Condylar resorption and its incidence at an early age or after orthognathic surgery are well-documented issues, but it is associated with high controversies regarding its etiology and management. Lack of clear understanding of its pathogenies and the limitation of the available evidence inherited for the development of a systematic treatment approach and most previous studies recommended further investigation of the problem. This review aims to cover the problem of condylar resorption and its association with orthodontic and orthognathic surgery as well as to discuss the prospective treatment measure available through the literature. According to the available literature, most previous studies covered the condylar resorption of this type of patient (skeletal class II with high mandibular plane angle) following the orthognathic surgery. However, no studies investigated the long-term condition of the condyles of the involved patient before the surgery. Furthermore, there is no evidence for the orthodontic management of this challenging condition, and most recommendations come from case reports. So, it is crucial to assess the changes that happened to the condyle during the preoperative orthodontic and correlate the finding with what happened after the surgery to provide clear evidence that may help further understand the problem of condylar resorption. This evidence could be beneficial for both patients and clinicians. CT or CBCT images have been regarded as the best choice of detailed diagnosis and investigation of condylar resorption. The incidence of condylar resorption after orthognathic surgery with or without disc repositioning of skeletal class II with high mandibular plane angle is extremely suspected. However, artificial joint replacement is considered a gold standard treatment measure in severe or relapsed cases.

Skeletal Class II malocclusion with mandibular deficiency is one of the most common problems that patients seek treatment. Adult patients with severe skeletal Class II malocclusion need orthognathic surgery for successful treatment. Bilateral sagittal split osteotomy (BSSO) is the most often preferred technique for these patients. This case report briefs about two male patient of age 24 years presented with Class II Skeletal relation, mesoprosopic facial form, horizontal growth pattern and Angle’s Class II div 1 malocclusion who were treated with Bilateral sagittal split osteotomy (BSSO) mandibular advancement. The ideal anteroposterior relation was established along with a Class I molar, incisor, canine relationship and ideal overjet, overbite and the overall facial esthetics were significantly improved. Combined surgical-orthodontic treatment aims to obtain a more harmonious facial, skeletal, dental and soft tissue relationship with an added patient self esteem.

ABSTRACT Objective To investigate the occlusal configurations of the hyperdivergent skeletal Class II malocclusion and their alterations during the camouflaging treatment in an attempt to identify occlusal changes that might be related to mandibular counterclockwise rotation. Materials and Methods Cephalograms of 126 subjects with hyperdivergent skeletal Class II malocclusion and 126 subjects with a clinically normal skeletal pattern were chosen. Several measurements were calculated and compared between the groups. To examine the effects of treatment, two groups were established according to mandibular rotation: counterclockwise rotation (CCR) and the opposite clockwise rotation (CR). After 40 subjects were excluded, the other 86 Class II subjects were assigned to CCR (n  =  22) and CR (n  =  64). Their pretreatment (T1), posttreatment (T2), and postretention (T3) cephalograms were obtained. Measurement changes (T3-T1) were analyzed in each group and compared between groups. Results Compared with the normal skeletal pattern, the cant of the occlusal plane (OP) of the study subjects was significantly steeper and the vertical heights of the incisors were significantly larger for the malocclusion. Compared with the changes in CR, there was a prominent reduction of the OP canting with remarkable intrusion of the maxillary incisor in CCR. Conclusion Increased OP canting with overerupted incisors is evident in the hyperdivergent skeletal Class II malocclusion. During the camouflaging treatment, reduction of OP canting could occur. It was accompanied by mandibular counterclockwise rotation and intrusion of the maxillary incisor.

Background: Following the morphological features of different races and ethnic groups, knowledge of standard dentofacial patterns of each ethnic group is essential. Therefore, this study aimed to explain cephalometric standards for the Zanjanian population according to Downs' analysis and compare them to Caucasian individuals. Methods: Seventy lateral cephalometries of Zanjanian adults (17 - 29 years old) who had been referred to a private orthodontic office in Zanjan, Iran with class I molar and canine relationship and normal overjet and overbite as well as minimum crowding/spacing/rotations were scanned and traced with the Novatech scanner and Dolphin software version 10. Next, statistical analyses were performed in order to compare the Zanjanian population to Caucasians. Results: We found a significant difference between males and females in terms of interincisal angle, incisor-occlusal angle, incisor-mandibular plane angle, upper incisor proclination, facial angle, and angle of convexity. Discussion: The analysis of six statistically significant parameters indicates that the upper and lower incisors in women of Zanjan were proclined and protruded compared with those of Caucasian subjects. Due to the statistical analysis on facial angle and angle of convexity, women also show more maxillary prognathism and skeletal class II pattern. Generally, a comparison of Zanjanian population cephalometrics based on Downs' analysis showed an increase in maxillary prognathism, maxillary and mandibular incisal protrusion and posterior rotation of the mandible. Conclusions: In conclusion, the Zanjanian population tends to have more dental and skeletal class II patterns than the Caucasians. In addition, gender comparison indicates lower and upper-incisors protrusion in women of Zanjan.

Abstract Background: To retrospectively evaluate postsurgical stability and condylar morphology for skeletal class II malocclusion patients with condylar resorption (CR) treated with orthognathic surgery.Methods: Thirty-five patients treated with combined orthodontic and orthognathic surgery between 2014 and 2018 were enrolled in this retrospective study. CT scans were acquired preoperatively (T0), 2–7 days after surgery (T1), and 1 year postoperatively (T2). The amount of mandibular advancement, postsurgical relapse, condylar morphology and joint spaces were analysed respectively. Statistical analysis was performed using R, version 3.4.3 (R Development Core Team 2010).Results: The average mandibular advancement and counter-clockwise rotation were 5.51 mm and -2.82 degrees respectively. The average relapse was 1.08 mm (19.6% of the advancement) and 1.13 degrees. The condylar volume showed a postoperative reduction of 161.86 mm3(13.7% of initial condylar volume). AJS increased after surgery and gradually returned to its original state, while SJS and PJS decreased and remained stable. Surgical advancement of B point was significantly correlated with skeletal relapse. The optimal cut-off values were as follows: MP-FH (40.75°); ramus height (51.125 mm); SJS (1.63 mm); surgical displacement (4.72 mm); CCR (-4.3°); AJSC (1.07 mm).Conclusions: Skeletal class II patients with CR have a high risk of postsurgical condylar resorption, which was most related to nonsurgical risk factors such as gender(female) and condylar angle. Condylar had moved posteriorly to the concentric position after surgery and remained quite stable through the 1-year follow-up. Preoperative skeletal patterns such as lower ramus height and surgical procedures such as larger advancement of the mandible were closely associated with sagittal relapse of the mandible.

ABSTRACT Objective: This study evaluated the dental, skeletal and soft tissue effects in Class II malocclusion patients treated with Distal Jet appliance, compared to an untreated control group. Methods: 44 patients with Class II malocclusion were divided into two groups: Group 1 (experimental) - 22 patients, mean age of 12.7 years, treated with the Distal Jet appliance for a mean period of 1.2 years; Group 2 (control) - 22 untreated patients, mean age of 12.2 years, followed by a mean period of 1.2 years. Lateral cephalograms were obtained before treatment (T0) and at the end of the distalization (T1).Independent t test was used to identify intergroup differences. Results: When compared to control group, the Distal Jet produced a significant increase in mandibular plane angle (0.7 ± 2.0o). The maxillary second molars presented distal inclination (6.6 ± 3.8o), distalization (1.1 ± 1.1 mm) and extrusion (1.3 ± 2.1 mm). The maxillary first molars distalized by 1.2 ± 1.4 mm. The maxillary first premolars mesialized by 3.4 ± 1.1 mm. The maxillary incisors showed slight labial tipping of 4.3 ± 4.7o and were protruded by 2.4 ± 1.7 mm. There were no significant changes in the facial profile. The overjet increased 1.5 ± 1.1 mm and overbite had no significant changes. Conclusion: The Distal Jet appliance is effective to distalize the maxillary first molars, but promotes increase in mandibular plane angle, distal inclination, extrusion and distalization of maxillary second molars, mesialization of maxillary first premolars, proclination and protrusion of maxillary incisors, and increase in overjet, when compared to a control group.

Functional appliance is an effective way of treating skeletal Class II malocclusion in children and adolescents. A 12 months stepwise mandibular advancement protocol with Herbst appliance has been proved to enhance condylar growth and improve mandibular prognathism. The present case report documents a 12-year-old boy presenting with Angle’s Class II, division 1 malocclusion associated with excessive overjet (11 mm), 100% deep bite, and retrognathic mandible. He was treated by a phase I growth modification therapy using twin block appliance with lip pads in a stepwise mandibular advancement protocol followed by a phase II preadjusted Edgewise appliance therapy.
