OBJECTIVE: The proportion of ill-defined causes of death (IDCD) was persistently high in some regions of Brazil in 2004. In 2005, the Brazilian government implemented a project in order to decrease this proportion, especially in higher priority states and municipalities. This study aimed to evaluate the performance of this project in Alagoas - a state from the Northeast region of Brazil. METHOD: We selected a probabilistic sample of 18 municipalities. For all IDCD identified in 2010, we collected the verbal autopsy (VA) questionnaires used for home investigation, and the Ministry of Health (MoH) form, which contains information about the final disease and cause of death taken from hospital records, autopsies, family health teams, and civil registry office records. The completion rate of the MoH form and VA was calculated using the number of deaths with specific causes assigned among investigated deaths. RESULTS: A total of 681 IDCD were recorded in 2010 in the sample, of which 26% had a MoH and/or VA3 forms completed. Although the majority of cases were attended by health professionals during the terminal disease, the completion rate was 45% using the MoH form and 80% when VA was performed. CONCLUSIONS: Our findings provide evidence that the training of the epidemiological surveillance teams in the investigation and certification of causes of death could contribute to improve the quality of mortality data.

Abstract Background The proportion of hospital deaths has declined in the past few decades, while the proportions of nursing home deaths have increased. This trend of increasing deaths in long-term care facilities underlines the importance of improving end-of-life care provisions in these settings to meet individual preferences and needs. Under these circumstances, a comprehensive understanding of end-of-life care preferences in local nursing home residents can help healthcare professionals and policymakers develop strategies to increase the advance directive completion rate and quality of care. This study aimed to explore and compare advance directive and end-of-life care preferences of nursing home residents in Hong Kong and Taiwan. Methods A structured questionnaire was developed by the research team to investigate advance directive and end-of-life care preferences in older Chinese nursing home residents. Nursing home residents with frail or pre-frail status and over the age of 64 were invited to participate in the study, and information on demographics, functional status, advance directive experiences, and end-of-life care expectations was collected through questionnaire interviews. Results A total of 325 eligible participants from 32 facilities completed the survey, including 238 older residents in Hong Kong and 87 in Taiwan. A significantly lower proportion of the Hong Kong residents had completed an advance directive compared with the Taiwanese (3 vs. 13%, p = 0.001). Among participants who did not have an advance directive, 46% of the Taiwanese participants said they would consider completing one in the future, compared with 20% of the Hong Kong participants (p < 0.001). A total of 79% of the Hong Kong participants and 80% of the Taiwanese participants responded that prolonging life in the given hypothetical dying scenario was “not important” (p = 0.76). Only 14% of participants in Hong Kong and 18% of participants in Taiwan reported prior occurrence of end-of-life care discussions with family members or health professionals (p = 0.37). Conclusions This paper adds evidence in support of improving end-of-life communication and the advance directive completion rate in nursing homes in Hong Kong and Taiwan. Further research is necessary to explore cross-cultural differences in end-of-life preferences and its applications in predicting decision-making and the quality of end-of-life care.

<p class="abstract"><strong>Background:</strong> Knee osteoarthritis (OA) is a most common rheumatological disorder that causes functional limitation and disability. The most common problem in knee OA are joint pain and stiffness. It will lead to decreased quality of life and it have a serious economic burden on any country due to effect of disability and treatment.</p><p class="abstract"><strong>Methods:</strong> A correlational study was conducted to find out correlation between Ahlback grading and knee society score (KSS) on a sample of 100 moderate to severe knee OA patients and 142 OA knees. Data was collected at orthopedics OPD, for a period of 3 months by purposive sampling.</p><p class="abstract"><strong>Results:</strong> On evaluation, mean age of the participants was 60.19±1.01. Out of 100 patients, 42 patients had bilateral knee OA, therefore total 142 knees included in the analysis. More than half (51%) participants were overweight. Only 34% subjects had compliament to physiotherapy. Around 76% subjects taking analgesics and massage therapy to reduce knee pain. Maximum 82.4% subjects had a poor knee condition in KSS and mean score is 49.07±1.06. Ahlback grading in X-ray had negative correlation -0.610 with KSS. Hence it is evaluated, both the scales have approximately same result as it is analyzed that both scales are moderately correlated. There is significant association of age, occupation and physiotherapy with Ahlback grading followed with KSS significant associated with BMI, occupation and physiotherapy.</p><p class="abstract"><strong>Conclusions:</strong> The study concluded that there is a moderate correlation found between Ahlback X-ray grading and knee society scoring. X-ray and knee society scoring (clinical evaluation) both are essential for effective treatment of OA.</p>

Summary Purpose Purpose of this retrospective study was to evaluate our own results after total knee replacement in patients with haemophilia. Patients, material, method: 30 patients with haemophilia who underwent total knee replacement between 1987 and 2005 were included. We used the clinical and radiological Knee Society Score. Furthermore, the Petterson and the Arnold and Hilgartner score were applied. Results: The mean age at the time of surgery was 43.2 (27–66). At the time of follow- up examination the mean age was 51.6 (30–82) years. The mean follow-up was 7.1 (2–20) years. Preoperative, he mean Arnold and Hilgartner score was 4.17 (± 0.59) and the mean Petterson-Score was 9 ± 2.29. Compared to the preoperative deficiency in knee function (KSS-Score 88.17 ± 33.58) an improvement with 166.67 (± 22.73) points was seen. 1 patient showed an aseptic loosening after 11 years. Discussion: Total knee replacement in patients with haemophilia improves knee function and quality of life. The results of our study represent results in earlier published studies. Compared to a non-haemo-philic normal population the rate of perioper-ative complications was not increased.

<p class="abstract"><strong>Background:</strong> Osteoarthritis of knee is a chronic degenerative joint disease which leads to pain, swelling, stiffness and decreased quality of life. The purpose of this study was to assess the effect of intra-articular steroid injection for osteoarthritis knee by using knee society score (KSS) and visual analog scale (VAS).</p><p class="abstract"><strong>Methods:</strong> A prospective study of 38 patients (44 knees) diagnosed with osteoarthritis of knee were given 80mg of intra-articular methylprednisolone injection. The outcome was assessed using KSS and VAS before the injection and 3 months post injection.<strong></strong></p><p class="abstract"><strong>Results:</strong> The mean VAS improved 3 months post injection whereas there was no improvement in the mean KSS. The scores of VAS 3 months post injection were statistically significant whereas the KSS scores were statistically insignificant compared with the pre injection values.</p><p class="abstract"><strong>Conclusions:</strong> Intra-articular steroid injection for osteoarthritis of knee produces significant pain relief for most patients even in severe cases but this not translate to better functional outcomes.</p>

AimsPrimary: To improve the quality of medical handover by increasing trust-wide completion rate for agreed quality standards from baseline to 80% by July 2019To standardise the medical handover across all 5 boroughs within the trust by July 2019Secondary: To separately assess the completion rate for new patient checks in relation to baselineBackgroundNot only is there a recognised variation in the medical handover across Pennine care foundation trust's (PCFT) 5 boroughs (Tameside, Rochdale, Bury, Oldham and Stockport), but there also appears to be diminished adherence to quality standards to a varying extent. This was shown to result from a combination of human factors and process errors consequently leading to near-miss clinical incidents. This project was therefore designed to highlight and address these issues in order to promote patient safety.MethodHaving identified the problems and agreed on quality standards, baseline data were gathered through May 2019 with the primary outcome measure being the percentage completion rate for quality standards across all handovers. Changes were made to the handover document in June 2019 followed by introduction of a new handover document and post-intervention data collection through July 2019. Doctors’ satisfaction with the new handover document using a typical 5-level Likert scale was measured using a trust-wide online survey.ResultOur results showed a considerable Trust-wide improvement in the completion rate for agreed quality standards from 54% at pre-intervention to 77% at post-intervention with all 5 boroughs demonstrating improvement to varying extents. This is noted to be reasonably close to the project aim of improving the completion rate from baseline to 80%. With regards to new patient checks, there was a marginal Trust-wide increase in completion rate from 53% at pre-intervention to 56% at post-intervention. Additionally, the trust-wide online survey measuring doctors’ satisfaction with the new handover revealed a majority (82%), either strongly agreed (27.3%), or agreed (54.6%), that the new handover document has improved the overall quality of handovers.ConclusionTrust-wide standardisation of the handover has now presented the opportunity to benchmark boroughs within PCFT against one another in terms of adherence to quality standards and alignment with best practice relating to patient safety. While substantial improvement may have been made in the overall quality of handovers across the trust, the inter-borough disparity in the extent of improvement identified gaps for future follow-up or related projects.

Abstract Background: Low knee awareness after minimally invasive total knee arthroplasty (TKA) has become the ultimate target of a natural-feeling knee that meets patient expectations. The objective of this research was to compare the clinical outcomes of TKA via the mini-midvastus (MMV) approach or the medial parapatellar (MPP) approach, and to evaluate which approach can lead to a better quality of life after surgery.Methods: From January 2015 to December 2016, a retrospective cohort study was conducted in 330 patients who underwent TKA via a mini-midvastus (MMV) approach. During this period, we also selected 330 patients who underwent TKA via a medial parapatellar (MPP) approach (MPP group) for comparison. Clinical results were assessed with the visual analog scale score for pain, range of motion, and the Knee Society Score. The Forgotten Joint Score was used to analyze the ability to forget the joint. Results: There were significant differences with regard to visual analog scale score, range of motion, and the Knee Society Score until 6 months after surgery between the MMV and MPP groups (p < 0.05), but the differences were not significant at 12 months, 24 months, and 36 months after surgery. However, there were significant differences in the Forgotten Joint Score between the groups during the follow-up period (p < 0.05).Conclusion: When forgetting the artificial joint after TKA is the ultimate target, better quality of life can be acquired by performing TKA via the MMV approach. In addition, compared with the MPP approach, the MMV approach can offer less pain and a faster recovery.

Twelve years after the advent of MOOCs, the University of the Aegean (Greece) implemented its first MOOC on “Violence and bullying in schools”, in which about 2,000 people showed interest in attending. Eventually, 1309 people started it and 1050 (80.21%) completed it successfully, achieving high performance. The present work, which is part of the doctoral research of the first researcher, outlines the participation of the learners in the program and the obstacles they encountered during it while identifying the reasons for its high completion rate with high performance. The results showed that mainly the quality of the instructional material, the instructional design of the program, and its organization, as well as the timely support provided to learners, contributed significantly to the successful completion of the program achieving high performance. These findings can be considered by future MOOC program designers, in order to design and implement programs that meet the requirements and facilitate the participation of those who attend. Δώδεκα χρόνια μετά την εμφάνιση των MOOCs, το Πανεπιστήμιο Αιγαίου υλοποίησε το πρώτο του MOOC με θέμα την Ενδοσχολική βία και τον εκφοβισμό, στο οποίο εκδήλωσαν ενδιαφέρον για να το παρακολουθήσουν περίπου 2000 άτομα. Τελικά, το ξεκίνησαν 1309 άτομα και το ολοκλήρωσαν επιτυχώς 1050 (80,21%), πετυχαίνοντας υψηλές επιδόσεις. Η παρούσα εργασία, που αποτελεί τμήμα της διδακτορικής έρευνας του πρώτου ερευνητή, σκιαγραφεί τη συμμετοχή των εκπαιδευομένων στο πρόγραμμα και τα εμπόδια που αντιμετώπισαν κατά τη διάρκειά του, ενώ εντοπίζει τους λόγους του υψηλού ποσοστού ολοκλήρωσης του με υψηλές επιδόσεις. Τα αποτελέσματα έδειξαν ότι κυρίως η ποιότητα του εκπαιδευτικού υλικού, ο εκπαιδευτικός σχεδιασμός του προγράμματος και η οργάνωσή του, καθώς και η έγκαιρη υποστήριξη που παρεχόταν στους εκπαιδευόμενους, συνέβαλαν σημαντικά στην επίτευξη των συγκεκριμένων αποτελεσμάτων. Τα ευρήματα αυτά, μπορούν να ληφθούν υπόψη από τους σχεδιαστές μελλοντικών προγραμμάτων MOOCs, ώστε να σχεδιάζουν και να υλοποιούν προγράμματα που θα ικανοποιούν τις απαιτήσεις και θα διευκολύνουν τη συμμετοχή, όσων τα παρακολουθούν. <p> </p><p><strong> Article visualizations:</strong></p><p><img src="/-counters-/edu_01/0727/a.php" alt="Hit counter" /></p>
