Use of recombinant canine granulocyte-colony stimulating factor to increase leukocyte count in dogs naturally infected by canine parvovirus

Abstract A 78-year-old man was admitted to our hospital with right upper abdominal pain and fever. His general condition was poor. The laboratory data showed severe inflammatory reactions. Computed tomography revealed an irregular tumor in the gallbladder. 18F-fluorodeoxy-glucose positron emission tomography (FDG-PET) showed high uptake by the tumor, with diffuse uptake in the spine. Based on the elevated leukocyte count and FDG-PET findings, a granulocyte-colony stimulating factor (G-CSF)–producing tumor was diagnosed (G-CSF 120 pg/mL). We performed cholecystectomy with central bisegmentectomy of the liver, lymph node dissection and right hemicolectomy. Histologically, the tumor was an adenosquamous cell carcinoma of the gallbladder. Immunohistochemical staining of the tumor cells was positive for G-CSF. Postoperatively, the general condition of the patient was improved. The fever subsided, the leukocyte count and serum G-CSF level normalized, and FDG-PET showed no uptake in the spine postoperatively. The patient showed no signs of recurrence at 27 months after undergoing surgery. FDG-PET is a useful method for diagnosing G-CSF–producing gallbladder carcinoma. Aggressive curative resection for G-CSF–producing gallbladder carcinoma may improve patients' general condition and prognosis.

Three children with acquired immunodeficiency syndrome (AIDS) and chronic anaemia and leucopenia were treated with 5 μg/kg recombinant granulocyte colony-stimulating factor subcutaneously three times a week and 50 IU/kg erythropoietin subcutaneously twice a week. The therapy was not interrupted during the follow-up period. All children showed an increase of leukocyte count and haemoglobin levels. No transfusion was necessary and the number of admissions into hospital fell. These results suggest that combined therapy with granulocyte colony-stimulating factor and erythropoietin may improve leukopenia and anaemia, which is not zidovudine-related, in children who have AIDS.

Canine parvovirus (CPV) adalah salah satu penyakit infeksius pada anjing dengan angka morbiditas dan mortalitas yang tinggi. Infeksi CPV umumnya menyerang organ sistem pencernaan sehingga mengakibatkan gejala klinis berupa muntah dan diare. Organ lain yang diserang oleh virus ini antara lain sumsum tulang, sistem limfatik, dan myocardium (terutama pada hewan muda). Salah satu gejala klinis yang kerap muncul adalah leukopenia akibat perusakan prekursor leukosit dan sel-sel limfoid oleh virus. Filgastrim (Leucogen®) adalah rekombinan Human Granulocyte – Colony Stimulating Factor (rH G-CSF) yang berfungsi menstimulasi proliferasi dan diferensiasi sel-sel granulosit. Tiga ekor anjing ras Cocker Spaniel (a) (2 tahun), Bernese Mountain (b) (6 bulan), dan mixed breed (c) (3 bulan) dibawa ke PDHB 24 Jam drh. Cucu K. Sajuthi, dkk. dengan kondisi klinis anorexia, lethargy, vomit, melena, dan dehidrasi. Uji rapid test kit CPV menunjukan interpretasi positif. Pemeriksaan darah lengkap menunjukan nilai WBC 2 x 103/µL (a); 3,5 x 103/µL (b); dan 3,6 x 103/µL (c), serta nilai absolut granulosit 1,1 x 103/µL (a); 2,1 x 103/µL (b); dan 2,6 x 103/µL (c). Ketiga pasien diberikan terapi filgastrim (10 µg/kg/hari) selama tiga hari berturut-turut bersama dengan terapi suportif lainnya. Darah lengkap kembali dilakukan pemeriksaan dan menunjukan nilai WBC 5,8 x 103/µL (a); 9,7 x 103/µL (b); dan 51,4 x 103/µL (c) serta nilai absolut granulosit 3,4 x 103/µL (a); 7,3 x 103/µL (b); dan 42,5 x 103/µL (c). Terapi menggunakan filgastrim pada ketiga pasien menunjukkan peningkatan kondisi yang nyata terhadap gejala leukopenia. Pasien juga menunjukan perbaikan kondisi klinis yang signifikan membaik.

Computed tomography revealed an irregular tumor in the left hepatic lobe and swelling of lymph nodes. 18F-fluorodeoxy-glucose positron emission tomography (FDG-PET) showed high uptake by the tumor, with diffuse uptake in the spine. Based on the elevated leukocyte count and FDG-PET findings, the patient was diagnosed with a granulocyte colony-stimulating factor (G-CSF)-producing tumor (G-CSF, 213 pg/mL). We performed left trisegmentectomy of the liver, bile duct resection, and lymph node dissection. Histologically, the tumor was a poorly differentiated adenocarcinoma with some lymph nodes metastasis. Immunohistochemical staining of the tumor cells was positive for G-CSF. Therefore, the tumor was diagnosed as G-CSF–producing cholangiocellular carcinoma. The inflammatory reactions and serum G-CSF level transiently improved immediately after surgery. However, 1 month later, the leukocyte count and serum G-CSF level increased again, and recurrence was observed in the remnant liver. The patient died 3 months after the operation. G-CSF–producing cholangiocellular carcinoma is rare. This tumor progresses rapidly, and surgical treatment for advanced condition should be carefully selected.
