Primary clear-cell urothelial carcinoma (CCUC) is an uncommon type of urothelial cancer with only 16 cases reported in published literature. Due to the rarity of the tumour, its clinical and prognostic values have not been clearly understood. We present one such rare clinical diagnosis in a 60-year- old man who underwent radical cystectomy (RC) with ileal conduit for urinary bladder cancer. Histopathology showed features of high-grade CCUC infiltrating the muscularis propria. Immunohistochemistry revealed diffuse immunopositivity of pan cytokeratin (CK), GATA3, P40, CK7 but was immunonegative for CD10 and vimentin. Our patient expired 4 months after diagnosis. CCUC has recently been included in the WHO 2016 classification of urothelial tumours. Most of the patients present with poor prognosis. Accurate diagnosis and recognition of this unusual variant are essential for better patient management and prognosis. Early RC seems to be the preferred way of management.

A cross sectional observational study was done to see the staging and grading of urinary bladder tumor along with their histopathological correlation at the Department of Pathology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka. The study was done during the period of July 2006 to June 2007 and total 150 cases were included those were clinically suggestive and cystoscopically or sonographically suspected cases of bladder tumor. Size of bladder, length of ureters, other organs present; tumor characteristics: size (including thickness), location extent of invasion, shape (papillary, ulcerated), multifocal lesion and appearance of non-neoplastic mucosa, thickness of bladder wall away from tumor were observed. Total 145 samples were collected by TURBT and 5 were by cystectomy. Mean age of the study patients was 58.5 years and 95.33% of them were male. Histopathologically 95.33% were papillary urothelial carcinoma, 2.66% were adenocarcinoma, 1.33% was squamous cell carcinoma and 0.66% were carcinosarcoma. Among 150 cases 72 (50.34%) bladder tumour were of high grade and 71 (49.65%) were low grade. In 7 (4.66%) cases grading were not done because they were other than papillary urothelial carcinoma. Staging were done in 93 cases and in remaining 57 cases staging was not possible due to absence of muscularis propria. 32 (21.33%) cases were stage T2, 31 (20.66%) cases T1 and 4 (2.66%) were in stage Ta. Overall findings of the cross sectional Bangladeshi population was almost similar to other developing and developed countries.

Abstract Objectives Bladder cancers invading the muscularis mucosae (MM) are treated differently from those invading the muscularis propria (MP). However, it may be difficult to determine the type of smooth muscle in transurethral resection (TUR) or biopsy specimens. We aimed to investigate the clinicopathologic features of bladder cancers involving smooth muscle of indeterminate type (SMIT) in TUR specimens in comparison with those invading the MM. Methods We identified 103 patients with bladder cancer involving SMIT (n = 27) or the MM (n = 76) in TUR specimens. All patients underwent subsequent restaging TUR or cystectomy. Results Bladder cancer with SMIT invasion showed a significantly higher rate of MP invasion in the subsequent specimens than those invading the MM (52% vs 29%). Lack of MP in the TUR specimens had a significantly higher risk of MP invasion in the subsequent specimens than those with the MP (61% vs 40%). The overall survival time for patients with SMIT invasion was significantly shorter than those with MM invasion. Conclusions Bladder cancers with SMIT invasion in TUR specimens show more frequent cancer upstaging in the subsequent specimens and a poorer clinical outcome than those invading the MM, which highlights the importance of a cancer restaging procedure for these patients.

We report two infants with the acquired immunodeficiency syndrome (AIDS) and rectal bleeding due to cytomegalovirus (CMV) ileitis and colitis with minimal focal mucosal ulceration but with extensive leiomyolysis of the muscularis propria. Immunostaining and in situ hybridization for CMV showed numerous viral inclusions in the myocytes of the muscularis propria and vascular endothelium/smooth muscle with only occasional inclusions present in the muscularis mucosae. Colectomy was curative in one patient; in the other the bowel was only examined at postmortem.
