Introduction. Malnutrition is a common problem in patients with chronic renal failure (CRF) with Diabetes Mellitus and Non-Diabetes Mellitus. Assessments of nutritional status in CRF patients are required to measure some important parameters including serum albumin and hemoglobin. This study aimed to fi gure out the differences of nutritional status between CRF patients with diabetes mellitus and without diabetes mellitus in the inpatient unit of PKU Muhammadiyah Hospital Yogyakarta, by examining the levels of albumin and hemoglobin serum. Methods. This study applied cross sectional method of which measuring instruments were patients’ medical records of laboratory tests. This research used accidental sampling method involving 30 patients as samples. Result. The results of independent t-test showed that there was no signifi cant differences between the levels of albumin (p = 0.917) and hemoglobin (p = 0.168) between the group of non DM CRF patients and CRF patients with  M. Discussion. Therefore, further research should be performed by using a larger sample size as well as considering patients’ historical background of hemodialysis treatment.Keywords: Diabetes Mellitus (DM), Chronic Renal Failure (CRF), albumin, hemoglobin

An assessment of nutritional status in CRF patients is encouraged to see the level of its severity. The general parameters for assessing nutritional status are serum albumin and hemoglobin. Objective:The objective of this research was to investigate the differences on thenutritional status of CRF patients with Diabetes Mellitus (DM) compared to those without DM at PKU Muhammadiyah Hospital Yogyakarta. Methods:This was a comparative descriptive research applying accidental sampling and was conducted for 6 months (June-November 2014). The main measuring devices were patients’ medical records and laboratory results of albumin and hemoglobin levels. Sampleof this study consisted of 30 respondents. Independent t-test was applied to analyze the differences of albumin and hemoglobin levels in both groups. Result:The test result showed that the albumin (p=0.917) and hemoglobin levels (p=0.168) of CRF patients without DM and that of CRF patients with DM were not significantly different. Conclusion:CRF patients with and without DM had low level of albumin and hemoglobin. Nurses shouldbe more careful in regulating fluidbalance to reduce pulmonary edema. ABSTRACT Homocysteine (Hcy) is methylated by methionine synthase to form methionine with methyl-cobalamin as a cofactor. The reaction demethylates 5-methyltetrahydrofolate to tetrahydrofolate, which is required for DNA and RNA synthesis. Deficiency of either of the cobalamin (Cbl) and/or folate cofactors results in elevated Hcy and megaloblastic anemia. Elevated Hcy is a sensitive biomarker of Cbl and/or folate status and more specific than serum vitamin assays. Elevated Hcy normalizes when the correct vitamin is given. Elevated Hcy is associated with alcohol use disorder and drugs that target folate or Cbl metabolism, and is a risk factor for thrombotic vascular disease. Elevated methionine and cystathionine are associated with liver disease. Elevated Hcy, cystathionine, and cysteine, but not methionine, are common in patients with chronic renal failure. Higher cysteine predicts obesity and future weight gain. Serum S-adenosylhomocysteine (AdoHcy) is elevated in Cbl deficiency and chronic renal failure. Drugs that require methylation for catabolism may deplete liver S-adenosylmethionine and raise AdoHcy and Hcy. Deficiency of Cbl or folate or perturbations of their metabolism cause major changes in sulfur amino acids.
