AbstractWe report an unusual case of fully developed fetal intestinal segment(s) within a nodule on the chorionic plate of the placenta of a 27-year-old female patient at 37 weeks gestation with spontaneous vaginal delivery. Gross examination of the placenta revealed a chorionic plate nodule near the insertion of the umbilical cord, which, upon microscopic evaluation, raised the differential diagnostic possibilities of placental teratoma, vitelline/omphalomesenteric duct anomaly, and intestinal organoid differentiation. We discuss the distinguishing features, morphogenesis, and clinical significance of the aforementioned entities and review the pertinent medical literature.

Unusual presentation of multiple myeloma (MM) poses a diagnostic dilemma. The objective of this case report is to document the unique presentation of MM and to highlight the importance of backache which could be the only indicator for the diagnosis of MM. Although a history of trauma in the past is present in this case, but there is no molecular basis/evidence to declare trauma as a risk factor for MM. It can be concluded that the patients with backache should be thoroughly investigated to rule out any possibility of malignant disease process.Bangladesh Journal of Medical Science Vol.17(2) 2018 p.311-315

Parameniscal cysts are a common entity in daily practice and are found in association with a meniscal tear. Vacuum phenomenon within joints and discs can be seen secondary to degeneration or application of traction, which is believed to be released from adjacent soft tissues. Although the vacuum phenomenon is frequently present within the joints, it is rare within a parameniscal cyst which resulted in a diagnostic dilemma. In this article, we present an unusual case with a combination of two rare entities, vacuum phenomenon within a large parameniscal cyst, which has not been reported so far along with management considerations.

ABSTRACT A 21-year-old lady, primipara presented with breathlessness on exertion and generalized swelling of three weeks duration. Clinical examination revealed anasarca and features of cardiac failure. After evaluation, a diagnosis of peripartum cardiomyopathy was established based on echocardiographic findings of dilated cardiac chambers and poor left ventricular function. She responded well to treatment. The case is being reported for the diagnostic dilemma and rarity.
