Safety and efficacy of herbal supplements used for weight loss

The dangers of herbal supplements were highlighted when a number of young women attending a weight loss clinic in Brussels, Belgium developed kidney failure. It turned out that the herb Aristolochia had been mistakenly substituted for another benign herb. Scientists linked the type of kidney damage found in the women in Brussels to a long-standing mysterious disease in the Balkans, referred to as Balkan Endemic Nephropathy. Studies using molecular techniques have shown that the cause of both conditions is a compound in the Aristolochia plant, which can cause kidney damage as well as a rare cancer of the upper urinary tract. This research underscores the dangers of certain herbal supplements.

The safety and efficacy of preoperative weight loss before elective nonbariatric surgery is controversial. We evaluated the effect of planned surgical delay for a preoperative weight loss trial in hernia repairs. Four hundred and fourteen patients undergoing elective hernia repair between July 2008 and May 2012 at a Level 1B VA Medical Center were identified. Included patients were divided into two groups: those who underwent immediate hernia repair (nontrial) and those who underwent weight loss trial before hernia repair (TRIAL). Twenty-two patients were categorized in the TRIAL group, and 392 in nontrial. Time from surgical evaluation to operation was longer in the TRIAL vs nontrial group (226 days vs 113 days, P = 0.001). Outcome measures were similar between groups. Net change in body mass index (BMI) was -2.2 per cent in TRIAL vs -0.86 per cent in nontrial patients (P = 0.440). Of the TRIAL patients, ten obtained a poor result (<3% decreased BMI), nine a moderate result (3–10% decreased BMI), and three a good result (>10% decreased BMI). Weight loss trials in elective hernia patients appear to be safe, although they result in significant delay to surgery and confer no difference in postoperative outcomes. Thus, efficacy of preoperative weight loss trials may be limited.

Obesity is an epidemic with high burden of disease. It is directly proportional to increased risk of morbidity and mortality. Ketogenic diet and herbal supplements have recently gained popularity amongst patients struggling with weight loss. There are limited data available for most of these supplements contrary to the claims laid by the mainstream media. Due to lack of awareness, this patient population is at high risk of adverse effects. We present a case of severe acidosis secondary to ketogenic diet and acetic acid (vinegar) ingestion. The use of dietary acetic acid is usually well tolerated; however, in this case, the large quantities and presence of acute renal injury and metformin use may have worsened the acidosis. Severe ketosis in setting of ketogenic diets is a serious complication, which is infrequently reported in literature. Many of these diets and supplements may seem harmless, but as our case illustrates, when combined with other risk factors, patients can face serious adverse effects and even require hospitalization. It is imperative that such dietary practices are physician supervised to avoid complications. With the recent surge of over-the-counter weight loss supplements and ketogenic diets, physicians also need to engage in dietary discussion with patients when attempting to lose weight.

AbstractA 16-year-old male was admitted to the paediatric ICU with acute onset of vomiting, somnolence, and chest pain, and electrocardiogram showing 2nd degree heart block after ingesting an Aleurites moluccana (Candlenut) seed as a herbal weight loss supplement. Electrocardiogram showed progressively worsening heart block with down-sloping of the ST segments, resembling digoxin toxicity. After 2 days of ICU observation, his symptoms began to improve and eventually resolved. The side effects of herbal supplements are often unknown but by analysing cases such as these, physicians can develop a better understanding of these substances to help guide management.

Several canine weight loss protocols were evaluated to determine their relative safety and efficacy. Dogs were fed 100%, 75%, 60%, or 50% of maintenance energy requirements (MERs) using the dogs' target body weights. No indications of adverse health effects were observed with any weight loss protocol. Triiodothyronine (T3) levels and apparent MERs decreased in dogs restricted to 50% to 60% of their MERs. The rate of weight loss was correlated linearly with degree of calorie restriction, although there was considerable individual variation. Percent overweight by the end of the test was not different between protocol groups for dogs fed 50%, 60%, or 75% of MERs. Therefore, any of the protocols tested in this study may be used in the management of overweight dogs; however, individual responses will be expected to vary, and severe calorie restriction may predispose dogs to weight rebound.
