ABSTRACTPhysical inactivity in older adults presents a significant problem within modern societies globally. Using a mixed-method approach, this study explored strategies for the development and delivery of physical activity (PA) interventions by investigating what behaviour change techniques (BCTs) are useful, and how these techniques should be implemented to be feasible for older adults. Sixty-six older adults completed a survey indicating the most useful BCTs, mapping on to motivational, volitional and automatic factors. Of these, 48 older adults participated in an interview exploring strategies for a PA intervention targeted at older adults. The most useful BCT identified in the survey was autonomy support (61.3%), followed by instruction to perform the behaviour (43.5%) and having a credible source of information about PA (42.6%). The key themes discussed in the interviews included providing support in making an informed choice, instruction on how to perform PA, information about health consequences, social support, goal setting, action and coping plans, behavioural demonstration and practice, and monitoring PA. The interviews also revealed key aspects of programme implementation including face-to-face delivery, followed up with additional materials; low cost; age-appropriate PA level; and individualised approach. Interventions assisting older adults in increasing their PA participation across a range of settings should incorporate BCTs targeting multiple processes, while tailoring their delivery to older adults’ preferences to ensure their feasibility in supporting regular PA engagement.

Optimal diet quality and physical activity levels are essential for healthy ageing. This study evaluated the effects of a multi-component telemonitoring intervention on behavioural determinants of diet quality and physical activity in older adults, and assessed the mediating role of these determinants and two behaviour change techniques in the intervention’s effects. A non-randomised controlled design was used including 214 participants (average age 80 years) who were allocated to the intervention or control group based on municipality. The six-month intervention consisted of self-measurements of nutritional outcomes and physical activity, education, and follow-up by a nurse. The control group received regular care. Measurements took place at baseline, after 4.5 months and at the end of the study. The intervention increased self-monitoring and improved knowledge and perceived behavioural control for physical activity. Increased self-monitoring mediated the intervention’s effect on diet quality, fruit intake, and saturated fatty acids intake. Improved knowledge mediated the effect on protein intake. Concluding, this intervention led to improvements in behavioural determinants of diet quality and physical activity. The role of the hypothesised mediators was limited. Insight into these mechanisms of impact provides directions for future development of nutritional eHealth interventions for older adults, in which self-monitoring may be a promising behaviour change technique. More research is necessary into how behaviour change is established in telemonitoring interventions for older adults.

BACKGROUND Just-in-time-adaptive-interventions (JITAIs) provide real-time ‘in the moment’ behaviour change support to people when they need it most. JITAIs could be a viable way to provide personalised physical activity support to older adults in the community. However, it is unclear how feasible it is to remotely deliver a physical activity intervention via a smartphone to older adults, or how acceptable older adults would find a JITAI targeting physical activity in everyday life. OBJECTIVE (1) to describe the development of “JITABug”, a personalised smartphone and activity tracker delivered JITAI designed to support older adults to increase or maintain their physical activity level; (2) to explore the acceptability of JITABug in a free-living setting, and (3) to assess the feasibility of conducting an effectiveness trial of the JITABug intervention. METHODS The intervention development process was underpinned by the Behaviour Change Wheel. The intervention consisted of a wearable activity tracker (Fitbit) and a companion smartphone app (JITABug) which delivered goal setting, planning, reminders, and just-in-time adaptive messages to encourage achievement of personalised physical activity goals. Message delivery was tailored based on time of day, real-time physical activity tracker data, and weather conditions. We tested the feasibility of remotely delivering the JITAI with older adults in a 6-week trial using a mixed-methods approach. Data collection involved assessment of physical activity by accelerometery and activity tracker, self-reported mood and mental wellbeing via ecological momentary assessment, and contextual information on physical activity via voice memos. Feasibility and acceptability outcomes included: (1) recruitment capability and adherence to the intervention; (2) intervention delivery ‘in the wild’; (3) appropriateness of data collection methodology; (4) adverse events and; (5) participant satisfaction. RESULTS Of 46 recruited older adults (aged 56-72 years old), 65% completed the intervention. The intervention was successfully delivered as intended; 27 participants completed the intervention independently, 94% of physical activity messages were successfully delivered, and 99% of Fitbit and 100% of weather data calls were successful. Wrist-worn accelerometer data were obtained from 96% at baseline and 96% at follow up. On average, participants recorded 8/16 (50%) voice memos, 3/8 (38%) mood assessments, and 2/4 (50%) wellbeing assessments via the app. Overall acceptability of the intervention was very good (77% satisfaction). Participant feedback suggested that more diverse and tailored physical activity messages, app usage reminders, technical refinements regarding real-time data syncing, and an improved user interface could improve the intervention and make it more appealing. CONCLUSIONS This study suggests that a smartphone delivered JITAI utilizing a wearable activity tracker is an acceptable way to support physical activity in older adults in the community. Overall, the intervention is feasible, however based on user feedback, the JITABug app requires further technical refinements that may enhance usage, engagement, and user satisfaction before moving to effectiveness trials. Abstract Background Physical Activity Referral Schemes (PARS), including exercise referral schemes, are a popular approach to health improvement, but understanding of effectiveness is limited by considerable heterogeneity in reporting and evaluation. We aimed to gain consensus for a PARS taxonomy as a comprehensive method for reporting and recording of such schemes. Methods We invited 62 experts from PARS policy, research and practice to complete a modified Delphi study. In round one, participants rated the need for a PARS taxonomy, the suitability of three proposed classification levels and commented on proposed elements. In round two, participants rated proposed taxonomy elements on an 11-point Likert scale. Elements scoring a median of ≥7, indicating high agreement, were included in the final taxonomy. Results Of those invited, 47 (75.8%) participated in round one, with high retention in round two (n = 43; 91.5%). 42 were UK-based, meaning the resultant taxonomy has been scrutinised for fit to the UK context only. The study gained consensus for a three-level taxonomy: Level 1: PARS classification (primary classification, provider, setting, conditions accepted [have or at risk of], activity type and funding). Level 2: scheme characteristics (staff structure, staff qualifications, behaviour change theories, behaviour change techniques, referral source, referrers, referral process, scheme duration, session frequency, session length, session times, session type, exit routes, action in case of non-attendance, baseline assessment, exit assessment, feedback to referrer and exclusion criteria) and Level 3: participant measures (demographics, monitoring and evaluation, and measures of change). Conclusion Using a modified Delphi method, this study developed UK-based consensus on a PARS classification taxonomy. We encourage PARS practitioners and public health colleagues, especially those working with similar service models internationally, to test, refine and use this taxonomy to inform policy and practice.

When people have more negative perceptions about aging or attribute health decline to old age, they engage in less health promotion behavior. We tested whether an intervention of brief anti-ageism messages addressing views of aging could motivate engagement in physical activities at senior centers. Attendees aged 50 and older ( n = 349; Mage = 72, SD = 9) at seven centers were randomly assigned to read one of three intervention messages (different approaches addressing views of aging) or to not read a message before rating their likelihood of attending a variety of center programs, including physical activities. Multilevel regression models indicated the intervention increased motivation to attend physical activities compared with the control group among participants aged 72 and older. The three anti-ageism messages were similarly effective suggesting some flexibility in framing. The results indicate anti-ageism messages may be a scalable, low-cost approach to promoting physical activity in older adults.
