Background Online customer feedback has become routine in many industries, but it has yet to be harnessed for service improvement in health care. Objectives To identify the current evidence on online patient feedback; to identify public and health professional attitudes and behaviour in relation to online patient feedback; to explore the experiences of patients in providing online feedback to the NHS; and to examine the practices and processes of online patient feedback within NHS trusts. Design A multimethod programme of five studies: (1) evidence synthesis and stakeholder consultation; (2) questionnaire survey of the public; (3) qualitative study of patients’ and carers’ experiences of creating and using online comment; (4) questionnaire surveys and a focus group of health-care professionals; and (5) ethnographic organisational case studies with four NHS secondary care provider organisations. Setting The UK. Methods We searched bibliographic databases and conducted hand-searches to January 2018. Synthesis was guided by themes arising from consultation with 15 stakeholders. We conducted a face-to-face survey of a representative sample of the UK population (n = 2036) and 37 purposively sampled qualitative semistructured interviews with people with experience of online feedback. We conducted online surveys of 1001 quota-sampled doctors and 749 nurses or midwives, and a focus group with five allied health professionals. We conducted ethnographic case studies at four NHS trusts, with a researcher spending 6–10 weeks at each site. Results Many people (42% of internet users in the general population) read online feedback from other patients. Fewer people (8%) write online feedback, but when they do one of their main reasons is to give praise. Most online feedback is positive in its tone and people describe caring about the NHS and wanting to help it (‘caring for care’). They also want their feedback to elicit a response as part of a conversation. Many professionals, especially doctors, are cautious about online feedback, believing it to be mainly critical and unrepresentative, and rarely encourage it. From a NHS trust perspective, online patient feedback is creating new forms of response-ability (organisations needing the infrastructure to address multiple channels and increasing amounts of online feedback) and responsivity (ensuring responses are swift and publicly visible). Limitations This work provides only a cross-sectional snapshot of a fast-emerging phenomenon. Questionnaire surveys can be limited by response bias. The quota sample of doctors and volunteer sample of nurses may not be representative. The ethnographic work was limited in its interrogation of differences between sites. Conclusions Providing and using online feedback are becoming more common for patients who are often motivated to give praise and to help the NHS improve, but health organisations and professionals are cautious and not fully prepared to use online feedback for service improvement. We identified several disconnections between patient motivations and staff and organisational perspectives, which will need to be resolved if NHS services are to engage with this source of constructive criticism and commentary from patients. Future work Intervention studies could measure online feedback as an intervention for service improvement and longitudinal studies could examine use over time, including unanticipated consequences. Content analyses could look for new knowledge on specific tests or treatments. Methodological work is needed to identify the best approaches to analysing feedback. Case studies in the Netherlands and the UK of asylum applicants excluded or under consideration of exclusion pursuant to Article 1Fa of the Refugee Convention reveal that some applicants falsely implicated themselves in serious crimes or behaviours in order to enhance their refugee claim. This may have serious consequences for the excluded persons themselves, as well as for national governments dealing with them. For this reason we suggest immigration authorities could consider forewarning asylum applicants i.e. before their interview, about the existence, purpose and possible consequences of exclusion on the basis of Article 1F.

This chapter points to the ‘pluralization of the lifeworld’ involved in globalization as a key context for changing dissenting spiritualities through the twentieth century. These have included a remarkable upsurge in Spirit-movements that fall under categories such as Pentecostal, charismatic, neo-charismatic, ‘renewalist’, and indigenous Churches. Spirit language is not only adaptive to globalized settings, but brings with it eschatological assumptions. New spiritualities emerge to disrupt existing assumptions with prophetic and often critical voices that condemn aspects of the existing culture, state, and church life. This chapter outlines this process of disruption of the mainstream in case studies drawn from the USA, the UK, India, Africa, and Indonesia, where charismaticized Christianity has emerged and grown strongly in often quite resistant broader cultures.

Case studies of corporate providers of long-term care in the UK demonstrate that their mode of organization has important implications for both user choice and the organization of care work. Based upon a series of focus group discussions carried out in East Yorkshire, this article contributes to debates on both the nature and theorising of heterosexual relationships that have recently been investigated from diverse perspectives. These group discussions represent the launch of the first major empirical study of heterosexuality and ageing that has been undertaken in the UK. In drawing upon preliminary data from these focus groups, our findings reinforce and add to the challenging of a representation of heterosexuality which is both monolithic and inflexible, by exploring accounts of peoples’ actual lived experiences. Through this research we begin to generate a theoretical approach which highlights the complexity of these lived realities. We particularly explore the intersections of gender, age, class and family location. In doing so, we pinpoint differences, contradictions, but also continuities, in the ways in which people discuss and comment on their own and other people's perceptions and experiences of heterosexuality.

<p>This paper aimed to investigate the up-to-date levels of sustainability in the UK construction with special interest to sustainable housing. It also aims to examine the justification behind construction and housing sustainability, and to look at the practicality of transferring current expertise within the UK as well as to an emerging Central Asian country such as Kazakhstan. A synergy of case studies, survey and numerical simulation research methodologies were applied to undertake a wide-spectrum analysis of the topic. Regardless of difficulties related to applying sustainable practices the considered housing developments are found to be satisfactory in terms of environmental and socio-economic effects. Technical evaluation of the case studies compared to standard housing parameters has shown encouraging outcomes and confirmed the claimed energy and water efficiency. Findings of the survey indicated that construction companies of the UK and Kazakhstan are at different stages of application of sustainability measures. It was also established that companies and public are mainly optimistic about sustainability if suitable economical and legal conditions are ensured. Finally, numerical simulations have shown that selected sustainability measures made the studied housing projects competitive on the sustainability market. </p>

This paper proposes that there is a need to push beyond the popular discourses of ‘flexibility’ and ‘work-life balance’. Developing a feminist-Bourdieuian approach and drawing on three illustrative case studies from my interview research with 27 mothers in the UK, I show the importance of maintaining a focus on class and gender inequalities. In the first part of the paper the concepts of capitals, dependencies and habitus which shaped, and were shaped by, this interview research are discussed. An analysis of three women's accounts of their experiences across work and family life is then used to illustrate that although these women all used terms such as ‘flexibility’ and ‘juggling’ in describing their work, the experience of that work was crucially influenced by their histories and current positioning. Tracing each of these women's trajectories from school, attention is focused on the influence of differential access to capitals and relations of dependency in the emergence of their dispositions toward work. Overall, the paper points to the significance of examining the classed and gendered dimensions of women's experiences of employment and motherhood.
