Background Albumin transfusion for the treatment of neonatal hypoalbuminemia may reduce morbidity. In conditions with disrupted endothelial integrity (e.g., sepsis and critical illness), the administered albumin may leak into the interstitial space, hence, serum albumin levels may fall below expected levels after transfusion. To date, few studies have been done to evaluate the risk factors for failure to achieve normal neonatal albumin levels after transfusion.Objectives To determine the risk factors for failure to achieve normal neonatal albumin levels after transfusion.Methods We performed a case-control study in the Neonatal Ward of Dr. Sardjito Hospital from 2007 to 2012. Normal albumin level was defined as above 3 g/dL. The case group included neonates with post-transfusion albumin levels <3 g/dL and the control group included those with post-transfusion albumin ≥3 g/dL. Subjects received intravenous transfusions of 25% or 20% albumin according to the clinical standard of the Neonatal Ward of Dr. Sardjito Hospital. Neonates with very low birth weight, severe birth trauma, burn injuries, severe bleeding, or incomplete medical records were excluded. The data were analyzed with logistic regression test.Results From January 2007 to December 2012, 124 neonates were enrolled in the study. Multivariate analysis showed that low albumin levels before transfusion (OR 12.27; 95%CI 2.17 to 69.30), presence of critical illness (OR 4.01; 95%CI 1.49 to 10.79), diagnosis of sepsis (OR 3.56; 95%CI 1.36 to 9.32), and the >24-hour interval between albumin examination and transfusion (OR 0.06; 95%CI 0.01 to 0.37) were significant risk factors affecting the failure to achieve normal albumin levels.Conclusions Failure to achieve normal albumin levels after transfusion in neonates was significantly associated with low albumin level prior to transfusion, critical illness, sepsis, and >24-hour interval between transfusion and post-transfusion albumin examination.[Paediatr Indones. 2016;56:129-33.].

The USDA has established processing schedules for beef products based on the destruction of pathogens. Several enzymes have been suggested as potential indicators of heat processing. However, no relationship between the inactivation rates of these enzymes and those of pathogenic microorganisms has been determined. Our objective was to compare the thermal inactivation of Escherichia coli O157:H7 and Salmonella senftenberg to those of endogenous muscle proteins. Inoculated and noninoculated ground beef samples were heated at four temperatures for predetermined intervals of time in thermal-death-time studies. Bacterial counts were determined and enzymes were assayed for residual activity. The D values for E. coli O157:H7 were 46.10, 6.44, 0.43, and 0.12 min at 53, 58, 63, and 68°C, respectively, with a z value of 5.60°C. The D values for S. senftenberg were 53.00, 15.17, 2.08, and 0.22 min at 53, 58, 63, and 68°C, respectively, with a z value of 6.24°C. Apparent D values at 53, 58, 63, and 68°C were 352.93, 26.31, 5.56, and 3.33 min for acid phosphatase; 6968.64, 543.48, 19.61, and 1.40 min for lactate dehydrogenase; and 3870.97, 2678.59, 769.23, and 42.92 min for peroxidase; with z values of 7.41,3.99, and 7.80°C, respectively. Apparent D values at 53, 58, 63, and 66°C were 325.03, 60.07, 3.07, and 1.34 min for phosphoglycerate mutase; 606.72, 89.86, 4.40, and 1.28 min for glyceraldehyde-3-phosphate dehydrogenase; and 153.06, 20.13, 2.25, and 0.74 min for triose phosphate isomerase; with z values of 5.18, 4.71, and 5.56°C, respectively. The temperature dependence of triose phosphate isomerase was similar to those of both E. coli O157 :H7 and S. senftenberg, suggesting that this enzyme could be used as an endogenous time-temperature indicator in beef products.

<em>Premature infants had been experience of pain in neonatal ward that occur each day during treatment. Nurse need to do an intervention for reduce the scale of pain on premature infants. Pain management have to do at birth because of the repeated painful procedures in early life can affect the development of central nerve system permanently. The objective of this study was to describe the application of Kolcaba Comfort’s theory through facilitated tucking accompanied with ‘being with-talking to’ techniques in premature’s infant at high risk infant care. The method that used was case study by applying the four contexts of comfort’s experience associated with the three types of comfort based on Kolcaba Comfort’s theory in providing nursing care on five premature infants who have some painful procedures. The nursing intervention through facilitated tucking accompanied by ‘being with-talking to’ based on the principles of Kolcaba Comfort’s theory gave a positive result against premature infants’s comfort level. Four from five premature infants are in the level of transcendence which is a type of supreme comfort after the ease and relief. Kolcaba Comfort’s theory can be applied within the scope of neonatal care due in accordance with the developmental care of the infants and could reduce the scale of pain.</em>

Background: Preterm Low Birth Weight babies carry relatively higher risk of perinatal and neonatal mortality, retarded growth and development subsequently. In this study, some maternal risk factors were investigated as it seems to be important to study these factors on regional basis. Objective: To find out the maternal risk factors, clinical profile and short term outcome of pre-term low birth weight neonates by gestational age, birth weight, Occipitofrontal Circumference, length, different presentation (asphyxia, hypothermia, feeding difficulties, signs of infection) on admission and the morbidity and mortality of preterm LBW. Materials and Methods: A descriptive type of cross sectional study was done in neonatal ward of Jalalabad Ragib Rabeya Medical College Hospital, Sylhet from May, 2015 to April, 2016. Hundred cases were selected from neonatal ward and 100 controls were selected from obstetrics ward. Results: Among 100 pre-term LBW neonates, 52 were male and 48 were female. Only 22% mother took regular antenatal check up in case group and 55% in control group. Mean gestational age of neonates were 33.3±2.1 weeks. Important risk factors were primi parity (58% in cases vs 34% in control), hypertension (19% vs 5%), pre-eclampsia (26% vs 12%), prolonged rupture of membrane (33% vs 7%), APH (22% vs 0%), acute infection (8% vs 1%), multiple birth (17% vs 2%). Common clinical presentations were prematurity and LBW. Commonest morbidity was feeding problem (70%). The overall survival rate was 87%. Most common cause of death was septicemia (30%). Conclusion: all the identified risk factors were significantly higher in pre-term LBW babies than those of control. KYAMC Journal Vol. 11, No.-2, July 2020, Page 77-82
