Purpose In individuals with cochlear implants, speech recognition is not associated with tests of working memory that primarily reflect storage, such as forward digit span. In contrast, our previous work found that vocoded speech recognition in individuals with normal hearing was correlated with performance on a forward digit span task. A possible explanation for this difference across groups is that variability in auditory resolution across individuals with cochlear implants could conceal the true relationship between speech and memory tasks. Here, our goal was to determine if performance on forward digit span and speech recognition tasks are correlated in individuals with cochlear implants after controlling for individual differences in auditory resolution. Method We measured sentence recognition ability in 20 individuals with cochlear implants with Perceptually Robust English Sentence Test Open-set sentences. Spectral and temporal modulation detection tasks were used to assess individual differences in auditory resolution, auditory forward digit span was used to assess working memory storage, and self-reported word familiarity was used to assess vocabulary. Results Individual differences in speech recognition were predicted by spectral and temporal resolution. A correlation was found between forward digit span and speech recognition, but this correlation was not significant after controlling for spectral and temporal resolution. No relationship was found between word familiarity and speech recognition. Forward digit span performance was not associated with individual differences in auditory resolution. Conclusions Our findings support the idea that sentence recognition in individuals with cochlear implants is primarily limited by individual differences in working memory processing, not storage. Studies examining the relationship between speech and memory should control for individual differences in auditory resolution.

Purpose Models of speech recognition suggest that “top-down” linguistic and cognitive functions, such as use of phonotactic constraints and working memory, facilitate recognition under conditions of degradation, such as in noise. The question addressed in this study was what happens to these functions when a listener who has experienced years of hearing loss obtains a cochlear implant. Method Thirty adults with cochlear implants and 30 age-matched controls with age-normal hearing underwent testing of verbal working memory using digit span and serial recall of words. Phonological capacities were assessed using a lexical decision task and nonword repetition. Recognition of words in sentences in speech-shaped noise was measured. Results Implant users had only slightly poorer working memory accuracy than did controls and only on serial recall of words; however, phonological sensitivity was highly impaired. Working memory did not facilitate speech recognition in noise for either group. Phonological sensitivity predicted sentence recognition for implant users but not for listeners with normal hearing. Conclusion Clinical speech recognition outcomes for adult implant users relate to the ability of these users to process phonological information. Results suggest that phonological capacities may serve as potential clinical targets through rehabilitative training. Such novel interventions may be particularly helpful for older adult implant users.

Neural measures of working memory storage, such as the contralateral delay activity (CDA), are powerful tools in working memory research. CDA amplitude is sensitive to working memory load, reaches an asymptote at known behavioral limits, and predicts individual differences in capacity. An open question, however, is whether neural measures of load also track trial-by-trial fluctuations in performance. Here, we used a whole-report working memory task to test the relationship between CDA amplitude and working memory performance. If working memory failures are due to decision-based errors and retrieval failures, CDA amplitude would not differentiate good and poor performance trials when load is held constant. If failures arise during storage, then CDA amplitude should track both working memory load and trial-by-trial performance. As expected, CDA amplitude tracked load (Experiment 1), reaching an asymptote at three items. In Experiment 2, we tracked fluctuations in trial-by-trial performance. CDA amplitude was larger (more negative) for high-performance trials compared with low-performance trials, suggesting that fluctuations in performance were related to the successful storage of items. During working memory failures, participants oriented their attention to the correct side of the screen (lateralized P1) and maintained covert attention to the correct side during the delay period (lateralized alpha power suppression). Despite the preservation of attentional orienting, we found impairments consistent with an executive attention theory of individual differences in working memory capacity; fluctuations in executive control (indexed by pretrial frontal theta power) may be to blame for storage failures.

Purpose Music training has been proposed as a possible tool for auditory training in older adults, as it may improve both auditory and cognitive skills. However, the evidence to support such benefits is mixed. The goal of this study was to determine the differential effects of lifelong musical training and working memory on speech recognition in noise, in older adults. Method A total of 31 musicians and nonmusicians aged 65–78 years took part in this cross-sectional study. Participants had a normal pure-tone average, with most having high-frequency hearing loss. Working memory (memory capacity) was assessed with the backward Digit Span test, and speech recognition in noise was assessed with three clinical tests (Quick Speech in Noise, Hearing in Noise Test, and Revised Speech Perception in Noise). Results Findings from this sample of older adults indicate that neither music training nor working memory was associated with differences on the speech recognition in noise measures used in this study. Similarly, duration of music training was not associated with speech-in-noise recognition. Conclusions Results from this study do not support the hypothesis that lifelong music training benefits speech recognition in noise. Similarly, an effect of working memory (memory capacity) was not apparent. While these findings may be related to the relatively small sample size, results across previous studies that investigated these effects have also been mixed. Prospective randomized music training studies may be able to better control for variability in outcomes associated with pre-existing and music training factors, as well as to examine the differential impact of music training and working memory for speech-in-noise recognition in older adults.

PurposeWe sought to determine whether speech perception and language skills measured early after cochlear implantation in children who are deaf, and early postimplant growth in speech perception and language skills, predict long-term speech perception, language, and neurocognitive outcomes.MethodThirty-six long-term users of cochlear implants, implanted at an average age of 3.4 years, completed measures of speech perception, language, and executive functioning an average of 14.4 years postimplantation. Speech perception and language skills measured in the 1st and 2nd years postimplantation and open-set word recognition measured in the 3rd and 4th years postimplantation were obtained from a research database in order to assess predictive relations with long-term outcomes.ResultsSpeech perception and language skills at 6 and 18 months postimplantation were correlated with long-term outcomes for language, verbal working memory, and parent-reported executive functioning. Open-set word recognition was correlated with early speech perception and language skills and long-term speech perception and language outcomes. Hierarchical regressions showed that early speech perception and language skills at 6 months postimplantation and growth in these skills from 6 to 18 months both accounted for substantial variance in long-term outcomes for language and verbal working memory that was not explained by conventional demographic and hearing factors.ConclusionSpeech perception and language skills measured very early postimplantation, and early postimplant growth in speech perception and language, may be clinically relevant markers of long-term language and neurocognitive outcomes in users of cochlear implants.Supplemental materialshttps://doi.org/10.23641/asha.5216200

Background: Previous studies investigating speech recognition in adverse listening conditions have found extensive variability among individual listeners. However, little is currently known about the core underlying factors that influence speech recognition abilities. Purpose: To investigate sensory, perceptual, and neurocognitive differences between good and poor listeners on the Perceptually Robust English Sentence Test Open-set (PRESTO), a new high-variability sentence recognition test under adverse listening conditions. Research Design: Participants who fell in the upper quartile (HiPRESTO listeners) or lower quartile (LoPRESTO listeners) on key word recognition on sentences from PRESTO in multitalker babble completed a battery of behavioral tasks and self-report questionnaires designed to investigate real-world hearing difficulties, indexical processing skills, and neurocognitive abilities. Study Sample: Young, normal-hearing adults (N = 40) from the Indiana University community participated in the current study. Data Collection and Analysis: Participants' assessment of their own real-world hearing difficulties was measured with a self-report questionnaire on situational hearing and hearing health history. Indexical processing skills were assessed using a talker discrimination task, a gender discrimination task, and a forced-choice regional dialect categorization task. Neurocognitive abilities were measured with the Auditory Digit Span Forward (verbal short-term memory) and Digit Span Backward (verbal working memory) tests, the Stroop Color and Word Test (attention/inhibition), the WordFam word familiarity test (vocabulary size), the Behavioral Rating Inventory of Executive Function–Adult Version (BRIEF-A) self-report questionnaire on executive function, and two performance subtests of the Wechsler Abbreviated Scale of Intelligence (WASI) Performance Intelligence Quotient (IQ; nonverbal intelligence). Scores on self-report questionnaires and behavioral tasks were tallied and analyzed by listener group (HiPRESTO and LoPRESTO). Results: The extreme groups did not differ overall on self-reported hearing difficulties in real-world listening environments. However, an item-by-item analysis of questions revealed that LoPRESTO listeners reported significantly greater difficulty understanding speakers in a public place. HiPRESTO listeners were significantly more accurate than LoPRESTO listeners at gender discrimination and regional dialect categorization, but they did not differ on talker discrimination accuracy or response time, or gender discrimination response time. HiPRESTO listeners also had longer forward and backward digit spans, higher word familiarity ratings on the WordFam test, and lower (better) scores for three individual items on the BRIEF-A questionnaire related to cognitive load. The two groups did not differ on the Stroop Color and Word Test or either of the WASI performance IQ subtests. Conclusions: HiPRESTO listeners and LoPRESTO listeners differed in indexical processing abilities, short-term and working memory capacity, vocabulary size, and some domains of executive functioning. These findings suggest that individual differences in the ability to encode and maintain highly detailed episodic information in speech may underlie the variability observed in speech recognition performance in adverse listening conditions using high-variability PRESTO sentences in multitalker babble.

Verbal working memory resources may impact syntax comprehension. Thirteen Italian children with cochlear implants (CIs) were assessed in relative clause (RC) comprehension, digit span and nonword repetition and compared to 13 chronological age peers (CA) and 13 younger controls (LA) with normal hearing (NH). The RC comprehension task tested subject relatives (SR), object relatives with preverbal (OR) and postverbal subjects (ORp) where number features were manipulated. Children with CIs show worse performance than controls in RC comprehension and nonword repetition. In the RC task, number features facilitated comprehension by children with NH, but not by children with CIs. The memory measure that predicted RC comprehension was digit span. In the LA group, backward digit span predicted comprehension of all RC types. Forward digit span predicted comprehension of ORs with number mismatch in the CA group, and comprehension of ORs with number mismatch and ORps in children with CIs. In these conditions, high memory resources are needed to exploit number features in theta-role assignment.

Background: There is a pressing clinical need for the development of ecologically valid and robust assessment measures of speech recognition. Perceptually Robust English Sentence Test Open-set (PRESTO) is a new high-variability sentence recognition test that is sensitive to individual differences and was designed for use with several different clinical populations. PRESTO differs from other sentence recognition tests because the target sentences differ in talker, gender, and regional dialect. Increasing interest in using PRESTO as a clinical test of spoken word recognition dictates the need to establish equivalence across test lists. Purpose: The purpose of this study was to establish list equivalency of PRESTO for clinical use. Research Design: PRESTO sentence lists were presented to three groups of normal-hearing listeners in noise (multitalker babble [MTB] at 0 dB signal-to-noise ratio) or under eight-channel cochlear implant simulation (CI-Sim). Study Sample: Ninety-one young native speakers of English who were undergraduate students from the Indiana University community participated in this study. Data Collection and Analysis: Participants completed a sentence recognition task using different PRESTO sentence lists. They listened to sentences presented over headphones and typed in the words they heard on a computer. Keyword scoring was completed offline. Equivalency for sentence lists was determined based on the list intelligibility (mean keyword accuracy for each list compared with all other lists) and listener consistency (the relation between mean keyword accuracy on each list for each listener). Results: Based on measures of list equivalency and listener consistency, ten PRESTO lists were found to be equivalent in the MTB condition, nine lists were equivalent in the CI-Sim condition, and six PRESTO lists were equivalent in both conditions. Conclusions: PRESTO is a valuable addition to the clinical toolbox for assessing sentence recognition across different populations. Because the test condition influenced the overall intelligibility of lists, researchers and clinicians should take the presentation conditions into consideration when selecting the best PRESTO lists for their research or clinical protocols.

Brain imaging and behavioral studies of working memory (WM) converge to suggest that the ventrolateral prefrontal cortex (PFC) mediates a capacity-limited storage buffer and that the dorsolateral PFC mediates memory organization processes that support supracapacity memory storage. Previous research from our laboratory has shown that the extent to which such memory organization processes are required depends on both task factors (i.e., memory load) and subject factors (i.e., response speed). Task factors exert their effects mainly during WM encoding while subject factors exert their effects mainly during WM retrieval. In this study, we sought to test the generalizability of these phenomena under more difficult memory-demand conditions than have been used previously. During scanning, subjects performed a WM task in which they were required to maintain between 1 and 8 letters over a brief delay. Neural activity was measured during encoding, maintenance, and retrieval task periods using event-related functional magnetic resonance imaging. With increasing memory load, there were reaction time increases and accuracy rate decreases, ventrolateral PFC activation decreases during encoding, and dorsolateral PFC activation increases during maintenance and retrieval. These results suggest that the ventrolateral PFC mediates WM storage and that the dorso-lateral PFC mediates strategic memory organization processes that facilitate supracapacity WM storage. Additionally, high-performing subjects showed overall less activation than low-performing subjects, but activation increases with increasing memory load in the lateral PFC during maintenance and retrieval. Low-performing subjects showed overall more activation than high-performing subjects, but minimal activation increases in the dorsolateral PFC with increasing memory load. These results suggest that individual differences in both neural efficiency and cognitive strategy underlie individual differences in the quality of subjects' WM performance.
