Successful endodontic treatment depends upon the clinician’s precise knowledge and ability to manage complex internal anatomy in order to avoid failure during endodontic treatment. Mandibular canines normally have single root with centrally placed single canal and internal anatomy as simple as could be expected, but there are such canines with a single root with two canals, two roots or fused roots. The occurrence of two canal joining to one for a men may be a rare entity with 4-15% prevalence. Canine is the cornerstone of mouth due to its position and very important as abutment for any type of restoration, so long term success of the restorative depend directly on the quality of endodontic treatment. This article presents a case series of successful endodontic management of mandibular canine with two canals which merge into one canal with one foramina and mandibular canine with two roots and two canals.

ABSTRACT Background Parathyroid carcinoma is a rare entity that has diverse presentation and diverse treatments. This case-series highlights the atypical and diverse presentation of parathyroid carcinoma and stresses the importance of the diagnostic approach and intraoperative strategy. Methods We report three patients with parathyroid cancer. Results In one patient, a parathyroid lesion was found to be a parathyroid carcinoma intraoperatively, resulting in a change of operative strategy. In another, parathyroid carcinoma was suspected preoperatively and was treated accordingly. A third patient presented with an already a metastasized parathyroid carcinoma. Conclusion The diagnostic approach and intraoperative strategy are instrumental to determining the best treatment and yielding the best outcome.

Tubulointerstitial nephritis and uveitis syndrome (TINU) is a rare oculorenal inflammatory entity with a probable autoimmune etiology. Interstitial nephritis may be asymptomatic and usually has a benign course with spontaneous resolution. Uveitis, instead, is classically anterior, bilateral, and nongranulomatous, but it can be unilateral and presents as posterior uveitis or panuveitis, sometimes with a chronic or recurrent evolution. The frequent time lag of ocular and renal manifestations makes this diagnosis particularly challenging. The authors describe four cases of this rare entity, two with tubulointerstitial nephritis preceding ocular manifestations and the remaining, instead, with uveitis preceding renal involvement. The therapeutic approach included systemic corticosteroids in all cases. The addition of immunosuppressive therapy was required in three patients to achieve uveitis control. TINU is probably an underrecognized entity and should always be considered in the differential diagnosis of a chronic or recurrent idiopathic uveitis, especially in young patients who may have mild and asymptomatic renal disease.

Peripartum Cardiomyopathy, a type of dilated cardiomyopathy, is a rare entity with increasing trend. The aetiology and pathogenesis of peripartum cardiomyopathy are still unknown. Although the mortality and morbidity rates are high, recognising this condition earlier and treating it with multidisciplinary approach has brought out a better outcome. We, hereby are reporting a case series of eight cases of peripartum cardiomyopathy reported in our hospital and their clinical presentation, echocardiography findings and their subsequent follow up.

Sarcoidosis is multisystem disease that has been amusing physicians since its discovery in 1989 by Caesar Boeck. More than 10,000 articles have been published in the literature by far, and each time it affects a new organ. Involvement of bone has been recently discovered and because it is still a rare entity, it is important to focus on the presenting systems and also to be able to differentiate it from other closely related diseases including but not limited to tumor and other infectious processes. We describe two cases of osseous sarcoma in two relatively young but surprisingly asymptomatic patients.

Objective Nontraumatic acute cervical disk herniation resulting in acute severe neurologic deficit is a rare entity described in a limited number of case reports. We describe the management and outcome in patients presenting with severe neurologic deterioration caused by acutely herniated cervical disks. Methods Four patients (mean age 39.5 years) presented to our tertiary care academic medical center from September 2012 to September 2013 with severe progressive neurologic deficits due to cervical disk herniation and were included in the series. Patients' surgical, medical, and imaging records were retrospectively reviewed under an Institutional Review Board waiver of informed consent. Results Patients in the series presented with acute neurologic deterioration, including paraparesis, Brown-Séquard syndrome, or quadriparesis deteriorating to quadriplegia. Emergent magnetic resonance imaging (MRI) scans and emergent decompression and fusion for acute soft disk herniation were performed in all cases. All patients recovered to excellent functional status with Frankel score improvement from B (one patient)/C (three patients) to E (three patients)/D (one patient). Conclusions Acute cervical disk herniation with acute neurologic deterioration is a medical emergency necessitating emergent MRI and surgical decompression. Clinical presentation varies. In patients with rapid-onset neurologic deterioration, a high level of suspicion for this rare entity is indicated.
