Background :  The incidence of ebitis is one indicator of the quality of hospital services with the standard set by The Infusion Nursing of Practice, which is 5%. The incidence of phlebitis is an indicator of minimum hospital service quality with a standard incidence of ≤1.5%. Purpose : Knowing the effect of using betadine ointment on the incidence of phlebitis at the intravenous infusion site at the Army Hospital TK IV. 01.07.01 Pematangsiantar. Methods : This type of research the researcher used was a quasi experiment with the equivalent control group design. The research instrument used was an observation sheet with a sample of 30 patients who had an intravenous infusion attached. Results: There was a significant effect of using betadine ointment on the incidence of phlebitis on intravenous infusion therapy. Statistical analysis using normality test, homogeneity and hypothesis testing. Conclusions and suggestions : The use of betadine ointment against the incidence of phlebitis at the intravenous infusion site has a significant relationship. Therefore the hospital management must continue to make efforts to improve services to patients. As a suggestion, room nurses should increase their knowledge through training on infection control and prevention, nosocomial infection prevention training in hospitals.

Introduction: Hospital employee performance as the spearhead of health services is a very important issue to be studied in order to maintain and improve the quality of health services. The purpose of this study was to determine the relationship between leadership style, work discipline, and compensation on employee performance at the pathology installation of the Gatot Soebroto Army Hospital in 2019. Methods: Using quantitative research with Cross Sectional approach. The sample uses a total sampling of 40 respondents. The instrument of this research used a questionnaire. Results: Relationship between work discipline factors (Pvalue = 0.027; OR = 5,844) and compensation factors (P value = 0,018; OR = 7,111) with employee performance in the anatomical pathology installation of Gatot Soebroto Hospital in 2019. While the leadership style factor (P value = 0,800) does not relate. Discussion: Two of the three variables studied were related to employee performance, namely: work discipline and compensation. While one other variable, namely leadership style is not related to employee performance.

Tuberculosis is an infectious disease caused by the Mycobacterium tuberculosis. It is a global health problem and major cause of death in resource-limited countries like Nepal. Timely diagnosis with sensitive testing methods could assist in early management of the disease. This study was conducted to compare the diagnostic performance of GeneXpert MTB/RIF and conventional acid-fast staining with M. tuberculosis culture. The study was carried out in the Department of Microbiology, Shree Birendra Army Hospital, Nepal. Samples (n=500) were tested with a GeneXpert MTB/RIF assay and acid-fast bacilli (AFB) smear microscopy. All samples were sent for M. tuberculosis conventional culture by the German-Nepal Tuberculosis Project, Kathmandu, Nepal (GENETUP). Out of a total 500 pulmonary and extrapulmonary samples tested, 97 samples were positive for M. tuberculosis by GeneXpert MTB/RIF assay. Out of the positive samples, only 95 samples were found positive by the culture method. The sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of AFB microscopy was 45.3%, 99.5%, 99.5% and 88.5%, respectively. The sensitivity, specificity, PPV and NPV of GeneXpert MTB/RIF was found to be 100%, 99.5%, 97.5% and 100%, respectively compared to the gold standard culture method. The GeneXpert MTB/RIF test was comparable with culture diagnosis of both pulmonary and extrapulmonary tuberculosis cases.

The two Grand Embassies to Europe and his view on the world helped Peter the Great to start reforms.Already as a child, he had abroad interest in medicine. Peter often followed a two-track policy. One for immediate application in the current practice and one for the development of specialists in collaboration with science. Peter established a medico-surgical hospital school in Moscow to prepare the students to become doctor medicinae and learn to make their own medical instruments along the line of the Leiden medical school. In Saint Pe tersburg, he opened a navy and an army hospital, intended to train students as a barber-surgeon for the army and navy. Also in Saint Petersburg Peter built the first factory for "mass" production to provide the military with medical instruments.His successors followed his two-track policy. Catherine the Great started to merge the two tracks. During the reign of Tsar Aleksander I and his brother Nicholas I, the merger came together and was further developed. They understood that strong cooperation between a physician and a designer is essential to create and produce useful medical instruments. If correctly designed, medical instruments and devices increase safety for the patient. We will shed light on the development and manufacture of medical instruments and appliances in Imperial Russia, an underdeveloped subject in the world medical history.

Abstrak Spondilitis tuberkulosis (TB) atau Pott’s disease adalah infeksi tulang belakang yang disebabkan Mycobacterium tuberculosis akibat TB ekstrapulmoner. Penyakit ini tergantung berbagai faktor seperti usia, status gizi, dan penyakit penyerta. Penelitian deskriptif ini dilakuakn untuk mengetahui Profil pasien spondilitis tuberkulosis di Rumah Sakit Pusat Angkatan Darat (RSPAD) Gatot Soebroto. Data diperoleh dari rekam medis sebanyak 75 sampel. Penelitian ini menunjukkan jenis kelamin laki – laki (50,7%), kelompok usia 21 – 29 tahun (29,3%), dan status gizi normal (33,3%) merupakan kelompok yang terbanyak terkena spondilitis tuberkulosis (TB). Jenis pekerjaan terbanyak merupakan ibu rumah tangga (28,0%). Sebanyak 69 pasien (92%) tidak memiliki riwayat merokok dan semua pasien tidak memiliki riwayat mengonsumsi alkohol. Hanya 1 orang pasien spondilitis TB yang memiliki hasil basil tahan asam (BTA) positif. Sebanyak 41 orang (54,7%) pasien spondilitis TB didiagnosis melalui Magnetic Resonance Imaging/ Computerized Tomographic Scan (MRI/CT SCAN) dengan lesi spondilitis TB terbanyak terdapat pada torakal. Kemampuan MRI mendiagnosis lebih baik dibandingkan PCR dan BTA.   Kata kunci: Status Gizi, Torakal, Basil Tahan Asam   The Profile of Tuberculosis Spondylitis patients (Pott's Disease) at Rumah Sakit Pusat Angkatan Darat Gatot Soebroto   Abstract Tuberculous spondylitis (TB) or Pott's disease is a spinal infection caused by Mycobacterium tuberculosis due to extrapulmonary TB. This disease depends on various factors such as age, nutritional status, and comorbidities. This descriptive study was conducted to determine the profile of tuberculosis spondylitis patients at the Gatot Soebroto Central Army Hospital (RSPAD). Data obtained from medical records of 75 samples. This study showed that male gender (50.7%), age group 21-29 years (29.3%), and normal nutritional status (33.3%) were the groups most affected by tuberculosis spondylitis (TB). Most types of work were housewives (28.0%). A total of 69 patients (92%) had no history of smoking and all patients had no history of consuming alcohol. Only 1 TB spondylitis patient had positive acid-resistant bacilli (AFB). A total of 41 people (54.7%) TB spondylitis patients were diagnosed through Magnetic Resonance Imaging / Computerized Tomographic Scan (MRI / CT SCAN) with the most TB spondylitis lesions found in the thoracic. The ability of MRI to diagnose is better than PCR and BTA.   Key words: Nutritional Status, Thoracic, Acid Resistant Basil

In January 1863, a large Confederate force swept up from Arkansas. Kelso and his men discovered the advance and were chased back to the fort at Beaver Station and then to the one at Ozark, both of which the Confederates destroyed as they marched north to their target, Springfield. The Battle of Springfield pitted about 2,300 Federals defending the town, including local men and boys in the militia and patients in the army hospital, against a like-sized Confederate force commanded by General John S. Marmaduke. The men fought all day with artillery shells flying overhead. Kelso went spying at night, creeping among the wounded and the dead. Unable to take the town, the Confederates withdrew the next morning. In the aftermath, Kelso’s conflict with his drunken commanding officer lead to court martial proceedings, but Kelso was acquitted.

Serological assessment of SARS-CoV-2 specific responses are an essential tool for determining the prevalence of past SARS-CoV-2 infections in the population especially when testing occurs after symptoms have developed and limited contact tracing is in place. The goal of our study was to test a new 10-plex electro-chemiluminescence-based assay to measure IgM and IgG responses to the spike proteins from multiple human coronaviruses including SARS-CoV-2, assess the epitope specificity of the SARS-CoV-2 antibody response against full-length spike protein, receptor-binding domain and N-terminal domain of the spike protein, and the nucleocapsid protein. We carried out the assay on samples collected from three sample groups: subjects diagnosed with COVID-19 from the U.S. Army hospital at Camp Humphreys in Pyeongtaek, South Korea; healthcare administrators from the same hospital but with no reported diagnosis of COVID-19; and pre-pandemic samples. We found that the new CoV-specific multiplex assay was highly sensitive allowing plasma samples to be diluted 1:30,000 with a robust signal. The reactivity of IgG responses to SARS-CoV-2 nucleocapsid protein and IgM responses to SARS-CoV-2 spike protein could distinguish COVID-19 samples from non-COVID-19 and pre-pandemic samples. The data from the three sample groups also revealed a unique pattern of cross-reactivity between SARS-CoV-2 and SARS-CoV-1, MERS-CoV, and seasonal coronaviruses HKU1 and OC43. Our findings show that the CoV-2 IgM response is highly specific while the CoV-2 IgG response is more cross-reactive across a range of human CoVs and also showed that IgM and IgG responses show distinct patterns of epitope specificity. In summary, this multiplex assay was able to distinguish samples by COVID-19 status and characterize distinct trends in terms of cross-reactivity and fine-specificity in antibody responses, underscoring its potential value in diagnostic or serosurveillance efforts.

Background: In-vitro fertilization or Assisted reproductive techniques (ART) is the most advanced technique of infertility treatment. In-vitro fertilization (IVF) has helped couples all over the world. However, the use of IVF has raised significant concern about the outcome of resulting pregnancies and the health of the newborns. There is a range of possible factors associated with the treatment that may contribute to potential adverse outcomes. Thus, the study was conducted to analyze the neonatal outcomes of children born by ART in the Indian context.Methods: The neonatal characteristics and complications of the live-born infants through IVF at Army Hospital R and R were analyzed in this study between March 2019 to February 2020.Results: Total 231 babies were born to the study group cases. 126 (54.54%) were singletons, 102 (44.16%) were of twin pregnancies and 1.3% were triplets. There were 65.36% term and 34.63% preterm. Of the 231 IVF neonates, 58 needed neonatal intensive care. There were 219 (94.8%) survivors, while 12 (5.2%) did not survive a week. The 16 (6.92%) survivors needed readmissions mainly due to hyperbilirubinemia. There were 106 (45.8%) babies whose weight was less than 2 kg.Conclusions: Infertility cases are usually older, and this is one reason for increased pregnancy and newborn infant complications. Neonates born through IVF appeared to be at higher risk of multiple births, prematurity, low birth weight, and other disabilities.
