‘We struggle and muddle.’ A qualitative study exploring community ABI therapists’ experiences of using, training in and implementing behaviour interventions

Abstract Objectives: Individuals with acquired brain injury (ABI) may present with challenging behaviours (CB) that place themselves and others at risk of harm and impact their community integration. It is crucial for community ABI therapists to successfully train in and implement behaviour interventions. The current study aimed to investigate community ABI therapists’ experiences of using, training in and implementing behaviour interventions. An additional aim was to determine these therapists’ understanding of Positive Behaviour Support (PBS), one approach to addressing CB with a focus on improving quality of life. Methods: Semi-structured interviews were conducted with 24 Australian community ABI therapists about their experiences of using, training in and implementing behaviour interventions and understanding of PBS. Inductive thematic analysis and content analysis were performed on interview transcripts. Findings: The thematic analysis resulted in the generation of six themes which described the difficulties participants faced in training in and delivering behaviour interventions and identified their training and implementation needs. The content analysis resulted in 10 categories that characterised participants’ understanding of PBS, which centred around the absence of consequences, a focus on antecedents, person-centred practice and encouraging prosocial alternatives to CB. Conclusions: The findings highlight a need and desire for more practical and interactive clinician training in behaviour interventions for individuals with ABI. Moreover, the findings suggest a limited understanding of PBS amongst community ABI therapists. Important considerations for the development of clinician training in ABI behaviour interventions and subsequent implementation into community practice are discussed.

ObjectivesTo explore the services National Health Service (NHS)-based sport and exercise medicine (SEM) clinics can offer, and the barriers to creating and integrating SEM services into the NHS.MethodsSemi-structured interviews were undertaken to collect data from identified ‘stakeholders’. Stakeholders were identified as individuals who had experience and knowledge of the speciality of SEM and the NHS. An inductive thematic analysis approach was taken to analyse the data.ResultsN=15 stakeholder interviews. The management of musculoskeletal (MSK) injuries (both acute and chronic) and concussion were highlighted as the two key services that SEM clinics can offer that would most benefit the NHS. MSK ultrasound was also mentioned by all stakeholders as a critical service that SEM clinics should provide. While exercise medicine is an integral part of SEM, SEM clinics should perhaps not have a heavy exercise medicine focus. The key barriers to setting up SEM clinics were stated to be convincing NHS management, conflict with other specialities and a lack of awareness of the speciality.ConclusionThe management of acute MSK injuries and concussion should be the cornerstone of SEM services, ideally with the ability to provide MSK ultrasound. Education of others on the speciality of SEM, confirming consistent ‘unique selling points’ of SEM clinics and promoting how SEM can add value to the NHS is vital. If the successful integration of SEM into the NHS is not widely achieved, we risk the NHS not receiving all the benefits that SEM can provide to the healthcare system.

Abstract Background Although telehealth is becoming more popular for delivery of care for individuals with musculoskeletal pain, to our knowledge telehealth has not been used to manage Achilles tendinopathy. This research aimed to explore the experience of participants and physiotherapists with gym-based exercise interventions for Achilles tendinopathy monitored via videoconference. Methods A qualitative, interpretive description design was performed using semi-structured interviews (8 participants) and a focus group (7 physiotherapists). Participants and physiotherapists were interviewed about their experiences of the use of telehealth during a gym-based exercise intervention incorporating different calf load parameters for Achilles tendinopathy. We employed an inductive thematic analysis approach to analyse the data. Results Three themes identified from both participants and physiotherapists included i) acceptability of telehealth; ii) enablers to adherence with telehealth; and iii) barriers to adherence with telehealth. Two extra themes arose from participants regarding adherence with gym-based exercise, including enablers to adherence with the exercise intervention, and barriers to adherence with the exercise intervention. Both participants and physiotherapists expressed overall satisfaction and acceptability of telehealth monitoring of gym-based exercise. Conclusion Gym-based exercise intervention for Achilles tendinopathy involving weekly telehealth monitoring was acceptable to both participants and physiotherapists. Potential enablers and barriers were identified that may improve adherence to this type of intervention.

Abstract Introduction Good quality of care is dependent on nurses’ strong clinical skills and strong moral competencies, as well. However, the moral performance of some nurses in some organizations shows a deterioration in their moral sensitivity and actions. The aim of the study reported on in this paper was to explore the experiences of nurses regarding negative changes in their moral practice. Materials and Methods This was a qualitative study utilizing a inductive thematic analysis approach, which was conducted from February 2017 to September 2019. Twenty-five nurses participated in semi-structured interviews. Results The main theme that emerged from our analysis was one of moral neutralization in the context of an unethical moral climate. We found five sub-themes, including: (1) feeling discouraged; (2) normalization; (3) giving up; (4) becoming a justifier; and (5) moral indifference. Conclusions Unethical moral climates in health organizations can result in deterioration of morality in nurses which can harm both pateints and health syetms. Some unethical behaviors in nurses can be explaiened by this process.

Children with intellectual disability and behavioural needs (challenging behaviour) are vulnerable to exclusion from services and communities. The situation is exacerbated by difficulties in accessing appropriate support and services to effectively meet the needs of children and carers. Family perspectives on the ‘lived experience’ of children can provide insight into how behavioural needs can affect their ability to access everyday experiences. Semi-structured interviews were conducted with mothers of children with intellectual disabilities and challenging behaviours. Phenomenological thematic analysis provided four key themes: finding our way; square services, round needs; behaviour touches everything and belonging. Experience of inclusion and exclusion was a central tenet of the lived experience. Recommendations call for timely proactive and bespoke interventions to identify and support children at risk of exclusion from communities. Early intervention and effective local provision will avoid increased burdens placed on families and services, in supporting children whose needs are currently unmet within child-centred provision.

<p><strong>INTRODUCTION: </strong>High quality educational systems are necessary for sustainable development and responding to the needs of society. In the recent decades, concerns have increased on the quality of education and competency of graduates. Since graduates of medical education are directly involved with the health of society, the quality of this system is of high importance. Investigation in the lived experience of educational leaders in the medical education systems can help to promote its quality. The present research examines this issue in Iran.</p><p><strong>METHODOLOGY:</strong> The study was done using content-analysis qualitative approach and semi-structured interviews. The participants included 26 authorities including university chancellors and vice-chancellors, ministry heads and deputies, deans of medical and basic sciences departments, education expert, graduates, and students of medical fields. Sampling was done using purposive snowball method. Data were analyzed using conventional content analysis.</p><p><strong>FINDINGS:</strong> Five main categories and 14 sub-categories were extracted from data analysis including: quantity-orientation, ambiguity in the trainings, unsuitable educational environment, personalization of the educational management, and ineffective interpersonal relationship. The final theme was identified as “Education in shadow”.</p><p><strong>CONCLUSION:</strong> Personalization and inclusion of personal preferences in management styles, lack of suitable grounds, ambiguity in the structure and process of education has pushed medical education toward shadows and it is not the first priority; this can lead to incompetency of medical science graduates.</p>

Nurse practitioners (NPs) provide medical care equivalent to that of physicians and facilitate access to healthcare. Although Japan’s first NP graduated in 2010, how Japanese expert NPs work effectively in clinical contexts is yet to be investigated. We aimed to identify the competencies that make expert NPs in Japan effective. Twelve Japanese expert NPs were purposely selected. The average age of the participants was 44.8 years, average NP experience was 7.5 years, and eight participants were women. Semi-structured interviews were conducted online from March to May 2021. Interviews were recorded, transcribed verbatim, and analyzed using thematic analysis. Thematic analysis revealed five themes: working in physicians’ contexts, interprofessional collaboration, involvement in nurses’ work, contribution to healthcare, and personal qualities for effective working. Japanese expert NPs can function effectively in clinical settings by flexibly and humbly collaborating with other medical professionals who have autonomous positions. They can improve the quality of healthcare by proposing practical solutions to problems faced by patients and medical organizations. These explored competencies can be applied to other aging and more complex societal contexts, and in updating the required competencies of Japanese NPs.

Este estudo objetivou descrever como os diabéticos acondicionam a insulina em seu domicílio e identificar o responsável pela orientação sobre o acondicionamento da insulina. Trata-se de um estudo exploratório-descritivo, que percorreu a trajetória metodológica qualitativa. A análise dos dados foi realizada por meio da técnica de análise de conteúdo temática. Os sujeitos da pesquisa foram 14 pacientes diabéticos assistidos pela Estratégia Saúde da Família. Os dados foram coletados através de um roteiro de entrevista semiestruturada e para sua análise utilizou-se a técnica de análise de conteúdo de Minayo. Frente aos resultados considera-se que os usuários de insulina entrevistados têm um conhecimento superficial do local adequado para o acondicionamento da insulina e como transportá-lo e tem a segurança que o enfermeiro é responsável pelas orientações do acondicionamento da insulina e nos remete a uma reflexão sobre a qualidade das informações prestadas em visto que muitos têm um baixo nível de escolaridade.Descritores: Enfermagem; Diabetes Mellitus, Insulina. Insulin of packaging at home diabeticAbstract: This study aimed to describe the pack diabetic insulin in your home and identify the person responsible for guidance on the packaging of insulin. This is an exploratory-descriptive study, which covered the qualitative methodological approach. Data analysis was performed using thematic content analysis technique. The study subjects were 14 diabetic patients assisted by the Family Health Strategy. Data were collected through a semi-structured interviews and their analysis was used to Minayo content analysis technique. Living to an old age is considered by many elderly women to bring about a decreased quality of life. Hydrotherapy allows individuals to engage in certain physical activities that cannot be done outside the water. The purpose of this study was to explore elderly women’s experience of the role of hydrotherapy in their health and was conducted in selected pools of Guilan province in Iran in 2015-2016. Twenty-three elderly women were selected using a purposeful sampling strategy. The data was gathered through semi-structured interviews; Researchers used a qualitative design, based on a content analysis approach. Analysis results were identified as 3 main themes and 12 subthemes. Improved physical health: reduced consumption of painkillers, improved balance, pleasant breathing, easy menopause; Moving toward ability: increased independence, role fulfillment, application of efficient strategies, will to health; Psychological and social relief: sense of liveliness, reduced stress, peaceful sleep and re-emergence in society. “Competence development” was identified as the central code. Results indicate that participation in hydrotherapy by elderly women supported the benefit of advising them to participate in hydrotherapy sessions, as well as government’s cooperation with regard to creating proper conditions for improving the health and life quality of this valuable population.

ABSTRACT Objective: to understand the challenges and strategies adopted by nurses for the exercise of Transformational Leadership in a university hospital. Methods: qualitative and exploratory study, in which 25 nurses working in a university hospital in the state of Bahia, Brazil participated. Data collection took place through semi-structured interviews and categorized according to Thematic Analysis, using Nvivo software. Results: The prevalent challenges involved: lack of encouragement from the institution for the training of leaders; professional inexperience and young age; resistance to leadership and insubordination. The strategies adopted by nurses consist of acting as team examples and establishing dialogic relationships. Final considerations: The practice of Transformational Leadership has been relevant in the daily lives of nurses and contributes to the quality of care.

Aim:  To  understand  the  importance  and  the  difficulties  faced  by women  when  adhering  to  exclusive  breastfeeding.  Method:  This  is  an exploratory  and descriptive study, using a qualitative approach, in which semi-structured interviews were conducted with 14 women. Data treatment occurred by means of the content analysis, in the form of a thematic analysis. Results: Most women revealed knowing the importance and  benefits  of  breastfeeding  for  being  an  important  practice,  since  it  provides  good nourishment   and   aids   the   development   of   the   child.   The   aspects   that   benefit breastfeeding  women  were  made  explicit;  however,  these  were  given  less  focus  when compared to the aspects related to the baby's health. Conclusion: We should point out the  need  to  incorporate  practices  beyond  the  biological  dimension,  to  understand  the characteristics  of  each  woman  and  to  encourage  self-care  as  an  important  issue  for them.
