Respiratory quotient level variation during storage: Critical period for low oxygen tolerance, metabolism, and quality of ‘Galaxy’ apples

255 Background: While there is increased attention on the importance of quality measurement in oncology, especially with the rise of value-based payment, limited data exist on national averages and practice level variation for proposed quality measures to establish benchmarks and targets for quality improvement initiatives or value-based contracts. Methods: UnitedHealthcare (UHC) developed peer comparison reports for eight cross cutting quality measures for practices with an active contract for at least one of its commercial, Medicare or Medicaid health plans and ≥1 provider from the following specialties: gynecologic oncology, hematology/oncology, pediatric hematology/oncology, radiation oncology, or surgical oncology. Adherence to the quality measures below was calculated using a mix of claims data, clinical data from a prior authorization for cancer therapy, and CMS MIPS data. Patients were attributed through an algorithm that selected the most probable physician responsible for the patient’s care - responsible prior authorization provider, servicing provider or most recent visited provider prior to the treatment, varying by each measure. Dates of service differ by measure, ranging from 1/1/2019 through 12/31/2020. Results: We identified 5,828 unique tax identification numbers (TINs) with UHC members with cancer attributed to them during 2019-20. The number of practices included in the measurement cohort per measure varied significantly from 301 to 4,120 (tobacco screening and performance status, respectively). 2,422 TINs met the minimum patient count for at least one measure (≥10 patients or events). Overall performance ranged from 13.5% to 77.3% (hospice admission and PS documented) for measures where higher adherence reflects better quality of care. For measures where lower scores represent higher quality of care the range was 11.4% to 22.6% (hospice < 3 days and ED admission, respectively). Observed adherence was statistically better than expected for 0.5%-5.8% and statistically less than expected for 0.9%-5.6% of TINs in UHC’s network; however, more than half of the practices had insufficient sample size to make a determination. Conclusions: We observed substantial variation in quality across a national cohort of oncology practices. However, even for a large national payer, small sample sizes limited the assessment of a substantial number of practices.[Table: see text]

Wetlands provide several ecosystem services including carbon capture and storage, water filtration, nutrient cycling, and support agriculture among others. The biogeochemical process and decomposition parameters in ‘Fadama' wetland soils comprising of Gada biyu, Pwomol and Kpang referred to as sites A, B, and C respectively all of Heipang District in Barkin Ladi, Plateau State was investigated using standard operating procedures (SOP). Results of investigations revealed that soils from Kpang had slightly higher water content (34.52%) than those from Pwomol (33.48%) and Gada biyu (32.03%). While soils from Gada biyu had the highest solid organic matter (SOM) (10.79%) followed by Pwomol (8.15%) as Kpang had the least (7.85%). Gada biyu soils had the lowest Phenol oxidases activity (1536.56 nmol dicq g-1 h-1) while those from Pwomol (5340.44 nmol dicq g-1 h-1) was highest. All sites had similar concentrations of soil phenolics (76.58 µg g-1, 79.98µg g-1, and 83.25µg g-1). The activity of hydrolyses (β-glucosidase) in Gada biyu soil (2.93 nmol g-1 min-1) was lower than those from Pwomol (6.13 nmol g-1 min-1). These parameters indicate the level of biogeochemical processes in the soil at each site. Gada biyu had the highest rate of CH4 (0.84 ug g-1h-1) flux. Decomposition of organic matter, carbon cycling and greenhouse gas storage in wetland soil, is due to the anoxic condition comprising of low oxygen availability, cool temperatures, anaerobic conditions, reduced microbial activity, and the quality of organic matter substrates in such soils.  Anthropogenic disturbances affecting wetlands must be discouraged to promote vital ecosystem services.

1-Methylcyclopropene (1-MCP) is applied as an inhibitor of ethylene action, which is widely used in postharvest technology to prolong the shelf life of many fruits. The aim of the study was to assess the possibility to apply 1-MCP treatment to maintain the quality of ‘Idared’ apples for long-distance transportation. The studied apples were assessed in three groups: (I) 1-MCP postharvest treatment; (II) 1-MCP postharvest treatment with Modified Atmosphere Packaging (MAP) selected gas permeability bags; and (III) control groups (with neither 1-MCP treatment, nor dedicated packaging). Apples were subjected to storage in the Ultra Low Oxygen (ULO) chamber that was applied for 0 weeks, 10 weeks and 20 weeks (three periods of storage); simulated long-distance transport (6 weeks); and simulated distribution, which was applied for 0 days, 5 days, 10 days, and 15 days (4 periods of distribution). The obtained 36 groups (three postharvest treatments per three periods of storage per four periods of distribution) were analyzed to assess firmness, total soluble solids (TSS) and titratable acidity (TA). There were differences between firmness values for control groups and those with 1-MCP applied, which were characterized by higher values of firmness (p < 0.05). Groups with 1-MCP and MAP applied combined were characterized by higher values of TSS than control groups (p < 0.05). The majority of groups with 1-MCP applied alone were characterized by higher values of TA than control groups (p < 0.05), but values for samples attributed to 1-MCP and MAP combined were not higher than for 1-MCP alone. It may be concluded that 1-MCP applied postharvest contributed to higher results of firmness and TA of ‘Idared’ apples after long-distance transportation, but combining 1-MCP with MAP did not contribute to further differences for TA. However, for TSS the observed influence was inconclusive. It may be stated that 1-MCP is a beneficial treatment for ‘Idared’ apples for long-distance transportation as it prolongs their shelf life and improves firmness.
