In Reply.— The letter-to-the-editor from Dr Frank is a cautionary note about the risks of glucocorticoid therapy for chronic lung disease in infants. These concerns are real and have been enumerated in most papers on the subject. Only one study to date has suggested a long-term benefit (decreased mortality); yet consistently respiratory therapy support is decreased by steroid use. I have performed a limited meta-analysis because all studies have had similar enrollment criteria and treatment regimens. Two lengths of treatment have been compared and the combined data compared with the controls (Table 1). [See table in the PDF file] This analysis suggests that there is no major difference in outcome by length of treatment, although short treatment was associated with less hyperglycemia and longer duration of intubation. Comparison of all steroid-treated patients with placebo-treated babies showed treated infants had more hyperglycemia (P < .01 χ2) but an average of 11 fewer days of mechanical ventilation (intermittent mandatory ventilation, ImV). (t test was not done due to lack of raw data; however, difference was significant in each subgroup.)

Commercial [methyl-3H]thymidine preparations tested here contain about a 0.2% contaminant which is rapidly incorporated into Chlamydomonas DNA. This contaminant obscures the measurement of the rate of chloroplast DNA synthesis when methyl-labeled preparations are used. Such contaminants are not present in ring-labeled (either 3H or 14C) thymidine preparations. In ring-labeled thymidine preparations, a slower incorporation rate commensurate with cell density is observed. These slower, long-term incorporation kinetics would be expected for the utilization of bona fide thymidine into chloroplast DNA.

AbstractExperiments to explain the rise in the floor of a hospital built on calcareous black pyritic shales have substantiated the reported chemical changes which bring about the formation of selenite (gypsum) along bedding planes. The experiments have also indicated that the sulphate content of a weak rock is not constant and depends on the weathering state. If the long-term importance of sulphates is to be determined there is strong evidence that it is the maximum potential S03 or total sulphur that should be assessed as this would be more beneficial than simply acquiring the S03% as is currently done in most site investigations.Although BS5930 recommends that sulphate and pH testing is carried out as soon as possible after the sample has been taken, it does not give any cautionary note to indicate that the soil chemistry is likely to change with exposure. Experiments are described to show the effect of this change with time.

A brief outline is given of the various factors which need to be considered when contemplating the use of computers in a hospital environment in terms of hardware, software, staff and financial implications. However advanced the technology, there is no escape from the requirement for detailed planning, both short and long term.

AbstractThis article analyses the historical roots and cultural characteristics of the Chechens and Albanians, and how they relate, in their recurrent conflicts with the Russians and Serbs, to long-term grievances, the second variable of my Brutalisation theory. In this article I also explain why the theory departs from the grievance rather than greed premise. Indeed, most of the Chechen and Albanian grievances appear to be based on real and correctly perceived, i.e. absolute deprivations. More fundamentally, given my post -constructivist proposition on the "acting-out" of norms, values and beliefs irrespective of factual or invented origins, I seek to show that martialism, (Sufiinspired) resistance, and (male) egalitarianism have helped to shape and sustain historic grievances, Islamisation (particularly among Chechens), nationalist aspirations and traditional violence-values in Chechen and Albanian societies. Finally, I describe how pre-1979 trauma's and devastations destroyed most records, buildings and symbols of their cultures and histories, as a cautionary note to my efforts to trace both the factual and mythologised foundations of their identities.
