towards Marriage. In this memorial essay, the author pays tribute to the late Dr Fisher and to his perennial book which explores the ways in which pathological narcissism, among other factors, inhibit the development of spousal intimacy, often destroying partnerships entirely. The author describes the creative way in which Fisher drew upon great works of literature, most notably William Shakespeare's A Winter's Tale, and T. S. Eliot's The Cocktail Party, as well as long-forgotten clinical material from Fisher's predecessors at the Family Discussion Bureau (forerunner of the Tavistock Centre for Couple Relationships), in order to understand the ways in which marital partners struggle with false self couplings. The author assesses the importance of Fisher's contribution in the context of the history of couple psychoanalysis.

In Uruguay, Lacanian ideas arrived in the 1960s, into a context of Kleinian hegemony. Adopting a discursive approach, this study researched the initial reception of these ideas and its effects on clinical practices. We gathered a corpus of discursive data from clinical cases and theoretical-doctrinal articles (from the 1960s, 1970s and 1980s). In order to examine the effects of Lacanian ideas, we analysed the difference in the way of interpreting the clinical material before and after Lacan's reception. The results of this research illuminate some epistemological problems of psychoanalysis, especially the relationship between theory and clinical practice.

The analysis of cases of detection of bacteria of the genus Aeromonas in clinical material from various loci of patients of medical organizations was carried out. Their species diversity, isolation in monoculture and associations, resistance to antibiotics and spring-autumn rises indicate the etiological significance of these microorganisms in the infectious process. To improve the biological safety of aquatic biotopes, it is important to study their microbiocenosis, in particular, the circulation of bacteria of the genus Aeromonas, in order to obtain information on pathogenic properties, antibiotic resistance, and seasonal fluctuations. To prevent the spread of Aeromonas infections, it is necessary to carry out sanitary and bacteriological studies of water bodies, environmental objects and food products.

A considerable number of men are received annually into Brixton prison remanded, or committed for trial, on a charge of attempted suicide. They are all specially examined as to their mental condition, and a large number of them are remanded for that purpose. From the plentiful clinical material thus studied these remarks were prepared whilst I was on the medical staff of that prison, and I have to thank the Prison Commissioners for their permission to publish them.
