In this work examined the effect of various types of basic therapy on laboratory, including immunological parameters in ankylosing spondylitis. The influence of basic treatment of patients with ankylosing spondylitis with sulfasalazine, methotrexate and rituximab on laboratory indicators of the disease such as ESR, CRP, fibrinogen and immunological indicators such as IgM, IgG and IgA was evaluated. It has been established that all three basic drugs (sulfasalazine, methotrexate and rituximab) in ankylosing spondylitis have a positive effect on the dynamics of laboratory and immunological parameters. But at the same time, rituximab has a more pronounced and earlier effect.

To overcome the lack of proper guidelines (except some basic information) in classical texts about procedures, including Sirasseka, Kayaseka, Pinda Sveda, Annalepana, Shirolepana, Kalavidhi, Snehapana Vidhi are elaborated in this text.

Coronary heart disease (CHD) is a leading cause of high mortality, disability and reduced quality of life for patients. At the heart of coronary heart disease - ischemia and activation of oxidative stress. With concomitant hypertension, the condition of patients and the prognosis for the course of their disease deteriorates significantly. The prognosis of coronary heart disease, the occurrence of new hospitalizations or deaths from cardiovascular disease can be further assessed by monitoring ST2 and malonic dialdehyde (MDA). Traditional treatment of coronary heart disease does not always guarantee the achievement of the desired clinical results. That is why it is advisable to use in the complex treatment of patients with coronary heart disease and concomitant hypertension cytoprotectors and drugs that have antioxidant properties, such as meldonium and folic acid. The aim of the research. Detect anti-ischemic activity in meldonium and folic acid and evaluate the effect of these drugs on the level of ST2, MDA in the blood in patients with coronary heart disease and concomitant hypertension. Materials and methods. The research included 80 patients (women - 4, men - 76, aged 57,7 ± 9,8 years) with diagnoses of coronary heart disease: stable angina, functional class II-III (FC), chronic heart failure (CHF) I-IIa, FC II-III ", with concomitant arterial hypertension II-III stages, 2-3 degrees, risk 4 (very high), CHF I-IIa, FC II-III". Patients were divided into 4 groups of 20 patients, respectively: 1) patients who, in addition to basic therapy, were prescribed folic acid at a dose of 0.8 mg / d for six months; 2) patients who, in addition to basic treatment, were prescribed meldonium at a dose of 750.0 mg / d for six months; 3) patients who, in addition to basic treatment, were prescribed folic acid at a dose of  0.8 mg / d and meldonium at a dose of 750.0 mg / d for six months; 4) patients who were prescribed only basic antianginal, disaggregating, hypolipidemic treatment.The number of anginal attacks and doses of nitroglycerin during the day were determined, the results of the test with a 6-minute walk and the plasma concentration of ST2 and MDA before and after 6 months of treatment with meldonium and folic acid were evaluated. Results. Found anti-ischemic activity in meldonium and its combination with folic acid. The combination of these drugs as an adjunct to basic therapy significantly reduced the concentration of ST2. Both drugs alone or in combination reduced the concentration of MDA in the blood of patients. Conclusions. Our research showed the presence of anti-ischemic activity in meldonium, both when added to the basic therapy of patients with coronary heart disease and concomitant hypertension, and when used together with folic acid. In the groups of patients where these drugs and their combinations were used, the frequency of anginal attacks and the number of doses of nitroglycerin used during the day were significantly reduced. The combined use of folic acid and meldonium when added to the basic therapy after 6 months significantly reduced the concentration of ST2 in the blood. Folic acid and meldonium showed pronounced antioxidant properties. At separate adding of these medications to basic therapy or their combined adding significantly decreased the concentration of MDA - a marker of oxidative stress. Therefore, we consider it appropriate to use folic acid and meldonium in the complex treatment of patients with coronary heart disease with stable angina and concomitant hypertension.

To summarize the experience of Chief Physician Wang Defen in the treatment of thyroid nodules. According to many years of clinical experience, Chief Physician Wang Defen believes that the occurrence of thyroid nodules is mostly affected by emotional factors. Qi stagnation, liver depression and fire, phlegm and Qi are the pathogenesis of the disease, which is closely related to liver Qi stagnation. Based on the basic treatment principle of Nourishing Yin, clearing heat, soothing liver and strengthening spleen, the prescription chose Danzhi Xiaoyao Powder plus or minus can often achieve good clinical efficacy.

Background. This study is aimed at exploring the significance of the expression of miR-23a and miR-146a in patients with periodontitis and their correlations with inflammatory factors. Methods. A total of 120 patients with chronic periodontitis admitted to the department of stomatology in Yantai Yuhuangding Hospital from August 2017 to December 2018 were enrolled as a study group, and 80 healthy volunteers in physical examination during the same period were enrolled as a control group. The expression of miR-23a, miR-146a, interleukin-1β (IL-1β), interleukin-6 (IL-6), and interleukin-17 (IL-17) in the saliva of people in the two groups was determined using the quantitative real-time polymerase chain reaction (qRT-PCR) and enzyme-linked immunosorbent assay (ELISA). Results. The study group showed significantly higher relative expression of saliva miR-23a and miR-146a than the control group. The area under the curve (AUC) of saliva miR-23a and miR-146a for diagnosing periodontitis was 0.857 and 0.886, respectively. The expression of saliva miR-23a and miR-146a increased with the deterioration of periodontitis in the patients. After basic treatment, the study group showed significantly decreased expression of saliva miR-23a and miR-146a. Patients in the study group showed significantly higher levels of saliva IL-1β, IL-6, and IL-17 than those in the control group, and their saliva miR-23a and miR-146a were positively correlated with their saliva IL-1β, IL-6, and IL-17, respectively. Conclusion. Saliva miR-23a and miR-146a can be used as biomarkers for the diagnosis and assessment of periodontitis, and they may have regulatory relationships with IL-1β, IL-6, and IL-17.

Pattern is central in the pattern-focused case conceptualization approach. This chapter highlights the five basic treatment challenges for each of the eight common patterns in everyday clinical practice. It first defines pattern and provides a clinical strategy for quickly identifying and differentiating the basic patterns. Then, it describes eight patterns: avoidant, borderline, dependent, histrionic, narcissistic, obsessive–compulsive, paranoid, and passive–aggressive. Each of these patterns is discussed in the following format: pattern description, pattern development, pattern types and triggers, and treatment challenges. Clinicians who understand patterns and can identify them easily in clients are more likely to be able to explain and guide treatment, as well as anticipate treatment challenges.

Objective. To systematically evaluate the effect of orthodontic treatment (ODT) on anterior tooth displacement (ATD) in patients with periodontal disease. Methods. PubMed, Web of Science, Embase, China National Knowledge Infrastructure, and Wanfang databases were electronically searched for relevant literature studies on ODT and basic treatment for ATD in patients with periodontal disease, and then the related journals and reference lists of the included studies were manually searched. The search time was set from January 2010 to May 2021. Stata 16.0 software was used for meta-analysis. Results. Totally, 783 articles were retrieved, and finally, 14 studies were included. The effective rate of basic treatment combined with OTD was significantly higher than that of basic treatment alone (OR = 7.27, 95% CI: 3.76, 14.04). Specifically, the combined treatment led to lower values of periodontal pocket depth (SMD = −2.30, 95% CI: −2.94, −1.66), anterior overjet (SMD = −2.75, 95% CI: −3.72, −1.78), anterior overbite (SMD = −2.13, 95% CI: −3.16, −1.10), and periodontal bleeding index (SMD = −4.25, 95% CI: −5.48, −3.03) compared with those of basic treatment alone. Conclusion. Compared with basic treatment, ODT combined with basic treatment is more effective for patients with periodontal disease-caused ATD and can also improve the clinical symptoms of patients.

Background. In recent years, thyroid diseases occupy the top places in the structure of the endocrine pathology. There exists a tight functional relationship between the thyroid and reproductive systems, which leads to a high probability of the development of combined disorders in one of these links of homeostasis. The problem of reproductive health disorders is of particular concern around the world and is relevant to the study of the impact of thyroid diseases on both fertility and pregnancy, especially in conditions of comorbidity. The purpose of the study was to investigate the effect of myo-inositol on the reproductive function of women with subclinical hypothyroidism on the background of autoimmune thyroiditis and obesity. Materials and methods. The study included 98 patients aged 18–40 years with subclinical hypothyroidism, overweight, or obesity on the background of autoimmune thyroiditis. They were randomly subdivided into two groups. Patients of the first group (n = 49) before the basic treatment received myo-inositol at a dose of 2000 mg/day and cholecalciferol at a dose of 2000 IU/day. Patients of the second group (n = 49) before the basic treatment received only cholecalciferol at a dose of 2000 IU/day. Results. Vitamin D deficiency was observed in 90.81 % of women with subclinical hypothyroidism, and vitamin D insufficiency in 9.19 %. A negative correlation was found between the level of 25(OH)D and the level of TPO-Ab (r = –0.189; p < 0.05). There was a weak negative correlation between the level of 25(OH)D and the level of the HOMA-IR (r = –0.168; p < 0.05). The administration of myo-inositol together with vitamin D led to a significant increase in the content of 25(OH)D, as well as to a decrease in the titer of TPO-Ab. Conclusions. The positive effect of myo-inositol drugs together with vitamin D on the functional state of the thyroid gland, on the level of TPO-Ab and HOMA-IR in women of reproductive age with subclinical hypothyroidism and obesity has been established.
