OBJECTIVEVarious surgical treatments have been reported for vertebral pseudarthrosis after osteoporotic vertebral fracture (OVF). However, the outcomes are not always good. The authors now have some experience with combined anterior-posterior short-segment spinal fusion (1 level above and 1 level below the fracture) using a wide-foot-plate expandable cage. Here, they report their surgical outcomes with this procedure.METHODSBetween June 2016 and August 2018, 16 consecutive patients (4 male and 12 female; mean age 75.1 years) underwent short-segment spinal fusion for vertebral pseudarthrosis or delayed collapse after OVF. The mean observation period was 20.1 months. The level of the fractured vertebra was T12 in 4 patients, L1 in 3, L2 in 4, L3 in 3, and L4 in 2. Clinical outcomes were assessed using the lumbar Japanese Orthopaedic Association (JOA) scale and 100-mm visual analog scale for low-back pain. Local kyphotic angle, intervertebral height, bone union rate, and instrumentation-related adverse events were investigated as imaging outcomes. The data were analyzed using the Wilcoxon signed-rank test.RESULTSThe mean operating time was 334.3 minutes (range 256–517 minutes), and the mean blood loss was 424.9 ml (range 30–1320 ml). The only perioperative complication was a superficial infection of the posterior wound that was cured by irrigation. The lumbar JOA score and visual analog scale value improved from 11.2 and 58.8 mm preoperatively to 20.6 and 18.6 mm postoperatively, respectively. The mean local kyphotic angle and mean intervertebral height were 22.6° and 28.0 mm, respectively, before surgery, −1.5° and 40.5 mm immediately after surgery, and 7.0° and 37.1 mm at the final observation. Significant improvement was observed in both parameters immediately after surgery and at the final observation when compared with the preoperative values. Intraoperative endplate injury occurred in 8 cases, and progression of cage subsidence of 5 mm or more was observed in 2 of these cases. Proximal junctional kyphosis was observed in 2 cases. There were no cases of screw loosening. No cases required reoperation due to instrument-related adverse events. Bone union was observed in all 14 cases that had CT evaluation.CONCLUSIONSThis short-segment fusion procedure is relatively minimally invasive, and local reconstruction and bone fusion have been achieved. This procedure is considered to be attempted for the surgical treatment of osteoporotic vertebral pseudarthrosis after OVF.

Η κύφωση από ένα οστεοπορωτικό σπονδυλικό κάταγμα θεωρείται παράγοντας κίνδυνου για νέα κατάγματα, ιδιαίτερα στους παρακείμενους σπονδύλους. Όμως, ο ρόλος της παραμόρφωσης της τελικής πλάκας δεν έχει εκτιμηθεί. Η παρούσα μελέτη ελέγχει την υπόθεση ότι η αλλαγή στις μηχανικές ιδιότητες του μεσοσπονδύλιου δίσκου μετά από κάταγμα της τελικής του πλάκας, ακόμη και επί απουσίας κύφωσης, αλλάζει την μεταφορά φορτίων και προδιαθέτει σε παρακείμενα σπονδυλικά κάταγμα. Χρησιμοποιήθηκαν οκτώ ανθρώπινα θωρακοσφυϊκά παρασκευάσματα, αποτελούμενα από πέντε σπονδύλους. Για την επιλεκτική θραύση της μίας τελικής πλάκας του μεσαίου σπονδύλου δημιουργήθηκε κενό στο σπογγώδες οστό κάτω από την τυχαία επιλεγμένη πλάκα και το παρασκεύασμα συμπιέστηκε σε θέση κάμψης. Στη συνέχεια εφαρμόστηκε ροπή έκτασης υπό συμπιεστικό φορτίο 150 Ν, η οποία αποκατέστησε το ύψους του πρόσθιου τοιχώματος και την κυφωτική παραμόρφωση του σπονδυλικού σώματος, ενώ η τελική πλάκα παρέμεινε σημαντικά παραμορφωμένη. Το σπονδυλικό σώμα πληρώθηκε στην αναταγμένη θέση με πολυμεθακρυλικό τσιμέντο μετά τον προσεκτικό καθαρισμό του από τις οστικές δοκίδες ώστε να διασφαλιστεί η ομοιόμορφη κατανομή του τσιμέντου κάτω από τις τελικές πλάκες. Κάθε παρασκεύασμα ελέγχθηκε σε κάμψη και έκταση (ροπή ±6 Nm) υπό φορτίο 400 Ν, ενώ γινόταν καταγραφή της πίεση στους δίσκους άνω και κάτω του μεσαίου σπονδύλου και της τάσης (strain) στο πρόσθιο τοίχωμα των παρακείμενων σπονδύλων. Η πίεση των δίσκων στα άθικτα παρασκευάσματα αυξήθηκε κατά την κάμψη κατά 26 ± 14%. Μετά την ενίσχυση με τσιμέντο η πίεση των δίσκων κατά την κάμψη αυξήθηκε κατά 15 ± 11% στους δίσκους με άθικτη τελική πλάκα, ενώ μειώθηκε κατά 19,0 ± 26,8% στους δίσκους με παραμορφωμένη τελική πλάκα. Κατά την κάμψη, η συμπιεστική φόρτιση στο πρόσθιο τοίχωμα του σπονδύλου που βρισκόταν δίπλα από την παραμορφωμένη πλάκα αυξήθηκε κατά 94,2 ± 22,8% σε σχέση με τα άθικτα παρασκευάσματα (p<0,05), ενώ δεν άλλαξε σημαντικά στο σπόνδυλο δίπλα στην μη παραμορφωμένη πλάκα (18,2 ± 7,1%, p>0,05). Επακόλουθη κάμψη με συμπίεση προκάλεσε παρακείμενα κατάγματα δίπλα από την παραμορφωμένη πλάκα σε έξι παρασκευάσματα και μη παρακείμενο κάταγμα σε ένα, ενώ σε άλλο ένα παρασκεύασμα δεν παρατηρήθηκε νέο κάταγμα.Στους δίσκους με ακέραιες τελικές πλάκες η πίεση του πυρήνα αυξάνεται κατά την κάμψη αποτρέποντας την υπερβολική συγκέντρωση φορτίου στο πρόσθιο τοίχωμα. Ο αυξημένος χώρος που διατίθεται στον πυρήνα μετά από ένα οστεοπορωτικό κάταγμα επηρεάζει την ικανότητά του να κατανέμει το φορτίο και αναγκάζει τον ινώδη δακτύλιο να μεταφέρει περισσότερο φορτίο κατά την κάμψη, με αποτέλεσμα την υπέρμετρη καταπόνηση του πρόσθιου τμήματος του παρακείμενου σπονδύλου που προδιαθέτει σε κάταγμα ακόμη και μετά από διόρθωση της κύφωσης. Η παρούσα μελέτη υποδεικνύει ότι η διόρθωση της παραμόρφωσης της τελικής πλάκας μπορεί να έχει ρόλο στη μείωση του κινδύνου παρακείμενων καταγμάτων.

OBJECT This study was designed to assess the relationship between insurance status and likelihood of receiving a neurosurgical procedure following admission for either extraaxial intracranial hemorrhage or spinal vertebral fracture. METHODS A retrospective analysis of the Nationwide Inpatient Sample (NIS; 1998–2009) was performed. Cases of traumatic extraaxial intracranial hematoma and spinal vertebral fracture were identified using International Classification of Diseases, Ninth Revision (ICD-9) diagnosis codes. Within this cohort, those patients receiving a craniotomy or spinal fusion and/or decompression in the context of an admission for traumatic brain or spine injury, respectively, were identified using the appropriate ICD-9 procedure codes. RESULTS A total of 190,412 patients with extraaxial intracranial hematoma were identified between 1998 and 2009. Within this cohort, 37,434 patients (19.7%) received a craniotomy. A total of 477,110 patients with spinal vertebral fracture were identified. Of these, 37,302 (7.8%) received a spinal decompression and/or fusion. On multivariate analysis controlling for patient demographics, severity of injuries, comorbidities, hospital volume, and hospital characteristics, uninsured patients had a reduced likelihood of receiving a craniotomy (odds ratio [OR] 0.76, 95% confidence interval [CI] 0.71–0.82) and spinal fusion (OR 0.67, 95% CI 0.64–0.71) relative to insured patients. This statistically significant trend persisted when uninsured and insured patients were matched on the basis of mortality propensity score. Uninsured patients demonstrated an elevated risk-adjusted mortality rate relative to insured patients in cases of extraaxial intracranial hematoma. Among patients with spinal injury, mortality rates were similar between patients with and without insurance. CONCLUSIONS In this study, uninsured patients were consistently less likely to receive a craniotomy or spinal fusion for traumatic intracranial extraaxial hemorrhage and spinal vertebral fracture, respectively. This difference persisted after accounting for overall injury severity and patient access to high- or low-volume treatment centers, and potentially reflects a resource allocation bias against uninsured patients within the hospital setting. This information adds to the growing literature detailing the benefits of health reform initiatives seeking to expand access for the uninsured.
