Abstract. In Ukraine, among the causes of death because of digestive tract diseases, alcoholic liver disease (ALD) has the second place. Due to the significant prevalence of obesity and the growing incidence of ALD, methods are being sought to prevent the progression of the pathological process in the liver, the occurrence of complications and to improve the quality of life of such patients. The aim of the study: to examine the effect of complex treatment with ademethionine, arginine glutamate and rosuvastatin on changes in lipid and carbohydrate metabolism in patients with alcoholic liver cirrhosis (ALC) in combination with obesity. Methods. The study included 156 patients diagnosed with ALC in combination with obesity, including 18 women and 138 men aged (45.3±8.9) years and a median duration of disease (5.1±2.8) years. Patients were divided into subgroups depending on the stage of Child-Pugh decompensation and depending on the applied treatment. Results. At the stage of decompensation, lipid metabolism and leptin levels were low, which indicates the depletion of body fat depots as the disease progresses. It may be due to the progression of the liver dysfunction, as it is actively involved in regulating the formation, destruction and accumulation of fats. Changes in carbohydrate metabolism in patients with ALC in combination with obesity were characterized by a significant increase in IRI, HOMA-IR index and a decrease in the QUICKI index, indicating the presence of insulin resistance (p<0.05). In determining the adipocytokine values, it was found that in decompensated liver function, the leptin rates decreased and the levels of adiponectin increased. Higher leptin content in the stage of compensation and subcompensation is also associated with increased secretion of adipose tissue. At the stage of decompensation, fat depots are depleted, so leptin levels are reduced. This decrease is directly related to the Child-Pugh and MELD scores. Adiponectin levels were decreased in the stage of compensation and increased with the progression of the disease and correlated with disease severity and the MELD score. It is thought that an increased adiponectin level indicates the level of anti-inflammatory reaction in response to hepatocyte damage. Significant deterioration in carbohydrate metabolism, adiponectin and leptin in patients receiving basic treatment was accompanied by deterioration of their condition and increased the risk of 3-month mortality. After the course of treatment in patients of group receiving ademethionine, arginine, glutamate and rosuvastatin at the stage of compensation and subcompensation, the rates of lipid, carbohydrate metabolism, adiponectin and leptin significantly improved and differed from those in patients receiving basic treatment and combination of basic treatment, ademethionine and arginine glutamate (p<0.05). At the stage of decompensation in the scheme with the inclusion of rosuvastatin it was possible to normalize the levels of HDL cholesterol, VLDL cholesterol, atherogenic coefficient and leptin, reduce the levels of adiponectin, IRI, HOMA-IR, HbA1c and increase the QUICKI index, which was accompanied by a decrease in Child-Pugh severity score and 3 month mortality MELD score. Conclusions. In patients with ALC in combination with obesity, the inclusion in the treatment of ademethionine, arginine glutamate and rosuvastatin helps to improve the course of the disease according to the lipid and carbohydrate metabolism, Child-Pugh and MELD scores.

The features of the clinical course of rotavirus infection in infants with perinatal exposure to HIV were studied. Serum level of procalcitonin as a marker of inflammatory activity and the need for antimicrobial therapy was determined. The efficiency of the proposed additions to basic treatment (low-lactose mixture for two months, antibiotic therapy in case of elevated procalcitonin level) was demonstrated.

Background. Gout is still one of the major health problems despite significant advances in treatment in recent years. It has been proved that pathogenetic mechanisms of development and progression of gout are associated with nonalcoholic fatty liver disease. Complex pathogenic treatment of patients aimed at different parts of the pathological process has recently been supplemented with the enterosorbents. Objective. The aim of the research is to study the clinical features of gout with concomitant nonalcoholic fatty liver disease (NAFLD) and to evaluate the effect of carbon enterosorbent on its course. Methods. 123 patients were involved in the study. They were divided into 2 groups: group 1 included patients with gout without liver damage, and group 2 included patients with concomitant NAFLD. Each of these groups was divided into subgroups, in which the patients received carbon enterosorbent carboline plus basic treatment. The control group consisted of 30 healthy persons. Anamnesis, physical examination, uric acid (UA), C-reactive protein (CRP) content, erythrocyte sedimentation rate (ESR) in serum were determined. Gout activity was evaluated using the Gout Activity Score (GAS). Results. Basic treatment in combination with carbon enterosorbent contributed to faster cure of intoxication, pain and joint syndromes, as well as decrease of the inflammatory process activity. Conclusions. The course of gout in the patients with concomitant NAFLD is more severe. Adding of carbon granular enterosorbent carboline in the complex treatment of patients with gout with or without concomitant NAFLD in the exacerbation phase contributes to a faster cureing dynamics of clinical and laboratory manifestations of the disease.

Abstract Background: Hip involvement is an important cause of disability and poor prognostic in patients with spondyloarthritis (SpA). TNF-α inhibitor treatment had been demonstrated to be effective in SpA patients with hip arthritis, however, quantitative assessment with magnetic resonance imaging (MRI) in the long-term follow-up still needs further application.Methods: A total of 239 patients were involved in this study, MTX and SSZ were given as basic treatment. 165 patients received TNF-α inhibitor plus basic treatment and 74 patients received basic treatment only as control. The clinical symptoms were assessed at baseline, week 12, week 24, and week 52. The MRI performances of hip arthritis including bone marrow edema (BME) and synovitis were quantitatively assessed using Hip MRI Inflammation Scoring System (HIMRISS).Results: The clinical symptoms of ESR, CRP, BASDAI, Harris, and ASDAS-ESR of both groups got significant clinical remission at week 52 (p<0.001). However, the disease activity change levels at week 52 in control group were significantly worse than TNF group. In week 52, MRI performance showed significant remission trend in TNF-α inhibitor group versus baseline of which total HIMRISS scores decreased significantly ( 26.49±10.37 vs 20.59±9.41, p<0.001), but the control group only had slight improvement trend (p<0.05).Conclusions: TNF-α inhibitor could significantly and better improve the clinical and MRI manifestation of hip involvement in SpA patients than controls. Quantitative MRI assessment combined with clinical assessment could accurately evaluate the treatment effect of TNF-α for SpA patients with hip involvement, and help to guide targeted treatment.Trial registration: Chinese Clinical Trial Registry, ChiCTR-ONRC-11001846, Registered 28 November 2011.
